Degradation of P(3HB) and P(3HB-co-3HV) in biological media.

The biodegradability of oriented fibers made of polyhydroxybutyrate (P(3HB)) and its co-polymer with beta-hydroxyvalerate (P(3HB-co-3HV)) was investigated in buffer solutions and in biological media in vitro and in vivo. The fibers of both polymer types demonstrated resistance to hydrolytic degradation in buffer solutions at 38 degrees C and pH from 4.5 to 7.0 (for up to 180 days). It has been found that the biodegradation of the fibers in vitro in blood and serum and in vivo is accompanied by weight losses and minor changes in the microstructure with no significant losses in the tensile strength over a long time (up to 180 days). The biodegradation rate of the less crystalline co-polymer P(3HB-co-3HV) fibers was 1.4-2.0-times higher than that of the homopolymer P(3HB). It has also been shown that the degradation of the fibers in vivo is influenced both by tissue fluid enzymes and cells (macrophages and foreign-body giant cells). The fibers were eroded on the surface only with no gross defects and no dramatic effects on their mechanical performance.

[A study of mydocalm efficiency in the treatment of chronic headache of tension].

Mydocalm was used for the treatment of 31 patients, mean age 45 +/- 4 years, with chronic headache of tension in a dosage of 150 mg 3 times per day during 4 weeks. Twenty-six patients have completed the therapy course. Before- and after treatment, neurological status, dynamics of psychoautonomic disorders, an extent of anxiety and depression, quality of sleep, quality of life, pain syndrome measured by a number of tests, were assessed, along with electromyography and examination of nociceptive reflex. Subjective evaluation of the drug efficacy was taken into account as well. A marked positive effect of mydocalm was observed in 70% of patients, moderate effect--in 30%. A predictor of mydocalm efficacy was severity of muscle tonic syndrome.

[Analysis of virulent and avirulent strains of Vibrio cholerae by the nest polymerase chain reaction].

The detection of cholera enterotoxin in environmental objects and isolation of patients are considered to be the most reliable indices of that the cholera agent is present. Described in the case study is a method of nested polymerase chain reaction (PCR) based on amplifying the ctxA gene fragment coding subunit A of enterotoxin. A possibility was shown to use the above method to confirm the virulence of strains Vibrio cholerae isolated from different sources. The method was tested with 18 virulent and avirulent strains V. cholerae as well as (for the sake of verifying the analysis specificity) with DNAs of other human-pathogenic microorganisms and with the human genome DNA. The results showed a high efficiency of nested PCR in detecting the pathogenicity of cholera-agent strains.

[A psychoneurophysiological evaluation of non-specific cerebral systems in patients with permanent autonomic disorders of psychogenic etiology during therapy].

Fifteen patients with permanent vegetative disorders combined with a pronounced anxiety were examined before and after treatment by the Nott drug. The clinical-and-neurological technique, psychometric evaluation of cognitive functions and the neurophysiological method of acoustic cognitive induced potentials P300 were made use of. Before treatment, pronounced vegetative anxiety-related and cognitive disorders were observed in patients. A reliably decreased amplitude of the P300 peak was noted in patients versus healthy subjects while registering the cognitive induced potentials. After treatment, there was a reliably decreased severity of vegetative, anxious and cognitive disorders in patients; the P300 amplitude went up reliably. Supposedly, the changed P300 amplitude, as observed in patients with permanent vegetative disorders; denotes the malfunction in non-specific limbic-reticular brain structures. A growing P300-peak amplitude combined with the arrest of psychovegetative and anxious disorders and with an improvement of cognitive functions in patients due to therapy are indicative of a lower degree of the functional disintegration in the non-specific limbic-reticular brain structures. Finally, the method of endogenous induced P300 potentials can be used as a diagnostic tool in the objective evaluation of a condition of non-specific brain systems in patients with the psychovegetative syndrome; it can also be an objective criterion in the efficiency assessment of a conducted therapy.

Psychoneurophysiological assessment of recovery of cognitive function in patients with asthenia during rehabilitation.

Forty patients with severe asthenia were subjected to clinical, neurological, psychometric, and neurophysiologic examination before and after Enerion therapy. Psychoneurophysiological assessment with cognitive evoked potentials P300 and psychometric tests revealed serious impairment of cognitive functions in patients before therapy (compared to healthy individuals). Enerion therapy decreased the degree of psychoautonomic syndrome and asthenia and improved cognitive functions.

Evaluation of nonspecific brain systems in patients with panic disorders by the method of P300 cognitive evoked potentials.

Measuring of the amplitude of auditory cognitive evoked potentials P300 showed its changes (decrease and increase) in patients with panic disorders compared to healthy individuals. The authors hypothesize that the decrease in P300 amplitude reflects increased activity of the cerebral reticulothalamic structures, while its increase is associated with activation of the septohippocampal limbic system in patients with panic disorders. P300 recording can serve as a valuable diagnostic method.

[Use of the nested polymerase chain reaction in the differential diagnosis of human herpes simplex virus].

Herpes is one of the most widespread human viral pathologies. The article depicts a special modification of polymerized chain reaction--(PCR)--(referred to as "nested"), which has a higher sensitivity, specificity and reliability as compared to the ordinary PCR, and which is designed to detect the herpes viruses. The method was initially tested at purified preparation of viral DNA, and later--at clinical materials obtained from patients and healthy donors. Secretions from the urogenital tract (smears), scrapes from the urogenital tracts and urinal cellular samples were examined in patients. Herpes simplex was detected in all cases. As for the healthy people, the identical examinations produced in them mainly the negative findings. Therefore, the nested PCR is a simple, sensitive and effective instrument in the diagnostics and prevention of herpes; it can be recommended for a comprehensive usage in medical practice.

[Use of nested PCR in detection of the plague pathogen].

Causative agents of plague, i.e. bacterium Yersina pestis (in the subcutaneous tissues of rodents) and their cutaneous parasites need to be isolated to enable plague prevention. A comparatively new method of polymerase chain reaction (PCR) opens up new possibilities of determining Y. pestis just within several hours and without any cultivation. The article contains a description of the PCR-method, which makes it possible to distinguish the culture of Y. pestis from cultures of other microorganism, including speci of Yersina. The method is of the cluster-type, i.e. it is made up of subsequent PC reactions with the substrate for the second reaction being the product of the first one. The cluster nature of the method preconditions a higher sensitivity and specificity versus the ordinary PCR.

[Use of enerion in psychovegetative syndrome with pronounced asthenia].

Enerion was used in the treatment of 40 patients with psychovegetative syndrome and asthenia. Before treatment, the patients have pronounced asthenia appearances, anxiety disorders, relevant autonomic disorders, mild depression and decrease of working capacity, attention concentration and memory. The treatment was conducted as monotherapy during 28 days with enerion dosage 400 mg/day. After treatment, expression of autonomic and asthenic disorders was reduced significantly, along with improvement of psychometric and neurophysiological indices. High efficacy of the medication was detected in 75% of the patients and moderate one--in 25%. The drug was well-tolerated.

[Clinical and neurophysiological aspects of cognitive disturbances in patients with panic disorders].

Auditory event-related potentials P300 were studied in 44 patients with panic disorders (PD) and 23 normal controls. Compared to the controls, the patients differed significantly by the reduced P300 wave amplitude and alteration of its habituation. PD patients also demonstrated more pronounced anxiety, depression, autonomic dysfunction, hyperventilation and attention insufficiency. The patients with atypical PD differed from those with typical PD by reduced P300 wave amplitude, higher depression and lower personality anxiety levels. The data obtained demonstrate that cognitive function disturbances in PD patients as well as qualitative psychoneurophysiologic differences between patients with typical and atypical PD which were most striking for P300 wave amplitude and alteration of its habituation. Therefore, event-related potentials P300 method may be used as a diagnostic tool and should be taken into account in deciding on therapy.

[Kinesthetic brain evoked potentials in schizophrenia].

Cortical kinesthetic evoked potentials (KEPs) of the brain to passive radiocarpal flexion of the hand in 16 schizophrenic patients with catatonic syndrome have been studied. Significant changes of amplitude and time parameters of the early components of the KERs were shown in patients being in the catatonic substupor state, as well as normalization of these parameters after the disappearance of catatonic symptoms. It is suggested that the phenomenon of excessive exitation of corpus striatum leads to irritation of motor zones of cerebral cortex regulating the processes of proprioceptive information transmission in the kinesthetic analyzer, which is accompanied by sensory projections blockade, is the basis of substuporous states.

Inhibition of telomerase activity of melanoma cells in vitro by antisense oligonucleotides.

The effect of antisense oligonucleotides complementary to the RNA component of human telomerase on telomerase activity in cell extracts of the melanoma cell line SK-Mel-28 has been studied. It has been shown that the antisense oligonucleotide complementary to the hTR component in the region of the template synthesis of telomeric repeats is the most efficient inhibitor of telomerase activity in comparison with other antisense oligonucleotides. Pronounced inhibition of telomerase activity was observed at the oligonucleotide (Tel P5) concentration in the reaction mixture of about 5 nM. Complete inhibition of the enzyme occurs at the oligonucleotide concentration in the sample of about 20 nM.

[Comparative results of surgical treatment for perforating and bleeding pyloroduodenal ulcers].

Results of treatment of 1309 patients with perforated and bleeding pyloroduodenal ulcers for 20-years period have been analysed. Resection of the stomach performed in 85 cases resulted in high postoperative lethality which made up in bleeding ulcers 14.8%. Drainage operations of the stomach with excision or suturing of ulcer combined with bilateral truncal vagotomy was performed in 60 patients, postoperative lethality rate being 8.4%. 128 patients underwent selective proximal vagotomy together with pyloro- and duodenoplasty, lethality rate being 1.6%. Combined vagotomy (posterior truncal and anterior sero-muscular) with excision of ulcer, transversal pyloroplasty and duodenoplasty was carried out in 1036 patients (postoperative lethality--2.4%). Excellent and good functional results were achieved in 79.6% of the patients.

[Use of laparoscopic suturing devices in abdominal surgery].

The article deals with the results of clinical use of a laparoscopic suturing instrument in appendectomy in 2 patients with acute appendicitis. The authors describe the techniques of laparoscopic appendectomy with application of a mechanical suture to the mesentery and stump of the process. The advantages of this operation over the traditional interventions in acute appendicitis are shown: the absence of the pain syndrome after the operation, shorter duration of in-patient treatment, and the best cosmetic effect.
