The effects of chemotherapy on bony metastases as measured by quantitative skeletal imaging.

The effect of chemotherapy on bony metastases from adenocarcinoma of the colon was investigated by quantitative skeletal imaging over a two-month interval. The quantitative skeletal imaging results correlated with conventional blood chemistry results over this time period. While chemical assay techniques furnish an average value of lesion response, the quantitative bone scan represents a method for individual lesion analysis. This methodology has the potential to provide a better understanding of metastatic bone disease therapy.

Evaluation of a sequence of diagnostic tests using the workup of ureteral stone as a model.

Additional tests are not always beneficial and can increase costs significantly while increasing the group of patients with an equivocal diagnosis. In general, diagnostic testing should cease for a group of patients when the tests that have been performed result in a sufficiently high level of diagnostic certainty and also result in the largest group of true positive or true negative diagnoses. Evaluation of diagnostic tests must include consideration of the results of sequences of tests, equivocal results of tests, and the effect of added costs on both advantageous sequences of tests and total expenditures. Evaluation of the diagnostic tests performed for ureteral stone in the emergency ward demonstrates that a significant group of patients, on whom an intravenous urogram could be avoided can be selected. As an alternative to the IVU in this selected group, hydration and diuresis could be considered. The IVU could be reserved for those patients who did not experience classic relief of pain and passage of stone.

Determining the size of pneumothorax in the upright patient.

A simple measurement, the average interpleural distance (AID), which is obtained from three linear measurements, closely predicts true pneumothorax size as determined by radiographic thoracic gas volume measurement (mean variation -2.4 +/- 7.23 SD). The distribution of pneumothorax volume between the affected lung and the ipsilateral thoracic cage is variable and unpredictable. It was found that the average volume of pneumothorax that is accommodated by thoracic cage expansion (8%) is much less than previously reported. This appears to be the result of variable degrees of splinting and chest wall pain associated with pneumothorax.

Radiographic evaluation of rib fractures.

A prospective study of 100 radiographs performed for possible rib fracture due to blunt trauma was conducted to determine if some of the films routinely obtained should be eliminated for medical or economic reasons. Of the 100 patients evaluated for fracture, 29 fractures were detected radiographically. The physical findings of fracture are seen to be nonspecific, and the clinical impression of the likelihood of fracture is unreliable. Therapy is symptomatic, aimed at relief of pain, and except for the complications of fractures, therapy does not differ significantly in patients with and without fractures. The complications of fractures that may influence therapy were seen in 13 of the 29 patients with fractures. All 13 complications were detected by the posteroanterior chest film while only three were seen on the rib films. Radiographs obtained solely to detect fractures are not warranted, but a posteroanterior chest film is necessary to assess pleural or pulmonary complication of chest trauma.
