Primary carcinoid tumor of the liver: report of four resected cases including one with gastrin production.

Four cases of primary hepatic carcinoid were identified during a retrospective study of liver resections for primary tumor. The cases included two adult males, one adult female, and a 9-year-old boy in whom gastrin levels were documented. The estimation of gastrin levels was prompted by symptoms suggestive of acid-peptic disease. One patient died postoperatively. The other three are alive and well at 3 years, 2 years, and at 1 year, respectively, after surgery, outcomes distinctly different from hepatocellular carcinomas. Diagnostic difficulties may be experienced in histologic assessment, and this may require recourse to immunohistochemistry and electron microscopy. Long-term follow-up and careful exclusion of a possible primary elsewhere are necessary for establishing the primary nature of liver carcinoids.

Rhabdoid phenotype in squamous carcinoma. A report of two cases.

We report two cases of squamous carcinoma in which a prominent rhabdoid phenotype was seen. The tumors which occurred in the buccal mucosa of a 70 year old man and the skin of a 85 year old man were positive for cytokeratin and epithelial membrane antigen indicating their epithelial nature. Co-expression of vimentin was also noted. The aggressive nature of the rhabdoid component is confirmed by the fact that the cervical lymph node metastasis in case I was also rhabdoid.

Psammomatous carcinoid tumor of the duodenum.

We report a case of psammomatous calcification in a carcinoid tumor of the ampulla of Vater. Although the tumor was glandular, its neuroendocrine nature was proved using chromogranin immunostain.

Heterotopic ossification in colorectal adenocarcinoma [corrected].

Heterotopic ossification in Adenocarcinomas of the colon is uncommon. The clinical presentation of these patients is similar to those without osseous metaplasia. Its importance lies in the fact that it may be misdiagnosed for a carcinosarcoma. Which has a poorer prognosis. These two cases are reported because of its rarity.

Filtration and agar embedding applied to fine needle aspirates for tumor diagnosis by electron microscopy. A combined technique.

OBJECTIVE: To develop a novel method for processing of fine needle aspirates subjected to electron microscopic (EM) study. STUDY DESIGN: Included 70 cases of poorly differentiated malignant tumors in which a definitive diagnosis was not possible on light microscopic (LM) examination and that thus required application of an ancillary technique such as FNA/EM, for diagnosis. We have established a novel method of processing, a technique of filtration through nylon mesh filters to eliminate red blood cells (RBCs) and necrotic debris, followed by agar well embedding to avoid loss of diagnostic material during processing without centrifugation at later steps after agar embedding, thus minimizing the time required for processing. It was successfully carried out in 70 cases. RESULTS: The combined technique was extremely effective in eliminating RBCs and necrotic debris. It also avoided further loss of valuable diagnostic material. An accurate diagnosis was rendered in 70 cases; that was not possible by LM alone. The whole procedure saves two to three hours of processing as centrifugation is not required after the agar embedding step. CONCLUSION: This technique was found to be cost- and time-effective, particularly suitable for developing countries, where financial resources are limited.
