[Argentine consensus on the treatment of bipolar disorders].

The consensus guidelines of argentine experts in the treatment of bipolar disorders are the result of three days of work of the 10 main local experts under the organization of the Argentine Association of Biological Psychiatry (AAPB). It was adopted a mixed criterion for its preparation: all the recent data of the evidence medicine based published until now were discussed and were balanced with the knowledge acquired from clinical experience of the local experts on the bipolar field. It presents general recommendations and suggested therapeutic sequences for the phase of maintenance, the manic/hypomanic or mixed episode and the depressive episode. These have been divided according to the classification in type I and II; with or without rapid cycling. Since the group of experts identified the delay and miss-diagnoses like the most important barrier for a suitable treatment enclosed a series of recommendations for differential diagnosis of bipolar disorders.

[Interactions of mood stabilizers].

This review on the drug-drug interactions of mood stabilizers is aimed to be practical and clinically relevant. Polypharmacy has became the most frequent approach in the treatment of bipolar disorders, and patients given with a long term treatment are expected to suffer occasionally medical intercurrences, which in turn lead to prescriptions of other drugs. Both factors render a possibility of drug-drug interactions rather frequent. Two actual clinical vignettes will be given, illustrating clinical situations in which drug interactions may often occur. It will be reviewed the main drug-drug interactions reported with lithium, carbamazepine, oxcarbazepine, valproate, lamotrigine, gabapentin and topiramate, and also some pharmacokinetic aspects of mood stabilizers that are important in order to understand the possibility of interactions.

[Psychotic depression].

This paper will bring a brief overview of the clinical aspects of psychotic depression, with emphasis in the differentiation between mood-congruent and mood-incongruent delusions. Biological aspects of this disorder will be reviewed. Changes have been found in serotonergic and dopaminergic neurotransmission and also a characteristic shortening of REM latency. Alterations of hypothalamic-pituitary-adrenal (HPA) axis are especially prominent and may have specific features in this disorder. Attention will be given to structural brain alterations, cognitive impairments and the relationships of both with the HPA alterations previously described. Finally, a treatment update will be provided. Combination of antidepressants and antipsychotics is still the preferred psychopharmacologic treatment. There is recent research in selective serotonin reuptake inhibitors as monotherapy, atypical antipsychotics and antiglucocorticoid strategies but more investigations are needed.
