Wound healing effect of silk fibroin/alginate-blended sponge in full thickness skin defect of rat.

Silk fibroin (SF) and alginate (AA) have been proved to be invaluable natural materials in the field of biomedical engineering. This study was designed to compare the wound healing effect of SF, AA and SF/AA-blended sponge (SF/AA) with clinically used Nu Gauze(TM) (CONT) in a rat full thickness wound model. Two circular skin wounds on the back of rat were covered with either of CONT, SF, AA or SF/AA. On the postoperative days of 3, 7, 10 and 14, residual wound area was calculated, and skin wound tissues were biopsied to measure the area of regenerated epithelium and collagen deposition as well as the number of proliferating cell nuclear antigen (PCNA)-immunoreactive cells. Half healing time (HT(50)) of SF/AA was dramatically reduced as compared with that of SF, AA or CONT. Furthermore, SF/AA significantly increased the size of re-epithelialization and the number of PCNA positive cells, whereas the effect of SF/AA on collagen deposition was not significantly different as compared with that of SF or AA. These results demonstrate that the wound healing effect of SF/AA is the best among other treatments including SF and AA, and this synergic effect is mediated by re-epithelialization via rapid proliferation of epithelial cell.

Nasal packs with X-ray indicators.

Nasal packing is routinely performed by many surgeons following nasal surgery and is also frequently used for the treatment of epistaxis. The use of nasal packing, however, is sometimes associated with serious complications such as aspiration, bowel perforation, obstructive apnea, and hypoxia. This author has also once had the experience of losing a nasal pack during a procedure performed on a mentally impaired patient. If a nasal pack could be easily tracked by x-ray, treatment would be faster and easier. We now use x-ray indicators quilted into our nasal packs, which enable us to find the packs quickly and easily with plain x-ray film.

Periciliary Y-V epicanthoplasty.

The epicanthal fold is a semilunar flap of eyelid skin on the medial aspect of the eye. Many surgical techniques have been described to eliminate the epicanthal fold, which is a feature uniquely predominant among the Asian population. However, many surgeons avoid epicanthoplasty because of the fear of visible scar formation. From January of 2002 to April of 2004, 118 patients with medial epicanthal folds underwent epicanthoplasty using a periciliary Y-V advancement procedure for the prevention of visible scar formation. The mean age of the patients was 25.2 years (range, 16 years to 56 years). The median follow-up period was 6 months (range, 2 weeks to 2 years). We performed periciliary Y-V epicanthoplasty only when a patient had wide interepicanthal distance (over 32 mm) or a completely eclipsed lacrimal caruncle. Most of the patients attained satisfactory results, and there were few complications in our cases. Scarring of the medial canthal area has not been a problem with this technique because we designed incisions along the eyelashes and skin-mucosal junctions. We have found that our periciliary Y-V epicanthoplasty is a very reliable and effective method for eliminating the epicanthal fold with minimal scar formation.

Topical application of epidermal growth factor accelerates wound healing by myofibroblast proliferation and collagen synthesis in rat.

Recombinant human epidermal growth factor (rhEGF) stimulates the proliferation and migration of epithelial cells in human cell culture systems and animal models of partial-thickness skin wounds. This study investigated the effect of a topical rhEGF ointment on the rate of wound healing and skin re-epithelialization in a rat full thickness wound model, and verified whether or not the rhEGF treatment affected both myofibroblast proliferation and collagen synthesis in the dermis. When rhEGF (10 microgram/g ointment) was applied topically twice a day for 14 days, there was significantly enhanced wound closure from the 5th to the 12th day compared with the control (ointment base treatment) group. A histological examination at the postoperative 7th day revealed that the rhEGF treatment increased the number of proliferating nuclear antigen immunoreactive cells in the epidermis layer. In addition, the immunoreactive area of alpha-smooth muscle actin and the expression of prolyl 4-hydroxylase were significantly higher than those of the control group. Overall, a topical treatment of rhEGF ointment promotes wound healing by increasing the rate of epidermal proliferation and accelerating the level of wound contraction related to myofibroblast proliferation and collagen deposition.

Condylar positioning in sagittal split ramus osteotomy by the triangle method.

BACKGROUND: Several methods for maintaining the preoperative condyle position after mandibular ramus surgery have been suggested. But when applied, those have been proven to be uncomfortable and less practical; hence, maintaining preoperative condyle position has mainly relied on the operator's personal experience. PATIENTS AND METHOD: We developed a new technique on the basis of the triangle's principle (namely the triangle method) for positioning of the condyle and applied it to 72 patients with sagittal split ramus osteotomy. We followed them for an average of 24.4 months. RESULTS: None of the patients who were operated on using the triangle method had postoperative temporomandibular joint problems or relapses related to inappropriate temporomandibular joint position. CONCLUSION: The triangle method for condylar positioning in the mandibular ramus osteotomy is a simple, reliable, cheap, and easy-to-learn tool for condylar positioning.

Overgrowth of a costochondral graft in nasal reconstruction.

Nasal reconstruction with autogenous costochondral graft has been one of the surgical managements in craniofacial clefts. We have recently seen one patient who underwent this operation and the graft had overgrown like as in Pinocchio's nose-overly projecting nasal tip for almost 7 years. This case supports the notion that the costochondral graft itself carries intrinsic factors and the growth of the graft depends mainly on the costochondral graft itself. Therefore we acknowledge "the primary growth center theory" rather than "the functional matrix theory".

Nonincisional blepharoplasty using the debulking method.

As more women work outside home, a growing number of women who want to have a natural-looking double eyelid choose the method with a short healing time and less pain. Thus blepharoplasty by the nonincisional method is performed frequently. This technique uses 7-0 nylon for simple triple ring-shaped or center doubled triple ring-shaped suture, with removal of subcutaneous fat, pretarsal muscle, and orbital septum through small incisions. This prevents the possibility that the folds will be loosened. By creation of a scar adhesion between the wider portion of the dermis and the tarsal plate after debulking of the soft tissue, the nonincisional method can be applied to a very thin eyelid without any difficulty.A retrospective review of the data for 327 patients who underwent nonincisional blepharoplasty using the debulking method from November 24, 2000 through March 9, 2003 is presented. Except for four cases, the procedures were successful. Two complications of conjunctivitis occurred, requiring removal of the buried suture, and two patients reported a mild scar.
