A case of erythrokeratoderma variabilis: loosened gap junctions in the acanthotic epidermis.

A 15-year-old Japanese female without contributory personal or family medical history had demonstrated irregular, keratotic plaques in the lower extremities since infancy that had been gradually enlarging. The keratotic plaques showed partial erythematous change, which altered shape over a relatively short period, leaving pigmentation. The biopsy specimen taken from the erythematous, keratotic plaque showed typical church-spire-like papillomatosis with acanthosis, and thickening of granular and horny layers. Gene analysis targeting connexin 30.3 and 31, based on the diagnosis of erythrokeratoderma variabilis, did not demonstrate any abnormality of these genes. However, ultrastructural observation disclosed an increased amount of gap junctions, some of which showed four layers on high-powered electron microscopy, suggesting loosened connection of the plasma membrane of the keratinocytes through the gap junctions. This loosened gap junction structure was also observed in a case of lamellar ichthyosis, examined as a reference. The disturbed cell-to-cell interaction through latent damage to the gap junctions may be related to the keratotic changes of the epidermis in these skin diseases.

Creeping hair: an isolated hair burrowing in the uppermost dermis resembling larva migrans.

A 55-year-old Japanese male presented with a slowly moving linear erythema that looked like an eruption of creeping disease, or cutaneous larva migrans. The eruption extended linearly along Langer's line of the lateral side of the abdomen to the lower back, leaving wave-like erythema. In the top third of the erythematous eruption, close examination demonstrated a black thin line, which was revealed to be a hair shaft by a shallow incision of the skin. After removal of the hair, the eruption diminished immediately, leaving a slight pigmentation. An ingrown pubic hair seemed to have migrated with the lower end forward along Langer's line, because of the arrangement of hair cuticle and the force of body motion. Linearly moving erythematous eruptions that look like that of larva migrans should be differentiated from creeping hair by close examination detecting burrowing hair.

Pyloric gland adenoma arising in Barrett's esophagus with mucin immunohistochemical and molecular cytogenetic evaluation.

Pyloric gland adenoma is a recently described and very rare entity. The occurrence of adenoma is very unusual in Barrett's epithelium of the esophagus. We report a case of esophageal polyp showing the features of pyloric gland adenoma, which was surrounded by so-called specialized columnar epithelium. Immunohistochemically, most tumor glands were strongly positive for MUC6, except in the superficial layer. MUC5AC was positive in almost all tumor cells, but MUC2 and CD10 were negative in the tumor. MIB-1-positive proliferating cells were distributed throughout the tumor. Microdissection and comparative genomic hybridization analyses revealed losses on 2p24-25.2, 2q14.1-ter, 5q31.3-32, 6q23-24, 8q23-24.2, 11q22.3-24 and 18q21.1-22. This is the first case of pyloric gland adenoma found to arise in Barrett's epithelium of the esophagus, showing its unstable and precancerous nature.

Prognostic factor analysis for plaque psoriasis.

BACKGROUND: Little is known about the long-term prognostic factors which affect clinical outcomes after the diagnosis of psoriasis. OBJECTIVE: To identify the prognostic factors which predict long-term outcomes after diagnosis. METHODS: Patients' clinical characteristics at diagnosis were associated with long-term prognosis using the logistic regression method for 169 psoriasis patients (median follow-up time 11.6 years). RESULTS: The factors age > or =40 [odds ratio 0.27, 95% confidence interval (CI) 0.10-0.72, p = 0.0091], male (odds ratio 2.67, 95% CI 1.02-7.00,p = 0.0459) and BMI > or =25 (odds ratio 2.74, 95% CI 1.01-7.42, p = 0.0480) had significant effects on long-term prognosis after diagnosis. A simple prognostic index based on these factors was proposed. CONCLUSIONS: Three major prognostic factors were identified. The proposed index may be useful in the design of future clinical trials for psoriasis and in the selection of appropriate therapeutic approaches for individual patients.

Augmentation of ethanol-induced cell damage and activation of nuclear factor-kappa B by annexin IV in cultured cells.

BACKGROUND: We previously reported that the amount of annexin IV expression was increased in culture cells by exposure to ethanol. Here, to investigate the physiologic role of annexin IV, we analyzed ethanol-induced cytotoxicity and nuclear factor (NF)-kappa B activity by using annexin IV-overexpressed cells. METHODS: Annexin IV overexpression was performed by transfection of expression vector, in which annexin IV complementary DNA was ligated, to culture cells (rat glioma C6 cells and human neuroblastoma SH-SY5Y cells). Ethanol-induced cytotoxicity was assayed by measuring the mitochondrial enzyme (dehydrogenase) activity or trypan blue exclusion. NF-kappa B activity was measured by electrophoretic mobility shift assay with a kappa B-oligonucleotide probe. RESULTS: Ethanol-induced cytotoxicity was increased by overexpression of annexin IV in both C6 cells and SH-SY5Y cells. Annexin IV overexpression augmented ethanol-induced NF-kappa B activation. CONCLUSIONS: Ethanol-induced increase in annexin IV expression might amplify ethanol-induced cytotoxicity via NF-kappa B activation.

Ethanol-induced augmentation of annexin IV in cultured cells and the enhancement of cytotoxicity by overexpression of annexin IV by ethanol.

The annexins are a family of highly homologous Ca(2+) and phospholipid binding proteins. The expressive amounts of several annexins have been shown to alter in certain pathological states such as brain ischemia and Alzheimer's disease. It has been demonstrated that ethanol induces cytotoxicity, which results in brain damage. In this study, we examined the relationship between ethanol-induced cytotoxicity and the intrinsic amount of annexins using cell lines (rat glioma C6 cells and human adenocarcinoma A549 cells). A decrease in the mitochondrial enzyme (dehydrogenase) activity, which is widely used to measure cytotoxicity, was observed with a high concentration of ethanol (200 mM or more) after a 24-h exposure in both C6 and A549 cells. Western blot analysis revealed that the amount of annexin IV was augmented in both cells by ethanol, whereas levels of annexins I and V were unchanged. The amount of annexin IV was augmented with increasing concentration of ethanol. The overexpression of annexin IV in C6 cells by transfection with annexin IV-DNA enhanced ethanol-induced cell lesion and was accompanied by NFkappaB activation. Thus, it might be indicated that the amount of annexin IV is selectively augmented and this augmentation facilitates the development of cell lesion by ethanol.

Ethanol-induced augmentation of annexin IV expression in rat C6 glioma and human A549 adenocarcinoma cells.

BACKGROUND: We previously reported that the amount of annexin IV significantly increased in the postmortem brains of alcoholics compared with controls. To investigate further whether ethanol directly affects cellular expression of annexins in a cell, we used cell lines and compared the amounts of annexins between ethanol-exposed and control cells. METHODS: Two kinds of cells (rat glioma C6 cells and human adenocarcinoma A549 cells) were used in the present study. Western blot analysis was performed to quantify expressed amounts of annexins using anti-annexin I, IV, and V antibodies. The mitochondrial enzyme (dehydrogenase) and caspase 3 activities were measured to assess cell damage (apoptosis) by ethanol. RESULTS: Expressive augmentation of annexin IV was shown in both C6 and A549 cells after a 5-hr exposure to 200 mM of ethanol, whereas amounts of annexins I and V were not changed. Both the mitochondrial dehydrogenase and caspase 3 activities were altered under the same conditions in C6 cells, indicating the induction of cell damage (apoptosis), whereas both of these enzyme activities were unchanged in A549 cells under the same conditions. CONCLUSIONS: The amount of annexin IV seemed to increase before the induction of cell damage by ethanol. Annexin IV might be one of the specific markers for the effect of ethanol.
