[Evolution of the methods of peritonization of the vermiform process stump in acute destructive appendicitis complicated by typhlitis].

A comparative assessment of effectiveness of various non-traditional and non-typical variants of appendectomy was made with using non-standard (non-classical) methods of treatment and peritonization of the vermiform stump developed by different authors for destructive forms of acute appendicitis complicated by inflammatory infiltration of the blind gut cupola and typhlitis. An analysis of results of using the proposed non-typical technical methods and techniques as well as an assessment of the authors' technique used in 43 patients suggests that the above variants of peritonization of the vermiform process stump should not be considered as competing. Each of the methods can be successfully used depending on the concrete clinical situations and for the corresponding indications as the most optimal method of choice or as an alternative of the classical variant of appendectomy in patients with acute destructive appendicitis complicated by inflammatory infiltration of the blind gut cupola and typhlitis.

[Complications of the terminal colostomy and their treatment].

The authors describe a comparative analysis of results of surgical treatment of complications after using different variants of end terminal colostomy in 34 patients aged from 23 to 72 years with stenosing carcinoma of the sigmoid (in 19 patients) and dolichosigmoid (in 6 patients) complicated by acute intestinal obstruction, traumatic injuries of the colon (in 5 patients) and nonspecific ulcerative colitis (in 4 patients) complicated by perforation of the intestinal wall and peritonitis. It was found that using a modified variant of formation of end terminal flat colostomy and performing a controlled intra- and postoperative decompression and intraluminal lavage of the colon with the help of an original author's new model of the aspiration-lavage device allows the number of postcolostomy complications to be decreased and the nearest and long-term results of surgical treatment to be improved.

[Surgical tactics, prophylaxis and treatment of complications at injuries of pancreas].

Results of surgical treatment of 93 patients (85 male, 8 female) aged 15 to 63 years with injuries of pancreas (IP) are analyzed. Stab-incised wound was the cause of IP at 59 patients, gunshot wound--at 3, closed abdominal trauma--at 31 patients. Organ-saving operations were the method of choice. Use of octreotid for the complex treatment of IP permitted to reduce the rate of postoperative suppurative inflammations from 57.1 to 29.5%, lethality--from 39.3 to 21.5%. It is revealed that trauma of pancreas at 94.6% patients associated with combined and multiple injuries of abdominal, pelvic and retroperitoneal organs, and characterized by great number of postoperative complications and lethality.

[Acute hemorrhoidal thrombosis: essence of the idea, correctness of the name and unification of terminology. alternative approaches to treatment].

Acute thrombosis of hemorrhoidal nodes (ATHN) was treated in 402 patients aged from 19 through 73 years of age. It was found that the decision for the volume and method of treatment should be determined individually and differentially, paying special attention to the degree of microcirculatory alterations in the walls of cavernous veins. Multi-component conservative therapy is the method of choice for treatment of ATHN of the 1st severity degree. For the II and III degree operation is indicated which should be fulfilled urgently (within 3 days) or in a postponed (within 5-7 days) order. Hemorrhoidectomy by Milligan-Morgan method with the tight suturing of the postoperative wounds and complete restoration of the mucous membrane of the anal canal allowed getting better immediate and long-term results.

[Strategy of surgical treatment for injuries of the pancreas].

Results of surgical treatment of injuries of the pancreas were analyzed in 89 patients aged from 15 years through 63. It was found that these traumas were accompanied by multiple and combined injuries of polyorganic localization, were characterized by very complex diagnosing, great number of postoperative complications and high lethality. The complex application of the present-day diagnostic methods and a differential approach to the selection of the rational strategy, pathogenetically grounded prophylactics of postoperative complication and their adequate operative correction are thought to be the perspective ways to improve the results of treatment.

[Primary radical surgical interventions in acute obstruction of the colon of tumor genesis in middle-aged and aged patients].

Results of surgical treatment in 262 (88.8%) of 295 elderly and old patients with tumor obstruction of the colon are analyzed. Based on comparative assessment of results of radical and palliative operations, the efficacy of radical operations characterized by lower lethality in these patients regardless of cancer location in the colon is demonstrated. It is concluded that expansion of indications for radical operations in elder groups of patients is one of real ways to improve surgical results in obturative obstruction caused by cancer of the colon of any location. In urgent surgical interventions the preference should be given to obstructive variants of radical operations which are substantiated pathogenically and permit to reach better immediate results particularly at the height of acute obturation of the left half of the colon.

[Debatable and unsolved questions of surgical tactics in perforation of the colon].

An analysis of operative treatment of 86 patients from 24 to 85 years of age with perforation of the colon of different genesis was made. The differential approach to decision on the surgical strategy allows the author to make a conclusion that a radical operation such as resection of the injured segment of the colon with the colostomy has the best immediate result independent of the cause of the perforation. Palliative operations of suturing the perforation and colostomy or bringing the injured parts of the colon onto the abdominal wall are followed by frequent pyo-septic complications and high rate of lethality.

[Injury of large intestine in urgent surgery].

The results of examination and surgical treatment of 108 patients (93 males, 15 females), aged from 19 to 73 years, with large intestine injuries of different origin are presented. Large intestine injuries as a result of stab-incised abdominal wounds was in 58 patients, gunshot wounds--in 29, blunt trauma--in 21. The diagnosis of large intestine injuries was based on clinico-laboratory, X-ray and instrumental examinations. The injury of blind intestine was revealed in 8 patients, ascending colon--in 11, transverse colon--in 39, descending colon--in 5, sigmoid colon--in 45. All the patients were operated. The method of choice in the surgical treatment of these injuries was suturing of damaged portion of large intestine, which was performed in 72 patients. In 14 patients the suturing was complemented by decompressive colostomy, in 3--by extraperitonisation of the damaged site, in 4--by extraperitonisation and decompressive colostomy, in 2--by terminal ileostomy. Resection of damaged intestinal segment with primary anastomosis was performed in 4 patients, right-side hemicolectomy--in 7, Hartmann's operation--in 17, resection of large intestine with bitrunk colostoma creation--in 3, transfer of damaged segment of large intestine--in 5. Repeated operations for intestine integrity repair and fistula closure were performed in 47 patients. 18 (16.7%) patients died after operation as a result of peritonitis (7), shock and acute hemorrhage (10), denutrition due to intestinal fistula (1).

[Strategy of the surgical treatment of postoperative complications in patients with obstructive ileus of the colon of tumorous etiology].

Under analysis were postoperative intraperitoneal complications and results of relaparotomies in 33 patients (13 women and 20 men) aged from 33 to 76 years with obturative ileus caused by stenosing cancer of different portions of the colon. High frequency of peritonitis in the general picture of postoperative complications was due to incompetent sutures of the colonic anastomoses applied during emergency and urgent operations in patients with sub- and decompensated forms of intestinal obstruction. The best variant of completion of radical operations in emergency surgery in this category of patients is the formation of a single- or double-trunk colostomy without a primary reestablishment of the continuity of the intestinal tube especially when stenosing cancer is localized in the left part of the colon.

[Surgical management in complicated sigmoid cancer].

The results of operative treatment of 242 patients aged from 30 to 85 years with complicated cancer of the sygmoid colon have been analyzed. It is shown that in the choice of the method for surgical intervention essential are the character of complications, peculiarities of tumor growth, as well as the stage of the disease and general condition of the patient. The removal of the tumor followed by forming of decompression colostomy, without restoration of bowel continuity results in achievement of better initial results. Resection of the tumor with one stage placement of the anastomosis is advisable as an elective procedure, after elimination of acute symptoms of complications and substantial preoperative preparation. The application of U-shaped terminolateral anastomosis allows to form a primary colonic anastomosis in acute bowel obstruction due to the tumor of the sygmoid colon.

[Surgical management in perforative tumors and diastatic ruptures of colon].

The results of examination and treatment of 38 patients aged from 24 to 85 years with perforated tumors and diastatic ruptures of the colon are analysed. According to the authors conception, optimal surgical procedures in perforative operable tumors and diastatic ruptures of the colon are resection of the affected segment without restoration of the bowel continuity. Palliative operations should be used in patients with inoperable tumors of the colon.

[Ways of improving the results of surgical treatment of colonic obstruction of tumorous genesis].

The work presents results of examinations and treatment of 522 patients with intestinal obstruction due to stenosing cancer of different parts of the colon. Operations were performed on 486 (93.1%) patients. It was established that the widening of indications to radical operative interventions on patients of all age groups independent of the level of localization of stenosing cancer of the colon was one of perspective ways for the improvement of results of surgical treatment. In patients with the acute obstruction of the left half of the colon it is expedient to complete radical operations making primary anastomoses.

[End-to-end anastomosis as a method of choice in right hemicolectomy].

The newest and long-term results of treatment of 239 patients were analyzed in whom right hemicolectomy with ileotransverse anastomoses "end-to side", "side-to side" and "end-to-end" were fulfilled. When estimating the results, the data of original methods developed and tested in the clinic were used for studying the motility and bioelectrical activity of the intestine in addition to clinical parameters. The main regularities of the development of postoperative complications were found in randomized groups. It was established that choice of the optimum variant of the anastomosis in right hemicolectomy was dependent on the understanding of pathophysiological consequences of either intestinal anastomosis. The anastomosis "end-to-end" is the quickest to fulfil, less traumatic and safe of the 3 types of ileotransverse anastomosis.

[Peculiarities of clinical features and surgical treatment policy in old patients with acute cholecystitis].

The results of treatment of 138 patients with acute cholecystitis aged from 75 to 89 years are analyzed. Calculous cholecystitis was revealed in 126 patients, acalculous cholecystitis--in 12 patients. Cholelythiasis was revealed in 63 (45.6%), stricture of the large duodenal papilla (LDP)--in 13 (9.4%), combination of choledocholithiasis and stricture of LDP--in 16 (11.6%). Mechanical jaundice was revealed in 54 (39.1%) patients, purulent cholangitis--in 18 (13%). From 138 patients 113 (81.8%) underwent surgery. Urgent operations were carried out in 35 (30.9%), early operations--in 54 (47.8%), delayed operation--in 24 (21.2%) patients. Cholecystectomy was performed in 99 patients, it was supplemented with choledocholithotomy and various types of external drainage of the duct (n = 28), choledochoduodenostomy (in 40), transduodenal papillosphincterotomy (n = 6) and endoscopic papillosphincterotomy (n = 7). Cholecystostomy was carried out in 14 patients (including laparoscopic cholecystostomy, n = 5). Repeated operations were performed in 29 patients. Postoperative complications appeared in 28 (24.7%) patients. 16 patients (14.1%) died after the operation. The causes of death were hepatic functional deficiency (6), peritonitis (2), pancreatic necrosis (1), acute cardio-vascular failure (4), pulmonary artery thrombo-embolism (2), acute cerebro-vascular failure (1).

[Psychological, moral-ethic and deontologic aspects of relaparotomy].

Analysis of the results of relaparotomy in 208 patients is presented. Psychological, moral, ethic and deontologic aspects of repeated laparotomy have been studied. In indications for relaparotomy the choice of the surgeon is advisable to make as a collective decision, on individual basis, after careful analysis of the causes of postoperative complications and with regard for the wish of the surgeon who performed the first operation. In 137 patients of 208 relaparotomy was carried out by the surgeon by whom the first operation was performed, with the assistance of more experienced surgeons. Lethality made up 35% (48 patients died after the operation). 60 patients were reoperated by the other surgeons with the assistance of the surgeon who had carried out primary operation. In 11 cases relaporotomy was carried out without participation of the surgeon who had performed the initial laparotomy. 24 (33.8%) patients died after relaparotomy performed by the other surgeons. "Radical" relaparotomies were carried out in 141 patients, in 94 (66.7%) of them being performed by the surgeon who had done the initial operation and in 47 (33.3%)--by the other surgeons. 25 (17.7%) patients died after "radical" relaparotomies and after "palliative"--47 (70.1%) patients. Total lethality after relaparotomy made up 34.6%.

[The surgical procedure in tumor-induced colonic obstruction in patients at an elevated surgical risk].

A comparative assessment of results of palliative and radical operations in 250 patients has proved efficiency of radical operations which were followed by less lethality rate in patients with high operative risk. A conclusion is made that wider indications to radical operations for patients of older age groups even those with different risk factors is one of perspective ways to improve results of surgical treatment of patients with obturative obstruction caused by colonic tumour of any localization.
