[Extensive and combined resections in poorly differentiated lung cancer].

The article generalizes an experience with surgical treatment of lung cancer of low-differentiated structures in 108 patients. Extended and combined resections of the lung in such patients were shown to be expedient. From 83 patients who had had bronchial biopsy in the process of preoperative examination 35 patients (42.2%) had results of histological examinations of the biopsy and operative materials which did not coincide. Extended and combined resections for lung cancer of low-differentiated structure gave 5-year survival in 15.6% of the patients.

[Megakaryocytopoiesis in patients with acute infectious destruction of the lungs].

Partial megakaryocytograms were studied in patients with acute infectious destructions of the lungs. Acid phosphatase and glycogen megakaryocytic levels, overall number of megakaryocytes elucidated the role of the central link of the system megakaryocyte--platelet in the onset of thromboembolism and hemorrhagic complications arising in the above patients. Inefficiency of megakaryocytopoiesis in pulmonary and pleural acute pyo-destructions is under discussion.

[Penetrating gunshot wounds of the chest: the surgical procedure and treatment times and outcomes].

On the basis of data obtained during the war in Afghanistan the article analyses surgical management and the results of treatment in three groups of patients with chest wounds. The authors consider that the efficiency of treatment could be improved by gaining time during evacuation period and providing the earliest surgical care to the wounded. Indications and peculiarities for various surgical interventions were ascertained depending on the character of trauma and subsequent pathological disorders. The article proves the necessity of preventive sanitation of pleural cavity, gives the data concerning the period and outcome of treatment.

[Disorders of erythrocyte deformability in patients with acute suppurative-septic diseases of the lung and pleura].

In 163 patients with acute suppurations of lungs and pleura a substantial reduction of deformability of erythrocytes was established. The maximum increased rigidity of blood red cells was noted at the peak of the pyo-destructive process and postoperative period as well as in the complicated course of the postoperative period. The curative plasmapheresis, general hyperbaric oxygenation exert positive influence upon plastic properties of erythrocytes.

[Possibilities of reconstruction of the fingers in relation to individual differences in their topographic-anatomic structure].

On the basis of the results of topographic and anatomical investigation carried out at 44 hands of cadavers are discussed the possibilities of restoration of the first finger by means of movement of damaged fingers on the neurovascular limb. The fundamental methods of surgical technique, developed with regard to the structure of neurovascular formations of the hand and individual variability of their topography, are expounded.

[Leukocyte transfusion in complex treatment of acute pulmonary suppurations].

The authors studied the effect of transfusions of a leukocyte suspension on the main clinico-immunological values of homeostasis in 56 patients with acute pyodestructive diseases of the lungs. They found that inclusion of leukocyte transfusions in the complex of therapeutic measures produced a marked clinical effect, reduced the term of treatment, and allowed successful control of endotoxicosis and immunodeficiency in patients with pulmonary suppuration. Immune leukocytes obtained according to a specially developed method had a more marked immunocorrective effect.

[Donor tissue source in plastic surgery of the hand].

From topographic and anatomical examination of 32 upper limbs the authors proved that an island skin-fascia graft supplied with blood by the septocutaneous branches of the posterior interosseous vascular bundle can be formed on the posterior aspect of the forearm. Such a graft on a peripheral vascular pedicle was transplanted in 6 patients with defects in the integumentary tissues of the hand; it healed completely in 4 cases, peripheral necrosis occurred in 2 cases but did not affect the end result. The use of means of optic magnification and prevention of compression of the vascular pedicle in the postoperative period ensure successful transplantation of the graft.

[The first experience with crural plastic surgery using island composite skin flaps].

An analysis of literature data, topographo-anatomical studies of 23 lower extremities and an experience with using 18 island fasciocutaneous flaps of the lower leg are presented. The article describes the possibilities of using a new donor area for plasty of limited defects of the lower leg and foot after various traumas and their sequelae.

[Variants of microsurgical autologous transplantation of composite skin flaps from the external surface of the hip].

Topographic anatomical investigations have established the possibility to make a new complex skin flap on the lateral surface of the femur on the branches of the second perforating artery and vein. The authors propose methods of making a transplant in this field on two vascular pedicles with anastomoses in one of them--a larger one and two separate flaps: a skin-fat and a muscular flaps on a single feeding pedicle. The experimental findings were confirmed by successful clinical observations.
