Home Talk/Healthy Talk: improving patient's health status with telephone technology.

This article describes Home Talk and Healthy Talk, two practical applications of telephone technology developed at the VNA of Cleveland. Home Talk is a disease-specific monitoring system with preprogrammed automated questions that immediately score patient responses. If any responses fall out of the safety zone, the system calls the supervisor and a live intervention follows for assessment and appropriate action. Healthy Talk is a series of age-specific automated health screens used by nurses in community centers to engage clients to follow through with self-selected health goals. The use, cost-effectiveness, and impact on outcomes are also discussed.

Therapeutic group work with depressed elderly.

This article presents several approaches to therapeutic group work with the elderly, drawing from experience in designing and implementing several group interventions for frail nursing home residents. What is important to recognize is that the common preconception that the elderly, in general, and the frail, in particular, cannot participate in therapy and groups is a misconception at least, if not a prejudice.

Venipuncture: an adjunct to home care services for older adults.

Attention to policy and procedure development specific to home care, and strong communication and collaboration with laboratory personnel at multiple sites have been essential in supporting venipuncture services within a home care agency. Orientation, inservice training, skills verification, updated supplies, and the concise information kept in each nurse's field handbook have supported comprehensive quality services to an older, frail, homebound population. In addition, our quality evaluations from physicians, patients, caregivers, and laboratories indicate a greater than satisfactory level of performance and quality care for this skilled service.

The time has come--a study of geriatric continuing education in Ohio Visiting Nurse Associations.

A majority of nurses working in the community lack formal educational preparation to care for older adults. This study examined the perceived learning needs of Visiting Nurse Association (VNA) executives and agency staff nurses in Ohio related to continuing education in gerontology. Twenty-six executives and 478 staff nurses were asked to complete a gerontologic education survey; 17 executives and 344 staff nurses returned surveys. Findings suggest that few Ohio VNAs have focused on continuing education in gerontology. Indications exist that executives and staff nurses are not always in agreement regarding the importance of content needed for updating gerontological knowledge and skills. Educational and organizational strategies are offered to provide minimal standards for gerontologic professional practice in home care.

A model to restructure nursing education. Vision on 22nd street.

Since 1993, the Cleveland State University Department of Nursing and the Visiting Nurse Association of Cleveland have been involved in an innovative partnership known as Vision on 22nd Street. This partnership has produced a model to restructure the nursing curriculum and the practice setting in such a way that a seamless process, fully integrated within university and agency operations, prepares undergraduate nursing students for community-based practice. The groundwork supporting the model, the development of the curriculum, the preparation of participants, and the implementation of the model for program evaluation are the subject of this article.

Gerontologic home care: a prescription for distance continuing education.

Visiting nurses are increasingly challenged by the health care delivery system to provide sophisticated technological care to a growing number of patients in their homes. Much of this care is devoted to the care of the older adult. Few nurses currently in practice have received formal preparation in gerontology; thus, a knowledge deficit exits. Recognizing the challenge of teaching visiting nurses to develop individual interests in the scientific and technologic aspects of gerontologic nursing, the Visiting Nurse Association of Cleveland designed and proposed a comprehensive project to teach gerontologic nursing using the American Nurses Association Standards of Gerontologic Nursing Practice (American Nurses Association, 1987) through discovery learning and distance education via computer and other educational interventions. Community health nursing offers a unique approach to linking practice to education and research and provides organizational and educational alternatives to traditional continuing education.
