[Strategy of surgical treatment of ulcer disease].

Results of treatment of 688 patients who have undergone palliative operations for complicated forms of gastric and duodenal ulcer, and 2,269 patients after radical surgery were analyzed. It is demonstrated that pharmacotherapy is ineffective in 9% patients with uncomplicated ulcer, in 32% patients with previous ulcer bleeding, and in 29% patients after suturing of perforated ulcer. The role of preventive operation is demonstrated: postoperative complications in this group are revealed rarely compared with those operated for complicated forms of the disease.

[Preventive surgical treatment of ulcer disease].

The state of gastric secretion in ulcer disease patients was investigated. Acidity was estimated by a computed analysis of 24 hour acidogastromonitoring with apparatus AG-24 MP "Gastroscan-24". The examination of 739 patients with chronic duodenal ulcers included 139 (18.8%) patients with exacerbation, 306 (41.1%) with bleeding, 185 (25%) after suturing the perforation, 59 (8%) with stenosis + 50 healthy subjects. A correlation was found between the efficiency of antisecretory drugs and "malignancy" of the course of ulcer disease. An aggressive type of the course of ulcer disease was determined in 178 patients with indications for operation, but the operations were not performed, in 102 out of them long-term results were followed up. The long-term results of operative treatment of uncomplicated ulcer were followed up in 181 (59%) patients.

[Clinical and economic assessment of treatment efficacy of uncomplicated duodenal ulcer with different antisecretory agents in hospital].

A study of 197 patients with uncomplicated duodenal ulcer revealed clinical and pH-metric particular features of the action of pariet (rabeprazole) and six secretolytics: cimetidine, ranitidine, ranisan, quamatel, omez. It was established that the main clinical criteria of treatment efficacy is duration of ulcer cicatrisation and percentage of cicatrized ulcers during 21 days, and additional criteria are duration of disappearance of clinical signs and stage of acid production suppression. Possibilities of applying methods of pharmacoeconomic study were examined: analysis of the treatment cost, cost minimization, and cost-efficacy with the definition of the increase of expense efficacy for drug selection. It was shown that the cost of the antisecretory therapy makes up from 0.45 to 9.14% of inpatient treatment. A cost-efficacy analysis showed that it is most expedient to use pariet.

[Rehabilitation of patients after suturing perforated duodenal ulcer].

516 patients with perforated duodenal ulcer were operated, 396 of them underwent palliative operations. Long-term results were studied in 237 patients. Determination of individual sensitivity to H2-blockers of histamine receptors with 24 hour pH-metry demonstrated tendency towards reduction of therapeutic effect time, and in 17-18% cases--resistance development. Individual selection of drugs with diet, obligative antihelicobacter therapy with eradication control permit to reduce ulcer recurrence rate from 65.5 to 4.8%. Success depends on organisation of rehabilitation and prophylactic treatment.

[Long term results of spleen trauma treatment].

269 patients with trauma of the lien were operated on, the long-term follow up results were analysed in 101 of them. 43 patients appeared to be practically healthy, 11 patients have developed various diseases, not linked with the trauma of the lien. 9 patients were operated on because of another surgical diseases. The postsplenectomy syndrome was noted in 17 (16.8%) patients. The signs of immune insufficiency were demonstrated in the operated patients.

[Reinfusion of blood in combined trauma of hollow abdominal organs].

The authors performed 122 blood reinfusions in patients with trauma to the chest and abdomen, in 25 of them with associated injury to the hollow abdominal organs. The mean volume of blood loss was 2225 ml, that of reinfusion--1092 ml. Nine patients died, 6 developed complications. The authors substantiate the tactics of transfusion therapy in massive hemoperitoneum in patients with associated trauma to the hollow abdominal organs.

[Two-stage splenic rupture].

Of the 185 patients with splenic trauma, in 27 (14.6%), a two-step splenic rupture was revealed. Diagnosis of the given pathology is difficult. After the operation, 12 patients developed complications, 4 died.

[Torsion of the greater omentum].

The greater omentum torsion was revealed in 10 of 10143 patients operated upon urgently. The complex of symptoms established by the authors facilitates making the correct diagnosis. It includes a typical pain syndrome, the presence of a mobile tumor-like mass in the abdominal cavity without disturbances in intestinal passage, disagreement between the satisfactory state of the patient and duration of the disease. The operation of choice for the greater omentum torsion is resection of the torsioned lobe. The postoperative period in 9 patients was good. In one patient there was suppuration of the operation wound.
