The relation of amino-terminal propeptide of type III procollagen and severity of coronary artery disease in patients without myocardial infarction or hibernation.

OBJECTIVES: The association of cardiac fibrosis and coronary artery disease (CAD) in patients without infarction or hibernation is unclear. We investigated the relationship between serum concentrations of procollagen propeptides and severity of CAD in such patients. DESIGN AND METHODS: Forty-six patients (32 men; mean age 64 years) with chest pain were enrolled. All patients received stress thallium-201 single photon emission computed tomography (SPECT) and analysis of the serum levels of the amino-terminal propeptide of type I and III procollagen (PINP and PIIINP). RESULTS: In patients with thallium-201 perfusion defects, the number of diseased vessels was associated significantly with PIIINP (p=0.024) rather than PINP (p=0.613). Follow-up serum PINP and PIIINP levels after coronary intervention (mean 84 days) revealed no significant decrease. CONCLUSIONS: Serum PIIINP level is significantly associated with the severity of CAD in patients without myocardial infarction or hibernation.

Network modulation by the subthalamic nucleus in the treatment of Parkinson's disease.

Deep brain stimulation of the subthalamic nucleus (STN DBS) has become an accepted tool for the treatment of Parkinson's disease (PD). Although the precise mechanism of action of this intervention is unknown, its effectiveness has been attributed to the modulation of pathological network activity. We examined this notion using positron emission tomography (PET) to quantify stimulation-induced changes in the expression of a PD-related covariance pattern (PDRP) of regional metabolism. These metabolic changes were also compared with those observed in a similar cohort of patients undergoing STN lesioning. We found that PDRP activity declined significantly (P < 0.02) with STN stimulation. The degree of network modulation with DBS did not differ from that measured following lesioning (P = 0.58). Statistical parametric mapping (SPM) revealed that metabolic reductions in the internal globus pallidus (GPi) and caudal midbrain were common to both STN interventions (P < 0.01), although declines in GPi were more pronounced with lesion. By contrast, elevations in posterior parietal metabolism were common to the two procedures, albeit more pronounced with stimulation. These findings indicate that suppression of abnormal network activity is a feature of both STN stimulation and lesioning. Nonetheless, these two interventions may differ metabolically at a regional level.

Usefulness of progressive inhomogeneity of myocardial perfusion and chronotropic incompetence in detecting cardiac allograft vasculopathy: evaluation with dobutamine thallium-201 myocardial SPECT.

BACKGROUND: The aim of the study was to investigate the value of longitudinal follow-up of dobutamine thallium-201 single photon emission tomography ((201)Tl SPECT) in the development of significant cardiac allograft vasculopathy (CAV) after orthotopic heart transplantation. METHODS: We studied 38 cardiac recipients (mean age 57 +/- 12 years) who underwent at least two follow-up dobutamine (201)Tl SPECT since January 1998. All patients had normal coronary angiography and normal left ventricular function initially. RESULTS: After 2.3 +/- 1.8 years, 12 patients developed significant CAV and there were 4 cardiac deaths (1 died suddenly). Of the 99 scans retrospectively analyzed, patients with significant CAV had elevated values of inhomogeneity score, lung/heart ratio (LHR) at stress and lower left ventricular ejection fraction (all p < 0.05). The higher values of inhomogeneity were significantly correlated with higher stress LHR (r = 0.301, p = 0.021), and lower ejection fraction (r = -0.379, p < 0.001). Progressive inhomogeneity was noted in all heart recipients, and more rapid, although statistically insignificant, in patients who developed significant CAV. Ten patients had inadequate chronotropic response to dobutamine infusion up to 40 mug/kg/min in the follow-up studies. The late onset of chronotropic incompetence was an independent predictor of CAV development (p = 0.03). CONCLUSIONS: Progressive inhomogeneity of myocardial perfusion, higher lung uptake at stress and chronotropic incompetence assessed by dobutamine (201)Tl myocardial SPECT provide incremental diagnostic value in detecting significant CAV.

Early restaging whole-body (18)F-FDG PET during induction chemotherapy predicts clinical outcome in patients with locoregionally advanced nasopharyngeal carcinoma.

PURPOSE: This study was undertaken to evaluate the utility of whole-body (18)F-FDG PET in monitoring therapeutic effect during induction chemotherapy (IC) and in predicting prognosis in patients with locoregionally advanced nasopharyngeal carcinoma (NPC). METHODS: Fifty patients who had histologically proven, locoregionally advanced NPC without distant metastasis and had received IC were recruited in this study. The study cohort consisted of 19 females and 31 males (age 17-72 years, mean 45.9+/-11.9). Whole-body (18)F-FDG PET was performed in each patient after completion of one (33 patients) or two (17 patients) courses of IC. Each patient was restaged on the basis of the (18)F-FDG PET results. Patients who were downstaged to stage I or II were classified as major responders; the rest were classified as non-major responders. RESULTS: Only 1 of the 23 major responders subsequently developed local recurrence. At the time of data analysis, all major responders were alive; by contrast, of the 27 non-major responders, 15 had locoregional recurrence or distant metastasis and nine had died (seven of NPC and two of treatment-related complications). Kaplan-Meier survival analysis showed significantly longer recurrence-free survival and overall survival in major responders (56.4+/-9.2 and 58.1+/-2.2 months) as compared with non-major responders (33.7+/-23.2 and 44.7+/-20.0 months), with p<0.0001 and p=0.0024, respectively. CONCLUSION: The results of this study suggest that early restaging by a single whole-body (18)F-FDG PET scan after the first or second course of IC is useful for predicting therapeutic response and outcome in patients with locoregionally advanced NPC.

Diagnostic and prognostic value of dobutamine thallium-201 single-photon emission computed tomography after heart transplantation.

BACKGROUND: Cardiac allograft vasculopathy (CAV) is a major cause of mortality in heart transplant recipients. We investigated the diagnostic and prognostic value of dobutamine thallium-201 ((201)Tl) single-photon emission computed tomography (SPECT) in heart transplant recipients. METHODS: We studied 47 patients (age 51.6 +/- 11.7 years, 37 men), at a mean of 34.0 +/- 21.4 months after heart transplant, who received dobutamine (201)Tl SPECT, echocardiography and coronary angiography within 1 month of each other. SPECT was considered abnormal in the presence of reversible or fixed defects in >/=2 segments. Significant CAV was defined as >/=50% luminal stenosis. RESULTS: Coronary angiograms were normal in 37 patients. Non-significant CAV was detected in 1 patient and significant CAV in 9 patients. The sensitivity, specificity, positive predictive value and negative predictive value of SPECT for the detection of significant angiographic CAV were 89%, 71%, 42% and 96%, respectively. Large reversible perfusion defects (>/=6 segments) always indicated significant CAV. In patients with normal left ventricular function, a lung/heart ratio (LHR) of >/=0.37 during stress was also an independent predictor of significant CAV (odds ratio 15.5, p = 0.04). A higher stress LHR was associated with greater vessel involvement (r = 0.516, p = 0.0002). Patients with impaired left ventricular function also had higher stress and resting LHR. Over 40.3 +/- 21.9 months after the first SPECT, 1 patient developed significant angiographic CAV and another 4 had cardiac death. Large reversible perfusion defect was a significant predictor of cardiac death (p = 0.002). CONCLUSIONS: Dobutamine (201)Tl SPECT is a useful method for detecting patients with significant CAV and assessing prognosis. It is reasonable and safe to design individualized surveillance intensity of coronary angiography for post-transplant patients on the basis of non-invasive monitoring of dobutamine (201)Tl testing.

Whole-body 18F-FDG PET in recurrent or metastatic nasopharyngeal carcinoma.

UNLABELLED: The aim of this retrospective study was to evaluate the sensitivity and prognostic significance of whole-body (18)F-FDG PET for nasopharyngeal carcinoma (NPC) patients for whom there was a suspicion of recurrence or metastasis by conventional radiologic or clinical findings during their follow-up examinations. METHODS: Whole-body (18)F-FDG PET examinations were performed on 64 Taiwanese NPC patients (14 female, 50 male; mean age +/- SD, 45.8 +/- 13.0 y; age range, 16-75 y) 4-70 mo (mean +/- SD, 14.1 +/- 13.5 mo) after radiotherapy or induction chemotherapy followed by concurrent chemoradiotherapy from February 1997 to May 2001. The accuracy of (18)F-FDG PET detection for each patient was determined by the histopathologic results or other clinical evidence. RESULTS: The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of (18)F-FDG PET images in the diagnosis of NPC recurrence or metastases and secondary primary cancers were 92%, 90%, 92%, 90%, and 91%, respectively. Furthermore, the presence of (18)F-FDG hypermetabolism was highly correlated with the survival time of NPC patients. CONCLUSION: Whole-body (18)F-FDG PET is a sensitive follow-up diagnostic tool for the evaluation of NPC recurrences and metastases. It is also an effective prognostic indicator for NPC patients. To determine the optimized utilization of (18)F-FDG PET in the follow-up for NPC patients, further cost-effectiveness analysis of (18)F-FDG PET in combination with conventional management is necessary.

Early detection of splenic metastasis of lung cancer by 18F-2-fluoro-2-deoxyglucose positron emission tomography.

Diagnosis of splenic involvement by lung cancer in the early stage is helpful for introducing proper treatment. We report a case of pulmonary adenocarcinoma with splenic metastasis which was first detected by 18F-2-fluoro-2-deoxyglucose (18F-FDG) positron emission tomography (PET). This 56-year-old male underwent left pneumonectomy, extensive lymph node dissection and postoperative adjuvant chemotherapy and radiotherapy. Because progressive elevation of carcinoembryonic antigen was detected 2 years after the operation, whole-body 18F-FDG PET was performed and revealed an 18F-FDG hypermetabolic spot in the splenic hilum. Although subsequent computed tomography (CT) did not reveal any lesions in the spleen, the second and third CT scans performed 10 and 13 months later confirmed the previous PET finding of splenic metastasis. This case suggests that 18F-FDG PET may be useful for early detection of splenic metastasis not visible on CT.

The relation between myocardial cyclic variation of integrated backscatter and serum concentrations of procollagen propeptides in hypertensive patients.

Cyclic variation of integrated backscatter (IBS), or CVIBS, provides a noninvasive method to measure myocardial collagen deposition and ischemia in hypertensive patients. We hypothesized that serum procollagen propeptides can offer additional values to CVIBS for evaluating cardiac changes related to fibrosis or ischemia. A total of 21 patients were enrolled in this study and were divided into three groups according to the presence of hypertension and serum carboxyterminal propeptide of type I procollagen (PICP) concentration; these were: 7 hypertensive patients with PICP > or = 127 microg/L (group 1), 7 hypertensive patients with PICP < 127 microg/L (group 2), 7 normotensive subjects with PICP < 127 microg/L (group 3). In addition to PICP, serum aminoterminal propeptide of type III procollagen (PIIINP), stress 201thalium scintigraphy and CVIBS were examined. Phase-compensated amplitudes of CVIBS at mid posterior and mid anteroseptal segments were significantly lower in group 1 (p < 0.05). Patients with fixed 201thallium perfusion defects had lower phase-compensated amplitudes of CVIBS at mid anteroseptal segment and higher PIIINP concentrations (p < 0.05). In conclusions, decrease of myocardial phase-compensated amplitude accompanied with increase of serum PICP concentration may be indicative of the underlying fibrotic process of hypertensive myocardium. Decrease of this CVIBS parameter with increase of serum PIIINP implies concomitant myocardial ischemia.

Using 18-fluoro-2-deoxyglucose positron emission tomography in detecting infectious endocarditis/endoarteritis: a preliminary report.

RATIONALE AND OBJECTIVES: We evaluated the effectiveness of positron emission tomography (PET) with 18-fluoro-2-deoxyglucose (FDG) in the detection of infectious endocarditis/endoarteritis. MATERIALS AND METHODS: For this study, we recruited 6 patients (4 women, 2 men; age range, 35 - 78 years; mean age, 55.8 +/- 16.8 years) who were clinically diagnosed as having infective endocarditis/endoarteritis by their echocardiographic findings and by Duke criteria. RESULTS: For all 6 patients, we also found increased FDG uptakes in the corresponding areas detected in echocardiography. CONCLUSION: FDG-PET appears to be a promising tool in diagnosing infective endocarditis/endoarteritis, and further prospective studies on a large scale to fully exploit the usefulness of FDG-PET for infective endocarditis/endoarteritis are needed.

Plasma adiponectin levels and metabolic factors in nondiabetic adolescents.

OBJECTIVES: The relationship of plasma adiponectin levels with various anthropometric and metabolic factors has been surveyed extensively in adults. However, how plasma adiponectin levels are related to various anthropometric indices and cardiovascular risk factors in adolescents is not as vigorously studied. In this study, we investigated this among healthy nondiabetic adolescents. RESEARCH METHODS AND PROCEDURES: Two hundred thirty nondiabetic subjects (125 boys and 105 girls, approximately 10 to 19 years old) were included. The plasma adiponectin, fasting plasma glucose, insulin, lipids and anthropometric indices including body height, weight, waist circumference, and hip circumference were examined. Body fat mass (FM) and percentage were obtained from DXA scan. The homeostasis model assessment was applied to estimate the degree of insulin resistance. RESULTS: The plasma adiponectin levels were significantly higher in girls (30.79 +/- 14.48 micro g/mL) than boys (22.87 +/- 11.41 micro g/mL). The plasma adiponectin levels were negatively related to BMI, FM, FM percentage, waist circumference, waist-to-hip ratio, insulin resistance, plasma insulin, triglycerides, and uric acid levels, but positively with high-density lipoprotein cholesterol (HDL-C) with the adjustment for age and gender. Using different multivariate linear regression models, only age and HDL-C were consistently related to the plasma adiponectin levels after adjustment for the other variables. DISCUSSION: The relationship between plasma adiponectin and various anthropometric indices and metabolic factors, especially HDL-C, previously reported in adults was present in the healthy nondiabetic adolescents. Whether variation of plasma adiponectin levels in healthy nondiabetic adolescents may influence their future coronary artery disease risk warrants further investigation.

Lack of independent relationship between plasma adiponectin, leptin levels and bone density in nondiabetic female adolescents.

OBJECTIVES: Adiponectin has been implicated in the pathophysiology of metabolic syndrome and coronary artery disease in humans. Whether adiponectin is related to bone mineralization remains unclear in adults as well as in adolescents. In this study, we aimed to determine the relationship between plasma adiponectin, leptin concentrations and bone density, including total-body bone mineral density (BMD) and bone mineral content (BMC) in adolescence. PATIENTS AND MEASUREMENTS: We studied 105 nondiabetic female adolescents [mean age 15.4 +/- 1.9 years, and mean body mass index (BMI), 23.1 +/- 4.0 kg/m(2)]. A venous blood sample was taken after 12 h of fasting to measure fasting plasma adiponectin and leptin levels. BMD and BMC of the whole body were measured by dual-energy X-ray absorptiometry. RESULTS: In simple correlation analysis, plasma adiponectin and leptin levels correlated significantly with total-body BMD (r =-0.523 and r = 0.443, P < 0.001, respectively) and BMC (r =-0.471 and r = 0.396, P < 0.001, respectively). However, plasma adiponectin and leptin were related to both BMD and BMC in opposite directions. In multivariate linear regression analyses, only BMI or fat mass (FM) and Tanner stage, but not plasma adiponectin and leptin, were significantly related to BMD and BMC following adjustment for other variables. CONCLUSIONS: The results suggest that plasma adiponectin and leptin concentrations are not related to the total-body BMD and BMC independent of the chronological age, BMI or FM, and Tanner stage in nondiabetic female adolescents, although they were highly correlated in simple correlation analyses. The biologic roles of adiponectin in bone still need further clarification.

18-fluoro-2-deoxyglucose positron emission tomography in detecting residual/recurrent nasopharyngeal carcinomas and comparison with magnetic resonance imaging.

BACKGROUND: It is known that 18-fluoro-2-deoxyglucose positron emission tomography (FDG-PET) is effective in the early detection of residual/recurrent nasopharyngeal carcinomas (NPC). To compare FDG-PET with the conventional magnetic resonance imaging (MRI) for the detection of residual/recurrent NPC, the authors studied 67 follow-up cases of patients with NPC using both FDG-PET and MRI. METHODS: From February 1997 to February 2001, 67 NPC patients (14 women, 53 men; age range, 16-67 years; mean age, 46.6 +/- 12.5 years) were recruited. Both FDG-PET and MRI of the head and neck area for each patient were performed at least 4 months (duration range, 4-70 months; mean, 14 +/- 13.5 months) after radiotherapy or radiotherapy with concurrent chemotherapy. The final diagnosis was confirmed by biopsy or clinical follow-up for at least 6 months. RESULTS: The sensitivity, specificity, accuracy, positive predictive value (PPV), and negative predictive value (NPV) of FDG-PET images were 100%, 93.4%, 95.5%, 87.5%, and 100%, respectively. In contrast, the sensitivity, specificity, accuracy, PPV, and NPV of the MRI scans were 61.9%, 43.5%, 49.3%, 33.3%, and 70.0%, respectively. CONCLUSIONS: The results of the current study suggest that FDG-PET is much more effective than MRI in detecting residual/recurrent NPC.

Using thallium-201 SPECT to detect recurrent or residual nasopharyngeal carcinoma after radiotherapy in patients with indeterminate CT findings.

BACKGROUND: The aim of this study is to evaluate the usefulness of thallium-201 (Tl-201) single-photon emission CT (SPECT) to detect recurrent or residual nasopharyngeal carcinomas (NPC) when CT findings are indeterminate. METHODS: After radiotherapy, 30 NPC patients with indeterminate CT findings were included. CT, Tl-201, and biopsy were performed at least 4 months after radiotherapy and within 1 week of each other. The final results were based on histopathologic findings and clinical follow-up after at least 6 months. RESULTS: The rates for sensitivity, specificity, and accuracy of Tl-201 SPECT in detecting recurrent or residual NPC when there were indeterminate CT findings, were 86.7%, 93.3%, and 90.0%, respectively. CONCLUSIONS: These results suggest that Tl-201 SPECT is effective in detecting recurrent or residual NPC when CT findings are indeterminate.

Treatment of advanced Parkinson's disease by subthalamotomy: one-year results.

We studied effects on parkinsonian features at 6 and 12 months in 12 patients who underwent unilateral ablation of subthalamic nucleus (STN). Microelectrode mapping was used, and a lesion was created in the STN using thermal coagulation and confirmed with magnetic resonance imaging. At 6 months postoperatively, improvements were seen in several areas: 1) Unified Parkinson's Disease Rating Scales II and III (UPDRS II and III) scores, 30% in off period, 38% in on period; 2) Schwab and England Scale (S&E) score, 21%; and 3) on dyskinesia, 85%. Contralateral rigidity, bradykinesia, UPDRS II and III scores, and S&E scores remained improved at 12 months. Daily dosage of levodopa requirement was reduced by 42%. Axial motor features, gait, postural stability, off period tremor, and motor fluctuation improved at 6 and 12 months but showed a decline in benefits at 18 months. Complications include 3 cases of hemiballism, of whom 2 patients recovered spontaneously but 1 died from aspiration pneumonia. One patient had asymptomatic hematoma, and 2 suffered transient postural asymmetries. We conclude that unilateral subthalamotomy results in moderate improvement in all aspects of parkinsonian features, allows reduction in the dose of levodopa required, and ameliorates drug-induced complications throughout 12-month assessments.

Comparison of gallium-67 citrate and technetium-99m tetrofosmin scan to detect Hodgkin's disease.

OBJECTIVE: The purpose of this study was to compare the usefulness of gallium-67 citrate (67Ga) and technetium-99m tetrofosmin (Tc-TF) scan to detect Hodgkin's disease (HD). METHODS: In this study, 24 patients with HD underwent 67Ga and Tc-TF scan before receiving any therapy. RESULTS: 67Ga scan could detect HD in all 24 (100%) patients. Tc-TF scan could detect HD in 22/24 (91.6%) patients, but it was false-negative in one case of infradiaphragmatic HD and in another case with chemotherapy resistance. There was no significant difference in detection sensitivity between 67Ga and Tc-TF scan. CONCLUSIONS: We conclude that Tc-TF scan can not replace conventional 67Ga scan to detect HD.
