Sister Mary Joseph's nodule originating from endometrial carcinoma incidentally detected during surgery for an umbilical hernia: a case report.

INTRODUCTION: Umbilical metastasis (Sister Mary Joseph's nodule) is rare. It is encountered in 1-3% of patients with intra-abdominal and/or pelvic malignancy, with gastric carcinoma being the commonest origin in men and ovarian carcinoma-in women. Only 27 cases of Sister Mary Joseph's nodule originating from endometrial carcinoma have previously been documented in the literature. CASE REPORT: In a 51-year-old woman, a Sister Mary Joseph's nodule coexisting with a large fibroid uterus was incidentally detected during surgery for suspected strangulated umbilical hernia. Subsequent laparotomy confirmed endometrial carcinoma metastasizing to the umbilical region. CONCLUSION: This is the 28th case reported in the literature of Sister Mary Joseph's nodule originating from endometrial carcinoma and the first case of Sister Mary Joseph's nodule originating from endometrial carcinoma incidentally detected during surgery for umbilical hernia. Surgeons should be aware of the possibility of Sister Mary Joseph's nodule coexisting with an umbilical hernia.

On appropriate pathology for photothermal surgery.

BACKGROUND AND OBJECTIVES: The standard hematoxylin and eosin (H&E) stain can yield dramatic underestimates of tissue death when pathology is performed immediately after radiative surgery. Our objectives were to identify an accurate, consistent histology, and to understand under what conditions the problem is manifested. STUDY DESIGN/MATERIALS AND METHODS: Photothermal surgery with highly concentrated sunlight was performed ex vivo on chicken livers, and in vivo on healthy live rats that were sacrificed at periods of 0, 24, 48, and 72 hours. Pathology with both H&E and an alternative NADH-diaphorase (NADH) stain were prepared. RESULTS: H&E pathology performed shortly after surgery errs by close to an order of magnitude in appraising lesion volume, but the shortfall is mitigated when histological examination is postponed for 24 hours. The biochemical signals missed by the H&E technique at short times can be captured by the NADH stain which reveals the cessation of enzymatic activity, albeit at greater expense and complexity. CONCLUSIONS: The significance lies in avoiding false positives in surgery, providing reliable estimates of photothermal surgical efficacy, and highlighting the relative merits of alternative histological practices with photothermal surgery.

An unusual composite endometrial tumor combining papillary serous carcinoma and small cell carcinoma.

This is the first report in the English literature of a composite endometrial tumor composed of papillary serous carcinoma and small cell carcinoma. A 79-year-old woman underwent total abdominal hysterectomy and left salpingo-oophorectomy due to endometrial carcinoma. Grossly, the uterus was enlarged with an irregular and nodular serosal surface, thickened myometrium, and irregular endometrium. Microscopic examination revealed an endometrial carcinoma composed of papillary serous carcinoma and small cell carcinoma. There was a differential immunoreactivity between the two components: the cells of the papillary serous carcinoma were positive for cytokeratin, CA-125, CEA, and HER-2/Neu, whereas these markers were negative in the small cell carcinoma. Various neuroendocrine markers were positive in the small cell carcinoma and negative in the papillary serous carcinoma. Fluorescence in situ hybridization analysis using 4, 8, and 10 centromeric probes revealed hyperploidy (6-8 signals) in the small cell carcinoma cells. Most of the serous carcinoma cells were euploid, with scattered trisomies and tetrasomies of these chromosomes. The patient died of progressive disease 5 months after surgery. We suggest that the small cell carcinoma may have arisen from the endometrial papillary serous carcinoma undergoing tumor progression with neuroendocrine differentiation.

Photothermally induced delayed tissue death.

We report pronounced delayed tissue death in photothermal surgery performed on the livers of live healthy rats with highly concentrated sunlight (ultrabright noncoherent light). Exposure times and power levels were selected to produce immediate necroses of the order of hundreds of cubic millimeters. Pathology reveals that lesion volumes increase by up to a factor of 5 within approximately 24 h after surgery, and then stabilize. Islands of viable cells can persist within damaged tissue, in the immediate vicinity of blood vessels, but also necrose within about 48 h.

Fiberoptic surgery by ultrabright lamp light.

We report the first realization of interstitial surgery by ultrabright lamp light, on the kidneys and livers of live animals. A high-flux optic concentrates lamp emissions into an optical fiber for power delivery inside the body. The trials reveal surgical efficacy comparable to corresponding laser fiber optic treatments, as well as pronounced delayed tissue death.
