Extramammary Paget's disease. Arising in knee region in association with sweat gland carcinoma.

We report a case of extramammary Paget's disease (EMPD) arising in the knee region in association with an underlying sweat gland carcinoma. Inguinal lymph node metastases were present initially. Paget's cells, intradermal tumor cells, and metastatic tumor cells showed similar reactions to a variety of histochemical stains and positive results with immunohistochemical staining using carcinoembryonic antigen (CEA). Control sections in cases of Bowen's disease and superficial, spreading malignant melanoma showed a negative staining reaction with CEA, providing further evidence that this technique is useful in distinguishing Paget's disease (PD) from histologically similar conditions. We provide a summary of previous cases of EMPD arising in unusual locations and a brief consideration of possible mechanisms of the origin of PD.

A method of pre-surgical oral orthopaedics.

A preliminary report of a technique of pre-surgical treatment in cleft lip and palate patients is outlined utilizing an adjustable intra-oral appliance with extra-oral strapping. The appliance is constructed with an adjustable spring for expansion and two shelves overlapping in the midline to allow palatal continuity during treatment. Frequently, only one appliance for each patient is required. The techniques of appliance construction, pre-surgical management and surgical repair of the lip are outlined.

Functional analysis of human T cell subsets defined by monoclonal antibodies. II. Collaborative T-T interactions in the generation of TNP-altered-self-reactive cytotoxic T lymphocytes.

In previous reports we have demonstrated that human T cells, responding to soluble and alloantigens, release helper factor(s) that amplify primary in vitro hapten-altered-self-reactive CTL responses. In the present studies, we have employed complement-fixing monoclonal antibodies (OKT4 and OKT8) that recognize functionally distinct human T cell subsets to investigate the role of T-T interaction in the generation of these killer cells. In all experiments, purified OKT4+ responder T cells were deficient in cytotoxic activity, whereas responder populations containing OKT8+ T cells generated substantial cytotoxicity; demonstrating that TNP-altered-self-reactive CTL precursors are contained within the OKT8+ T cell subset. Further, optimal cytotoxic responses were obtained from responder populations containing both OKT4+ and OKT8+ T cells, suggesting that cooperative interaction between these subsets may result in an amplification of killer cell activity. This interpretation was supported by the following observations: (1) the amplifying effect of soluble antigen required the presence of both OKT4+ and OKT8+ responders; (2) during MLC, OKT4+ but not OKT8+ responder T cells generate helper factor(s) that amplify TNP-altered-self-reactive CTL responses; (3) helper factor(s) bypass the requirement for direct OKT4-OKT8 T cell interaction, triggering a CTL response that is proportional to the percentage of OKT8+ T cells present within the responder population. In additional studies, we determined that the TNP-altered-self-reactive effector CTL maintain the OKT3+, OKT4-, OKT8+ surface phenotype displayed by the CTL precursor.

Hapten-specific human T cell lines. I. Functional analysis with respect to cytotoxic and helper activity.

Human PBL cells, sensitized in vitro to TNP or FITC-modified autologous stimulators, have been successfully grown in long-term culture by using TCGF derived from PHA-activated tonsil cells. Long-term cultures consist almost exclusively of T lymphocytes as defined by the formation of spontaneous rosettes with sheep erythrocytes and C-mediated lysis with heteroantibodies to human T cells (R alpha T H). The majority, but not all of these T cells, bear surface Ia antigens as detected by C-mediated lysis in the presence of alpha p23,30. Functionally, these cultures maintain high levels of hapten-specific cytolytic activity during many weeks of culture. In addition, a number of these T cell lines exhibit hapten specific proliferation when co-cultured with x-irradiated hapten modified autologous cells. During this proliferative response, helper factor(s) are released that trigger B cell differentiation into AFC. Data are presented that demonstrate that helper factor(s) production by T cell line cells is preferentially triggered by autologous, but nt allogeneic stimulator cells, bearing the relevant hapten.

Multiple myeloma: presenting as a neurological disorder.

A case is reported of multiple myeloma presenting with signs and symptoms of paraplegia in a patient with a history of hypertension and remote cerebral vascular accident. The laboratory findings of hyperproteinemia and uricemia suggest a protein synthesizing abnormality. This case emphasizes that most patients with protein abnormality should be investigated by protein electrophoresis and immunoelectrophoresis.Unusual clinical presentation of multiple myeloma may result in an erroneous diagnosis unless proper investigation in the appropriate line is made.

An approach to the evaluation of hemostasis.

Effective hemostasis may be adequately and quickly evaluated using tests that are readily available in the usual hospital laboratory. This evaluation includes peripheral smear for platelet morphology and number, platelet count, bleeding time, activated partial thromboplastin time (aPTT), prothrombin time (PT), and clot retraction. Normal results of these tests exclude all but the mildest disturbances of hemostasis.

Pituitary adenomas in pediatric patients: are they more invasive?

Twenty patients with biopsy proven pituitary adenomas that became symptomatic prior to the age of 20 years are reviewed. Based on age, the patients are divided into prepubertal, pubertal, and postpubertal groups. An increased incidence of invasiveness was observed in those patients with onset of symptoms during the pubertal years compared to patients whose symptoms first occurred during the postpubertal years. Other series of pediatric pituitary adenomas in the English literature are reviewed, and ostensibly disparate conclusions are examined.

PulseNet: the molecular subtyping network for foodborne bacterial disease surveillance, United States.

PulseNet, the national molecular subtyping network for foodborne disease surveillance, was established by the Centers for Disease Control and Prevention and several state health department laboratories to facilitate subtyping bacterial foodborne pathogens for epidemiologic purposes. PulseNet, which began in 1996 with 10 laboratories typing a single pathogen (Escherichia coli O157:H7), now includes 46 state and 2 local public health laboratories and the food safety laboratories of the U.S. Food and Drug Administration and the U.S. Department of Agriculture. Four foodborne pathogens (E. coli O157:H7; nontyphoidal Salmonella serotypes, Listeria monocytogenes and Shigella) are being subtyped, and other bacterial, viral, and parasitic organisms will be added soon.
