[Bile-dependent gastrointestinal dyspepsia].

"Indigestion" caused by organic and functional alterations in the biliary ways was examined. The difficulty of accurately identifying this form of dyspepsia, whose frequency explains the interest it arouses, is emphasised. Dyspepsia attributable to dyskinesia of the biliary ways and post-cholecystectomy syndrome is specifically analysed. After a review of the results obtained by the medical and surgical treatment of biliary dyspepsia, a more accurate nosologic and pathogenetic classification of the disease is recommended, together with a more detailed definition of the action mechanism of the biliary acids.

[Jejuno-ileal latero-lateral bypass. Defects of mechanical sutures].

In 6 of our patients operated upon for pathological obesity, complete recanalization of the jejunum closed by a mechanical suture upstreams of the jejuno-ileal latero-lateral anastomosis resulted in failure of the intestinal bypass. We have modified our technique and instead of closing the jejunum with a mechanical stapler we now divide it, then bury and attach together its two ends to avoid torsion and invagination. The operation is completed by creating an anti-reflux system at the terminal end of the excluded ileum.

[Clinical use of percutaneous transhepatic biliary decompression].

Several interesting applications of P.T.C. with biliary drainage are described out of several years' experience with the technique. It was found to be extremely useful for precise preoperative diagnosis of biliary calculus and any associated pathologies. The information provided by biliary drainage is also obviously helpful in selecting the correct surgical approach to this condition.

[Latero-lateral jejuno-ileal shunt in the treatment of massive obesity (author's transl)].

Jejuno-ileal shunting was performed in 67 patients with massive obesity, last ten with a side-to-side jejuno-ileal anastomose. The technique is simple and rapid and carries little risk of sepsis. No complications were observed. This particular operation is one of several surgical treatments proposed for massive obesity and appears to be most satisfactory.

[Pancreatic cystadenoma. Description of 2 cases and review].

On the basis of two personal cases and a review of the most recent literature, the main features of this rare pancreatic condition are reported. Special attention is paid to the post-diagnostic importance of radical exeresis. This is the only way of preventing the patient running the risk of malignant degeneration. The observation of a bisalbuminaemic band and the association with an aberrant pancreatic adenoma were of interest in the two cases described.

[Adenomatous polyposis of the stomach associated with familial adenomatous polyposis of the colon].

A case of adenomatous polyposis of the stomach associated with familial polyposis of the colon is presented. The relevant literature is reviewed and the data thus collected (with the exception of definitely non-adenomatous, extracolic cases) are presented in tables, depending on whether familial polyposis of the colon or Gardner's syndrome was the accompanying form. A systematic account is given of the association and reference is made to simple polyposis of the stomach, and that of the duodenum. Close attention is paid to the question of diagnosis, the risk of cancerous transformation, and treatment. Stress is laid on the importance of optic fibre endoscopy in diagnosis and therapy (by means of the surgical fibroscope). Examination of the literature, it is felt, substantiates the need for: 1) routine systematic investigation of the digestive apparatus in all subjects with FPC and GS, and the report of even isolated cases to establish the true frequency of the association; 2) histological assessment of radically resected formations in the correct determination of both diagnosis and prognosis; 3) preventive and systematic endoscopic control as a means of investigation and postoperative surveillance; 4) correct strategy based on the nature, extent, number and size of the polyps and the polypogenetic capacity of the gastric mucosa.

[Historical development of the surgical treatment of gross obesity].

Reference is made to the experimental preliminary remark and the first, clinical observations underlying the surgical management of severe obesity, together with the techniques employed, starting with the jejunal-colic by-pass of Payne et al. This, however, led to serious liver damage and electrolyte imbalance, whereupon Payne et al. proposed end-to-side and Scott end-to-end jejunal-ileal by-pass. The end-to-end variety has been used with excellent results in 33 cases. Mason's gastric and gastroplastic techniques is till now unfrequently used. An account is also given of experimental ileo-cholecystostomy and biliopancreatic by-pass. In agreement with most other workers, a preference is expressed for the end-to-end jejunal-ileal by-pass as the most suitable technique available at present.
