Effect of lipoxygenase and cyclo-oxygenase inhibition on oleic acid-induced lung oedema.

The contribution of cyclo-oxygenase and 5-lipoxygenase metabolites on hemodynamics and oedema formation was investigated in 21 isolated rabbit lungs after a 10 min Oleic Acid (OA) infusion, by recording the changes on Fluid Filtration Rate (FFR) and Pulmonary Artery Pressure (PAP). Lungs (n = 7) were pre-treated with indomethacin (cyclo-oxygenase inhibitor) 50 min prior to OA or with Diethylcarbamazine (5-lipoxygenase inhibitor) (n = 7) or not pre-treated at all (control group, n = 7). The FFR in the indomethacin group was significantly greater than in the control and Diethylcarbamazine (DEC) groups 12 min after OA (7.6 +/- 2.3 mg.min-1 vs. 2.3 +/- 0.8 mg.min-1 and 0.96 +/- 0.8 mg.min-1 respectively) (P < 0.01). The FFR in the control lungs 20 min after OA was significantly greater than the corresponding DEC value (4.2 +/- 0.5 mg.min-1 vs. 1.6 +/- 1.0 mg.min-1) (P < 0.01). Mean Pulmonary Artery Pressure (MPAP) increased both in control and indomethacin groups (16.0 +/- 2.0 Torr to 24.3 +/- 3.7 Torr after 20 min OA and 14.4 +/- 2.5 Torr to 24.6 +/- 3.6 Torr at 10 min after OA, respectively), but MPAP value in DEC group did not significantly change 30 min after OA (14.7 +/- 1.5 Torr to 16.0 +/- 2.3 Torr) (P > 0.05). So we conclude that the selective inhibition of the 5-lipoxygenase metabolites (leukotriene-5hete) may play a protective role in OA induced oedema, whereas the selective inhibition of the cyclo-oxygenase pathway may have a deleterious effect on the hemodynamics and endothelial permeability in our experimental condition.

Effect of high frequency oscillatory ventilation on fluid filtration rate in isolated perfused rabbit lungs.

We have studied the effect of changing ventilatory frequency (VF) of high frequency oscillatory ventilation (HFOV) on fluid filtration rate (FFR) in twelve isolated rabbit lungs perfused at constant blood flow. Mean pulmonary artery pressure (Ppa), mean left atrial pressure (Pla), airway pressure (Paw), pulmonary vascular resistance (PVR), pH, O2 and CO2 partial arterial pressures (PaO2 and PaCO2) and plasma colloid osmotic pressure (COP), were measured. We ventilated the lungs with a modified Bird Mark 7 ventilator which could achieve HFOV (range 5-30 Hertz). In each experiment VF was randomly varied on ten different occasions, maintaining each variation for ten minutes. The first group of six rabbits was ventilated under normal haemodynamic conditions. The other six rabbits were ventilated after the production of hydrostatic lung oedema. Blood gas exchange in both groups of rabbits was satisfactory. There was no statistically significant correlation between VF and FFR. We conclude that variations in VF using HFOV does not alter lung fluid balance in normal and in hydrostatic oedema rabbit lungs.

Effect of hypercapnia on fluid filtration rate during forward and reverse perfusion of isolated rabbit lungs.

Fluid filtration rate (FFR) was measured under predominantly zone-three conditions in isolated rabbit lung perfused at constant flow. During forward and reverse perfusion, alveolar hypercapnia significantly increased mean pulmonary artery pressure but did not change FFR. We conclude that the pulmonary vasoconstriction induced by alveolar hypercapnia occurs on both arterial and venular sides of the pulmonary circulation.

[Respiratory function after traumatic rupture of the diaphragm].

In eight patients who underwent surgical treatment for diaphragmatic rupture, pulmonary function and differential ventilation of the lungs were evaluated four months-one year after surgery. The results of the study show that diaphragmatic rupture produces only a small reduction in total ventilatory function. Moreover our data indicate that the ventilation estimated to be contributed by the side injured, is decreased in a larger extent in abdominal than in thoracic trauma.

[Tiapride in acute stages of drug addiction].

We studied the effect of tiapride on alcoholic and drug-dependent inpatients. In both the treatment was initiated at a time of acute intoxication. Tiapride was perfectly tolerated and we recorded none of the side effects seen with neuroleptics. The method of administration (continuous intravenous infusion) together with the mild sedative effect prompted the regressive response which is a necessary step before the subsequent reconstructive phase. The unquestionable though not overpowering sedative effect gives the patient a feeling of comfort and allows him to participate actively in the treatment. The second original characteristic of tiapride is that it's anxiolytic effect is mild, with the drawback of leaving anxiety and insomnia unalleviated. But this treatment has a novel advantage in that it does not induce any drug dependency, even secondarily.

[Intractable menopausal disorders cured by veralipride (author's transl)].

We studied a case of surgical menopause in a hysterical patient. Castration was followed by depression with sexual disorders, somatic complaints, and specific menopausal manifestations, mainly hot flushes. Hysterectomy had been done a year earlier. Since then the patient had been admitted twice to a psychiatric ward following attempted suicide. Veralipride was given for twenty days. Noticeable improvement was recorded. Therapy was then discontinued for ten days. Symptoms recurred and long-lasting therapy was decided on. Subsequently, sustained overall improvement with disappearance of specific menopausal disorders, were recorded.

A control study of muscle force recovery in hemiparetic patients during treatment with functional electrical stimulation.

A group of forty-nine hemiparetic patients with limited emotional, communication and sensibility involvement and with recent lesion of cerebrovascular aetiology was randomly divided into two groups of twenty-four and twenty-five subjects. Both groups received traditional physiotherapy treatment for one hour/day, one group received twenty min/day of peroneal nerve stimulation. The maximum voluntary dorsal flexion moments of the ankle joint of the affected and non affected extremities were measured with an isometric brace twice a week for one month and for both groups. The recovery of moment in the stimulated group turned out to be about three times greater than in the control group and considerably less dependent upon age, time from lesion, initial value, side of lesion. Three patients using a peroneal brace at home as an assisting device were again evaluated two months later and a further improvement was observed. This work gives statistical support to previous observations based on very few cases and provides a statistically reliable answer concerning the entity of FES induced recovery of muscle force in hemiparetic subjects.

Warfarin, aspirin and microembolic signals in the acute phase of stroke in a patient with mechanical heart valves.

We report the case of a 54-year-old woman, with atrial fibrillation and mechanical heart valves, who suffered a left-hemispheric stroke while she was under warfarin. Daily bilateral simultaneous transcranial Doppler monitoring of the middle cerebral arteries showed the presence of microembolic signals (MES), whose number remained unchanged after adding aspirin 100 mg daily. We conclude that anticoagulant plus antiplatelet therapy does not influence the number of MES in the acute phase of stroke in patients with prosthetic heart valves, suggesting the gaseous origin of MES in these patients.

Migraine with aura and right-to-left shunt on transcranial Doppler: a case-control study.

Right-to-left shunt (RLS), usually due to patent foramen ovale, is a well-established risk factor for ischemic stroke in young patients, while the role of migraine as an independent factor is still debated. We evaluated 44 patients suffering from migraine with aura, and compared them with 73 patients younger than 50 with focal cerebral ischemia, and 50 controls, asymptomatic for cerebrovascular disease, and without a history of migraine. All the subjects underwent bilateral transcranial Doppler with injection of contrast medium in an antecubital vein. The test was performed during normal ventilation and during Valsalva maneuver, recording both the middle cerebral arteries and the basilar artery. Criteria for diagnosing RLS was the presence of at least 3 microbubbles within 15 s from injection. Eighteen out of 44 migraine patients (41%) showed RLS, as opposed to 8 of 50 controls (16%) (p < 0.005). Twenty-six out of 73 patients with cerebral ischemia had RLS (35%). We conclude that the prevalence of RLS in patients with migraine with aura is significantly higher than in normal controls, and is similar to the prevalence of RLS in young patients with stroke. These findings could be helpful in understanding the relationship between migraine and stroke.

Malignant melanoma in fine needle aspirates and effusions. An immunocytochemical study using monoclonal antibodies.

The value of monoclonal antibodies (MAbs) for the immunodetection of keratin, vimentin and two melanoma-associated antigens recognized by NKI/C3 and NKI/Bteb for the diagnosis of malignant melanoma has been previously established on histologic preparations. In the present study, cytologic preparations from 20 fine needle aspirates and effusions from patients with malignant melanoma were evaluated using these antibodies. Twenty of 20 smears were negative for keratin, and 20 of 20 smears were positive for vimentin. Positivity for NKI/C3 was seen in 12 of 12 cases studied, and for NKI/Bteb in 12 of 13 cases. These results indicate that a panel of MAbs consisting of anti-keratin, anti-vimentin, NKI/C3 and NKI/Bteb is useful for a more accurate diagnosis of malignant melanomas on cytologic preparations. The expression of these antigens in melanoma cells in cytologic smears can be a valuable aid in the detection of primary (noncutaneous) and metastatic melanomas by fine needle aspiration.
