Prospective, noninterventional, uncontrolled, open-chart, pharmacoepidemiologic study of prescribing patterns for lipid-lowering drugs at a tertiary care teaching hospital in North India.

BACKGROUND: The guidelines for management of dyslipidemia released by the US National Cholesterol Education Program (NCEP) have been questioned for their relevance in the South Asian Indian populations because these populations are reported to have significantly different lipoprotein parameters and atherogenic risk factors than Western populations. OBJECTIVE: The aim of this study was to determine current prescribing patterns for lipid-lowering drugs (LLDs) adopted by physicians in North India. METHODS: This prospective, noninterventional, uncontrolled, open-chart, pharmacoepidemiologic study was conducted from June 2000 to August 2000 at a tertiary care hospital in North India and included 200 dyslipidemic patients. The pattern of prescribing LLDs was recorded, along with the serum levels of lipid parameters-total cholesterol (TC), triglycerides (TG), low-density lipoprotein cholesterol (LDL-C), high-density lipoprotein cholesterol (HDL-C), and lipoprotein(a) (Lp[a])-at the time of initiating LLD therapy and compared with the 1993 NCEP-II therapeutic guidelines for dyslipidemia management. RESULTS: The mean (SD) levels of lipid parameters in the study population were as follows: TC, 223.2 (21.5) mg/dL; TG, 258.4 (61.3) mg/dL; LDL-C, 131.6 (26.5) mg/dL; HDL-C, 39.8 (8.9) mg/dL; and Lp(a), 44.8 (26.8) mg/dL. The LLDs prescribed were fibrates (53.5%) and statins (46.5%). Forty percent of patients prescribed LLDs did not meet the NCEP-II criteria for initiation of LLD therapy. CONCLUSIONS: Considerable differences in prescribing patterns of LLDs were observed compared with the then-prevalent NCEP-II guidelines. However, due to the abnormally high serum Lp(a) levels present in the average dyslipidemia profile in South Asian Indian populations, this pattern was in accordance with the specific recommendations made for these populations, as well as with the 2001 NCEP-III guidelines.

Biotechnological applications of cyclodextrins.

Cyclodextrins (CDs) are a family of cyclic oligosaccharides that are composed of alpha-1,4-linked glucopyranose subunits. Cyclodextrins are produced from starch by enzymatic degradation. These macrocyclic carbohydrates with apolar internal cavities can form complexes with and solubilize many normally water-insoluble compounds. This review describes recent applications of CDs in pharmaceuticals with a major emphasis on drug delivery systems. The utility of these water-soluble cyclic glucans in a variety of foods, flavors cosmetics, packaging and textiles is elaborated. The role of these compounds in biocatalysis is also discussed. Cyclodextrins are used in separation science because they have been shown to discriminate between positional isomers, functional groups, homologues and enantiomers. This property makes them a useful agent for a wide variety of separations.

Sound motion evoked magnetic fields.

OBJECTIVE: The aim of present study was to determine which brain regions are involved in the conscious perception of sound motion in humans. METHODS: Six kinds of sound stimuli were studied. Two static sound stimuli with durations of 100 or 1000 ms remained at a fixed position during the stimulation period. Four moving sound stimuli with duration of 100 or 1000 ms were moving from left to right, or right to left, during the stimulation period. Evoked magnetic fields were recorded using a 151-channel whole cortex magnetoencephalographic system. RESULTS: The response identified in all sound stimuli was M100. Responses identified only in moving sound stimuli were M180, M280 and M680. Contour maps and dipoles overlapped on magnetic resonance imaging indicated that both the M100 and M680 responses were generated in the superior temporal cortex (left and right), while M180 and M280 were generated in the parietal cortex (right). CONCLUSIONS: The results of this MEG study indicated that the right parietal cortex was involved in sound motion processing. We hypothesize that the right parietal cortex, in association with the left and right superior temporal cortex, forms a network to process sound motion information.

Botryococcus braunii: a renewable source of hydrocarbons and other chemicals.

Botryococcus braunii, a green colonial microalga, is an unusually rich renewable source of hydrocarbons and other chemicals. Hydrocarbons can constitute up to 75% of the dry mass of B. braunii. This review details the various facets of biotechnology of B. braunii, including its microbiology and physiology; production of hydrocarbons and other compounds by the alga; methods of culture; downstream recovery and processing of algal hydrocarbons; and cloning of the algal genes into other microorganisms. B. braunii converts simple inorganic compounds and sunlight to potential hydrocarbon fuels and feedstocks for the chemical industry. Microorganisms such as B. braunii can, in the long run, reduce our dependence on fossil fuels and because of this B. braunii continues to attract much attention.

Electroencephalographic dipoles of spikes with and without myoclonic jerks caused by epilepsia partialis continua.

We used electroencephalographic (EEG) dipole analysis to investigate the generators of spikes with and without myoclonic jerks in a 12-year-old patient with epilepsia partialis continua secondary to left parietal cortical dysplasia. We recorded EEG and right wrist extensor electromyography (EMG) and collected 42 spikes with jerks (jerking spikes) and 42 spikes without jerks (nonjerking spikes). We applied a single moving dipole model to the individual and averaged spikes. Dipoles at the negative peak of individual jerking and nonjerking spikes were localized in the dysplastic area. At the onset of the averaged jerking spike that preceded the EMG discharges by 20 ms, the dipole was in the motor cortex, whereas for the averaged nonjerking spike, the dipole was in the sensory cortex. The dipole moment at averaged jerking spike onset was twice that of the averaged nonjerking spike. Electroencephalographic dipole analysis of averaged spikes differentiated the generator of jerking and nonjerking spikes in epilepsia partialis continua. Individual dipoles demonstrated the area of epileptogenic cortical dysplasia.

Directed evolution: an approach to engineer enzymes.

Directed evolution is being used increasingly in industrial and academic laboratories to modify and improve commercially important enzymes. Laboratory evolution is thought to make its biggest contribution in explorations of non-natural functions, by allowing us to distinguish the properties nurtured by evolution. In this review we report the significant advances achieved with respect to the methods of biocatalyst improvement and some critical properties and applications of the modified enzymes. The application of directed evolution has been elaborately demonstrated for protein solubility, stability and catalytic efficiency. Modification of certain enzymes for their application in enantioselective catalysis has also been elucidated. By providing a simple and reliable route to enzyme improvement, directed evolution has emerged as a key technology for enzyme engineering and biocatalysis.
