Frequencies of blood type A, B and AB in non-pedigree domestic cats in Turkey.

OBJECTIVES: To determine the distribution of blood types and to estimate the proportion of matings at risk for neonatal isoerythrolysis in non-pedigree domestic cats. METHODS: The present survey determined the frequency of blood types in 301 cats from four distinct regions of Turkey. Ethylenediaminetetraacetic acid-anticoagulated blood samples were typed by simple tube and slide agglutination assays. Serum obtained from type B cats and an anti-B solution, prepared with Triticum vulgaris, were used to determine type A and type B blood, respectively. RESULTS: Of the 301 cats typed, 220 had type A blood, 74 had type B and seven had type AB. There was a significant difference (P<0.01) between the locations of the cats, with fewer type B cats in the eastern than in the western parts of Turkey. Risk for the development of neonatal isoerythrolysis due to A-B mismatch was estimated to be 18.6 per cent. CLINICAL SIGNIFICANCE: The overall type B frequency in Turkish domestic cats is high. Thus, untyped transfusions in these cats carry a high risk of life-threatening acute haemolytic transfusion reactions and neonatal isoerythrolysis. It is therefore strongly recommended that blood typing be performed before breeding or transfusing in order to minimise blood type incompatibility risks.

Bochdalek's hernia in an adult with colon necrosis.

The improper fusion of the posterolateral foramina of the diaphragm was first described by Bochdalek in 1848. The true incidence of this congenital anomaly ranges from 1/7,000 to 6%. Although it is a well-known neonatal disease, it can also be diagnosed incidentally in adult patients. Until now, a total of around 100 cases of occult asymptomatic Bochdalek's hernia have been reported. The symptomatic cases are encountered more rarely. Colon necrosis among the symptomatic cases was reported in some cases. We herein report a case of a 21-year-old man with a left-sided Bochdalek's hernia which led to total necrosis of the transverse colon. Following resection and colocolonic anastomosis, the defect on the diaphragm was repaired. After an uneventful recovery period, he was discharged from the hospital on the 10th postoperative day. Being aware of this rarely encountered disease and of the factors for early diagnosis is of utmost importance to decrease mortality and morbidity.

The relationship between antioxidant enzymes and bladder cancer.

Carcinogenesis proceeds through at least three distinct stages - initiation, promotion and progression. Free radicals play an important role in the multistep complex course of carcinogenesis. Urinary bladder has been recognized as a target organ for many carcinogens, including benzidine, beta-napthylamine, 2 napthylamine, 4-aminobiphenyl. Antioxidants have been shown to inhibit both initiation and promotion in carcinogenesis. The aim of presented study was to determine and compare the oxidant and antioxidant status in different clinical stages of bladder cancer and of control groups. Study was conducted in fifty-two (n=52) patients with transitional cell epithelial cancer of bladder and in twenty-four (n=24) healthy adults as plasma and erythrocyte controls. Malondialdehyde levels (4.636+/-1.118, 2.853+/-0.576 / 262.112+/-61.772, 203.788+/-35.340) were significantly higher and erythrocyte glutathione levels (6.272+/-1.708, 7.523+/-1.346) were significantly lower in bladder cancer patients group than in control group. Erythrocyte glutathione reductase and glutathione peroxidase (3.935+/-1.155, 5.481+/-1.626 / 8.729+/-1.614, 12.362+/-1.707) activities were significantly lower in cancer patients. In the other hand, glutathione S-transferase activities (3.100+/-1.177, 1.071+/-0.471) were found significantly increases. We suggest that the values of glutathione S-transferase enzyme activity can be used for a tumor detection approach and even as an indicator of the biological behavior of the bladder carcinoma.

In vitro activity of terbinafine and itraconazole against Aspergillus species isolated from otomycosis.

The minimum inhibitory concentrations (MIC, microg ml-1) of itraconazole and terbinafine against overall 34 Aspergillus isolates from the external ear canals with otomycosis have been determined with M38-P microdilution method suggested by National Committee for Clinical Laboratory Standards (NCCLS). MIC intervals in 48 h determined by taking MIC-2 value of itraconazole (the lowest drug concentration causing 50% inhibition of visible fungal growth) and MIC-0 value of terbinafine (the lowest drug concentration causing 100% inhibition of visible fungal growth) as a basis have been found as follows: 0.125-1 and 0.06-0.5 microg ml-1 for A. niger (22 strains), 0.06-0.25 and 0.06-0.125 microg ml-1 for A. flavus (10 strains), 0.125 and 0.125-0.5 microg ml-1 for A. terreus (two strains). It has been observed that both of the antifungal agents showed an in vitro activity against all Aspergillus species tested.

Titres of naturally occurring alloantibodies against feline blood group antigens in Turkish Van cats.

Seventy-eight Turkish Van cats were examined for alloantibody titres, of which 42.3 per cent had type A blood and 57.7 per cent had type B blood. No type AB cats were found. All type B cats (n = 45) showed gross evidence of agglutinating anti-A antibody, with titres ranging from 2 to 256. Sixty-seven per cent of type B cats had anti-A antibody in their plasma, with titres ranging from 8 to 32. However, 13 per cent of type B cats had plasma alloantibody titres of less than 8 and 20 per cent had titres that were higher than 32. A total of 33 type A cats were also tested for anti-B alloantibody titres in their plasma. Among the type A plasma, gross agglutination at titres of 2 and greater than 2 were determined in 24 per cent and 36 per cent of samples, respectively. Microscopic agglutination was seen in an additional 18 per cent of plasma samples. There was no significant association between gender and plasma alloantibody titres of cats (P > 0.05).

Primary hydatid cyst of vastus lateralis muscle.

Musculoskeletal system is rarely involved by hydatid cyst, the larval form of Echinococcus Granulosus. A 37-year old man was diagnosed as having a cystic mass within his left thigh musculature. Preoperative computerized tomography suggested an unusual location of echinococcus although serology could not confirm the diagnosis. During surgery, a white coloured cyst within left vastus lateralis muscle was enucleated. Pathological examination after removal of the mass revealed hydatid cyst of vastus lateralis muscle. This case emphasizes that hydatidosis should be included in differential diagnosis of any soft tissue mass especially in regions where it is endemic.

Hydatid disease in the breast: a case report.

A case of primary breast hydatid disease observed in a 42-year old female patient is reported. Physical examination, mammography and ultrasonography were the methods used for the diagnosis. The lesion was located in the upper outer quadrant of the right breast. The mass was surgically removed under local anaesthesia. No complications were observed postoperatively. We wanted to emphasize the importance of keeping in mind the possibility of hydatid disease in cystic breast lesions, particularly in endemic regions.

Irreducible inguinal hernia, bowel obstruction, and torsion of testis in a patient with testicular feminization syndrome.

Testicular feminization syndrome, irreducible inguinal hernia, bowel obstruction, and testicular torsion were diagnosed and treated in a 13-year-old girl. Diagnosis of this case in such a complex form in an emergency room is a rare occurrence. Through this case, we want to emphasize the coincidence of inguinal hernia and testicular feminization and the X-linked recessive transmission character of the disease.

Blood type A and B frequencies in Turkish Van and Angora cats in Turkey.

Blood typing of domestic cats has been performed in domestic and purebred cats in various parts of the world and is important in clinical practice in order to prevent neonatal isoerythrolysis (NI) and acute haemolytic transfusion reactions. Prevalence of blood types vary greatly between breeds of cats. Turkish Van and Angora cats are different breeds that originated in geographically distinct regions of Turkey. The present survey determined the frequency of blood types in these Turkish pedigreed cats in Turkey. Ethylenediaminetetraacetic-acid anti-coagulated blood of a total of 113 Turkish Van and Angora cats were examined for blood typing using a slide and tube agglutination assay. Of the 85 Van cats surveyed, 40% had type A, and 60% had type B blood. Of the 28 Turkish Angora cats, 53.6% had type A, and 46.4% had type B blood. No type AB cats were found between both breeds. There was no significant association between blood types and gender of both Angora and Van cats or eye colours of Van cats (P > 0.05). Although these are limited surveys, the overall prevalence of type B cats in these two breeds was very high compared with the results of previous studies worldwide. It appears likely that blood type incompatibilities responsible for feline NI and transfusion reactions are occurring in these breeds. The risk of transfusion incompatibility in Turkish Angora and Van cats was 46.4 and 60%, respectively. It is therefore strongly recommended to breeders and clinicians that blood typing be performed prior to breeding and transfusing cats.

Raman spectroscopy and imaging of beta-carotene in live corpus luteum cells.

Raman spectroscopy has been used to identify and locate beta-carotene within individual living luteal cells. The cells were either freshly prepared or cultured; the latter was incubated in the presence or absence of beta-carotene in the form of enriched bovine high-density lipoprotein. Luteal cells were investigated using several Raman spectroscopic and imaging techniques. These techniques did not give accurate concentration levels of beta-carotene within parts of the cell but illustrated the distribution of the molecule. Freshly prepared luteal cells were found to contain an appreciable concentration of beta-carotene. Over a period of several days, the concentration gradually reduced to a nearly undetectable level; similar results were found for cells cultured in the absence of the beta-carotene. For cells cultured in the presence of beta-carotene, the molecular concentration was maintained for as long as 2 weeks. The Raman spectra of fragmented cells showed that the beta-carotene is predominantly localised in the lipid-rich cell components, with the concentration highest in the microsomal fraction. The Raman imaging techniques revealed that beta-carotene was spread over the entire volume of the luteal cells with higher levels occurring at distinct sites, including the surface.

Comparison of microdilution and disc diffusion methods in assessing the in vitro activity of fluconazole and Melaleuca alternifolia (tea tree) oil against vaginal Candida isolates.

The in vitro activity of fluconazole and Melaleuca alternifolia (tea tree) oil was evaluated against 99 vaginal Candida strains by the broth microdilution and disc diffusion methods. The microdilution method was performed in accordance with NCCLS-M27A guidelines. An investigational method was used for the disc diffusion test. Fluconazole and tea tree oil minimum inhibitory concentrations (MICs) obtained at 48 h tended to increase 1- to 2-fold or remain the same compared to 24 h readings for most of the isolates tested. C. krusei and C. norvegensis had significantly higher MICs and smaller inhibition zones for fluconazole compared to other species. Tea tree oil MICs were found to be similar, in general, for all Candida spp. tested. The geometric mean MIC of tea tree oil for all isolates was 2.2% (range, 0.25-4%) at 24 h and 3.0% (range, 1-8%) at 48 h. Tea tree oil mean inhibition zone diameter was 24 mm (range, 14-42 mm) at 24 h and 15.8 mm (range, 10-35 mm) at 48 h. In vitro activity of tea tree oil against fluconazole-resistant Candida strains was of particular interest. The isolates had similar tea tree oil MICs and inhibition zone diameters regardless of their fluconazole susceptibility profile. Tea tree oil MIC ranges (inhibition zone diameter ranges) were 2-4% (12-21 mm) and 2% (35 mm) at 48 h for C. krusei and C. norvegensis, respectively. These results suggest that tea tree oil MICs of the fluconazole-resistant isolates are comparable to those of fluconazole-susceptible isolates. This in vitro finding is promising for potential use of topical tea tree oil formulations in the treatment of candidiasis due to fluconazole-resistant strains.

In vitro susceptibility testing methods for caspofungin against Aspergillus and Fusarium isolates.

We investigated the relevance of prominent reduction in turbidity macroscopically (MIC) and formation of aberrant hyphal tips microscopically (minimum effective concentration; MEC) in measuring the in vitro activity of caspofungin against Aspergillus and Fusarium. Caspofungin generated low MICs and MECs against Aspergillus, but not for Fusarium. While MICs increased inconsistently when the incubation time was prolonged, MEC appeared as a stable and potentially relevant endpoint in testing in vitro caspofungin activity.

Lipid peroxidation and antioxidant status in maternal and cord blood.

The present study was undertaken to determine the change of blood lipid peroxide and antioxidant status in healthy nonpregnant women (n = 20), pregnant women in the third trimester (n = 20), pregnant women during delivery (n = 26) and fetal cord blood. Plasma and erythrocyte malondialdehyde (MDA) levels were found to be significantly higher and erythrocyte glutathione (GSH) levels were significantly lower in pregnant women in the third trimester than in nonpregnant women (p < 0.02, p < 0.03 and p < 0.001, respectively). The highest plasma and erythrocyte MDA levels and the lowest GSH levels were obtained from the pregnant women during delivery (6.99 +/- 2.35 nmol/ml, 283.20 +/- 43.81 nmol/g Hb, 6.73 +/- 2.34 micromol/g Hb, respectively). Erythrocyte glutathione peroxidase (GSH-P) and glutathione reductase (GSH-R) activities were not different between the groups. Maternal plasma and erythrocyte MDA levels were significantly correlated with cord blood plasma and erythrocyte MDA levels (r = 0.63, p < 0.001, and r = 0.41, p < 0.001, respectively). There was a significant positive correlation in GSH-R and in GSH-P activities between maternal and cord blood erythrocytes (r = 0.81, p < 0.001, and r = 0.79, p < 0.001, respectively). A significant correlation was found between maternal erythrocyte GSH-P and both cord blood erythrocyte GSH-R activities (r = 0.74, p < 0.001) and cord erythrocyte GSH levels (r = 0.73, p < 0.001). There was also a significant negative correlation between maternal erythrocyte MDA and cord erythrocyte GSH-R levels (r = -0.9, p < 0.001). Our results suggest that lipid peroxidation and antioxidant status may be changed during delivery, and these changes may affect the fetus by creating oxidative stress.

Lipid-based antifungal agents: current status.

Immunocompromised patients are well known to be predisposed to developing invasive fungal infections. These infections are usually difficult to diagnose and more importantly, the resulting mortality rate is high. The limited number of antifungal agents available and their high rate of toxicity are the major factors complicating the issue. However, the development of lipid-based formulations of existing antifungal agents has opened a new era in antifungal therapy. The best examples are the lipid-based amphotericin B preparations, amphotericin B lipid complex (ABLC; Abelcet), amphotericin B colloidal dispersion (ABCD; Amphotec or Amphocil), and liposomal amphotericin B (AmBisome). These formulations have shown that antifungal activity is maintained while toxicity is reduced. This progress is followed by the incorporation of nystatin into liposomes. Liposomal nystatin formulation is under development and studies of it have provided encouraging data. Finally, lipid-based formulations of hamycin, miconazole, and ketoconazole have been developed but remain experimental. Advances in technology of liposomes and other lipid formulations have provided promising new tools for management of fungal infections.

Nystatin LF (Aronex/Abbott).

Nystatin LF (Nyotran) is a liposomal, intravenous nystatin formulation under development by Aronex, under license from the MD Anderson Cancer Research Center, as a systemic antifungal agent against strains including Aspergillus and Candida. Like amphotericin, nystatin is a polyene derivative that binds to ergosterol, a fungal cell membrane component, creating a pore in the membrane and thus killing the cell. Nystatin is an established antifungal agent, but is restricted to topical use as it is ineffective orally and severely toxic when administered iv [187583], [187589]. It has demonstrated good, broad in vitro antifungal activity against clinically relevant filamentous fungi [319465], including fungi resistant to fluconazole and amphotericin B products [264505], [2869821, [287790], 1289522]. The company is also conducting a phase III trial to evaluate its efficacy against cryptococcal meningitis [305531], [334156]. Aronex filed for approval of nystatin LF in Spain in December 1997 [272986] and expected to file an NDA in the US by the end of 1999 1311208], [342003]. However, in an effort to ensure that its US and European filings contained data from the phase III cryptococcal meningitis trial in its entirety, Aronex's marketing partner requested that all the 70-day data be gathered prior to unblinding this study. The filing had initially been based on interim data at the 14- and 21-day endpoint 1344887]. In September 2000, the company anticipated an NDA filing in the US in the fourth quarter of 2001 1382861], 1387947]. In December 1997, Aronex, together with Grupo Ferrer Internacional, filed an MAA in Spain seeking approval for Nyotran for the treatment of systemic fungal infections. Aronex intended to follow the filing with additional filings in other European countries 1272986]. In 1997, a commercialization agreement was signed with Ferrer for Spain and Portugal, with Aronex intending to form other such partnerships throughout Europe and Asia 1248346]. In November 1998, Aronex signed a licensing collaboration with Abbott Laboratories for the worldwide rights to nystatin LF [305531].

[A case of hydatid cyst in the pancreatic head causing obstructive jaundice].

The patient (male; age = 69) was referred to the Department of 1st Surgery upon suspicion of obstructive jaundice. He was admitted to the Department of Internal Medicine with back pain, loss of appetite, and jaundice and some laboratory tests were performed. He has no history of alcohol use. During his physical examination, pain was noted in the upper right quadrant and the gall bladder was palpable and hydropic. A 22 mm. cystic structure with smooth contours was detected in the abdominal CT scan. An exploratory laparatomy was planned. During the perioperative cyst aspiration, the laminar membrane of hydatic cyst led to the probable diagnosis of hydatic cyst. Cholecystectomy, choledochoduodenostomy, partial cystectomy and capitonnage was performed. The patient was started on 800 mg/day albendazole during postoperative period and then was discharged.
