[Pathomorphosis of infiltrative pulmonary tuberculosis: clinical aspects].

Based on the data available in the literature and their own findings, the authors have established that the pathomorphism of infiltrative pulmonary tuberculosis in the preantibiotic age was characterized by negative features, then underwent phases of positive changes and relative stabilization, thereafter it again acquired obviously marked negative peculiarities. At present, the clinical features of new cases of infiltrative pulmonary tuberculosis in Perm residents are the predominance of acute onset (67.3%), disseminated (55.8%) and destructive (91.6%) pattern of specific lesion accompanied by bacterial isolation (84.6%).

[Epidemiological features of the pathomorphism of infiltrative pulmonary tuberculosis in the Perm Region].

The time course of changes in current epidemiological indices and epidemically significant features of the pathomorphism of infiltrative pulmonary tuberculosis in those living in the Perm Region are mainly characterized by negative trends. They lie in the increase or stable maintenance of high rates of morbidity, mortality, and destructive pattern of a lesion accompanied by bacterial isolation. The reason for the established situation is incomplete realization of potentialities of improving actions to detect the disease early, to optimize diagnosis and medical therapy, and to considerably expand an phthisiosurgical aid.

[Immediate outcomes of medical therapy in patients with caseous pneumonia].

The immediate outcomes of treatment were studied in 119 patients with caseous pneumonia whose age was 20 to 64 years. In most cases, caseous pneumonia was found to preserve its classic traits by characterizing by acute onset (85.8%), prompt progression, high proportion of fatal outcomes in the early postoperative period (54.1% within 3 early postoperative months). There is an increase in cases of more "benign" course of this form of tuberculosis (subacute onset and prolonged slow progression) at the same time. The use of current methods for combined intensive antibiotic and pathogenetic therapies substantially improves the outcomes in this most severe group of patients, reduces the initiation of a phase of relative stabilization of a specific process and of cessation of bacterial isolation.

[Outcomes of surgical interventions in patients with infiltrative pulmonary tuberculosis].

Thirty eight patients underwent surgery. The most significant causes of ineffective therapeutical measures in patients were their treatment incompetences (100%), multidrug resistance of Mycobacteria tuberculosis (47.3%), lung tissue destruction (68.4%), concomitant alcoholism (47.3%), relapses of specific lung damage (23.7%). If medical treatment is ineffective in patients with infiltrative pulmonary tuberculosis for 6 months, it is expedient to make a surgical intervention that promotes recovery (96%) and restore working capacity (84%) in them.

[Immediate and late outcomes of surgical treatment in patients with tumor-simulating pulmonary tuberculosis].

The specific features and outcomes of surgical treatment were analyzed in 122 newly detected patients with pulmonary tuberculomas without prior specific therapy. Postoperative complications developed in 4.1% of the patients undergone surgery, recurrent tuberculosis was observed in 1.6%. Early surgical interventions in this group of patients were found to reduce the time of treatment and working rehabilitation. A value of intraoperative rapid cytological verification of diagnosis was shown while choosing the adequate volume of lung resection in difficult diagnostic cases.

[The use of intrapulmonary needle-jet injections of chemical preparations in the preoperative management of patients with pulmonary tuberculosis].

Among the various methods of antibacterial preparation injection for the management of pulmonary tuberculosis the intrapulmonary injection techniques with the help of a needle-stream injector (NSI-1) still remain little studied. The experience of using these techniques for treating more than 200 patients, among whom 23 patients were later operated on, showed that the early use of the method (immediately after the diagnosis was established) helped to stabilize the process in 1-1.5 months. This made it possible to perform operations within 3 months after the diagnosis was made. The method proved effective also in patients who did not present favourable dynamics during 3-6 months of the use of the traditional therapeutic methods. The results obtained allow for the use on a wide scale of needle-stream injections in the preoperative management of destructive tuberculosis patients.

[Postgraduate training of pathologists].

Specialization of institutions engaged in training of pathologists is a key condition of skilled specialists turn out. Medical colleges should be responsible for training hospital and junior physicians. Institutes for postgraduate medical education must ensure updating of the practicing pathologists knowledge and prepare them for getting special degrees and diplomas including "European Pathologist". The system of continuous training of pathologists should rest on fundamental legal base which should be warranted by the Russian Federation Ministry of Public Health.

[Surgical interventions in tuberculosis of the lungs without prior antibacterial therapy].

Operations were performed on 102 patients (93 with tuberculomas, 2 with fibrocavernous tuberculosis, and 7 with infiltrative tuberculosis of the type of caseous pneumonia) without preoperative chemotherapy. Certain difficulties in identification of the nature of the disease in tuberculomas are pointed out. It is shown that only emergency histological study helps to avoid errors. Postoperative complications occurred in 5.9% of patients. Exacerbation of tuberculosis after economical resections in tuberculomas occurred in 2.1% of patients due to a nonradical intervention. The expediency of early interventions which allow the term of treatment to be shortened is shown.

[Morphology of sepsis in young children].

The paper outlines early childhood sepsis as a systemic infectious disease acyclically proceeding in the presence of alterations in the body reactivity, analyzes the autopsy specimens obtained from over 200 infants who died from various sepsis types in the period of 1965-1989, describes the morphofunctional features of sepsis in infants, and yields a concept that immunodeficiency contributes to the disease.

[Occupational morbidity among coal miners in the RSFSR].

Analysis of occupational morbidity for 1982-1989 in coal extraction industry workers in the Russian Federation showed its high-level detection in coal-miners. Revealed was a decrease in dust-induced occupational diseases and a greater detection of vibration disease cases, which accounted for 50% in the occupational diseases' structure. In miners, vibration disease cases occurred equally often in coal mine face-workers and driving team workers. The face-workers also exhibited dust-induced diseases and locomotor system disorders. In open pit mines and coal-refining plants, occupational pathology cases were only sporadic. Detectability of occupational morbidity cases was highest in the Rostov Region, Kemerovo, Far-East and Komi coal mines. The performed analysis revealed factors which influence most of all the prevalent forms of occupational pathology, thus laying the basis for the elaboration of health improvement measures.

[Chronic interstitial lung diseases. The status of the problem].

Literature and original data on etiology, pathogenesis and morphogenesis of chronic nonspecific lung diseases provided the authors with evidence leading to conclusions on common pathogenesis of the above wide spectrum of the diseases characterized by affected intercellular relations in the lungs. Immunologic homeostasis is suggested as an essential factor resultant in the disease progression along the vicious circle in case of its defective functioning.

[Postgraduate instruction and certification in the system of the continuing education of pathologists].

The experience in changing pathology service, postgraduate training and attestation in the system of continuous education of pathologists is presented. The authors analysed different ways of pathologist training: specialization in subinternship, internship and clinical internship, short-term courses, at places of work and outpatient problem seminars, etc. The four-year program of pathologist training is suggested.

[International scientific relations in the study of the clinical aspects and pathology of cholera (1817-1850)].

This review deals with scientific-medical international connections mainly in the investigation of the pathology of cholera in the first half of the XIX century. The attention is drawn to the struggle between "contagionist" (materialistic) and "myasmatic" (idealistic) trends in the epidemiology of that time. The works of Russian and foreign physicians are listed which served to the approach and reciprocal enrichment of the medical culture in European countries. The investigations in the field of pathology of the Asiatic cholera during the first half of the XIX century were summarized in a N. I. Pirogov's comprehensive publication which was highly appreciated by the German pathologists R. Virchov and V. Grizinger. It should be emphasized that N. I. Pirogov thoroughly studied and summarized clinical and pathological experience of his numerous predecessors--Russian and foreign scientists of that time who contributed to the study of cholera.
