Spontaneous perforation of the cornea following silicone oil keratopathy.

PURPOSE: To report a sight-threatening complication of silicone oil injection for management of complex retinal detachment. METHODS: Review of case record. RESULTS: Spontaneous perforation of cornea may occur following silicone oil injection into an eye for complex retinal detachments. Risk factors are longer duration of oil in the eye, aphakia, silicone oil in the anterior chamber, and extensive and multiple surgeries. CONCLUSIONS: In high-risk cases, regular assessment of corneal status and removal of silicone oil may help to prevent such a complication.

Case report of tuberculous panophthalmitis.

BACKGROUND: Although tuberculosis is very common in the Indian sub-continent, tuberculous panophthalmitis has not been reported from this region so far. We report a case of a young girl with tuberculous panophthalmitis. CASE REPORT: A 12-year-old female child presented with painless progressive loss of vision in the right eye of two months' duration. Examination revealed diffuse corneal haze with deep vascularization, iris nodules, and scleral necrosis. Histopathological examination of the enucleated eye revealed necrotizing granulomatous inflammation, multiple epitheloid cell granulomas, and Langhan's giant cells, along with large areas of caseous necrosis. Chest X-ray revealed right hilar lymphadenopathy with right lower zone infiltration and a small pleural effusion. Considering the clinical picture, histopathology and chest findings, a diagnosis of disseminated tuberculosis was made, and standard four-drug anti tubercular treatment was started. At 2 months follow up there was radiological resolution of the lung lesions and pleural effusion. CONCLUSIONS: Clinical features suggestive of tubercular panophthalmitis are absence of pain, presence of nodules on or within the eyeball, and spontaneous perforation. We would like to emphasize that in all susceptible individuals presenting with clinical suspicion of ocular tuberculosis it would be prudent to start early anti-tubercular therapy to prevent progression to panophthalmitis. A complete systemic workup to look for any other possible focus of tubercular involvement must be done.

Insect wing conjunctival granuloma.

PURPOSE: To report a case of conjunctival granuloma formation in response to lodgment of an insect wing. METHOD: Case report and review of the literature. RESULTS Granuloma formation in the conjunctiva can occur following lodgment of an insect wing. This can cause the patient chronic redness, discharge, and discomfort. CONCLUSIONS: Small foreign bodies with relatively little mass and large surface area can evade the normal protective mechanisms for removal of a foreign body and give rise to a chronic inflammatory response.

Apoptosis: an ocular perspective.

Apoptosis is a programmed and controlled form of cell death, which is distinct from necrosis. It is a non-inflammatory process and plays an important role in numerous physiological and pathological events. It has an indispensable role in the development and homeostasis in tissues of all higher organisms. Apoptosis can be identified by various methods such as electron microscopy, agarose gel electrophoresis, deoxyribonucleic acid fragmentation analysis, Tdt-dUTP terminal nick end labelling (TUNEL) assay, flow cytometry, annexin V staining and enzyme assays. A number of genes have been identified which are involved in this process. Many approaches for the control of apoptosis are being developed from the understanding of its molecular mechanisms. These include pharmacological inhibition or overexpression of the involved genes, gene therapy by viral transduction of apoptotic inhibitors, inhibition of proteases, inhibition of intracellular rise in calcium concentration and inhibition by scavenging mediators of apoptosis such as reactive oxygen species. In the eye, apoptosis seems to play a role starting from embryogenesis to diseases of all its components. Modulation of apoptosis using different inducers and inhibitors could be of great importance.
