[Long-term results of M-VAC for the treatment of advanced urothelial cancer].

Total of 37 patients with measurable lesions originating in advanced urothelial cancers received M-VAC (methotrexate, vinblastine, doxorubicin and cisplatin) combination chemotherapy and have been followed for a minimum of 12 months. The study included initial diagnoses of 24 bladder cancers and 13 upper urinary tract cancers in patients whose mean age was 62 years. The patients received a mean of three cycles of M-VAC. Clinical complete remission was observed in five of the 37 patients (13.5%) and partial remission was achieved in 10 patients (27.0%) after mean treatment of 2.4 and 2.1 cycles, respectively, for an over-all objective response rate of 40.5%. The mean duration of response was 11.6 +/- 7.1 months and 4.4 +/- 3.5 months for complete and partial remissions, respectively. A mean of 5.2 +/- 1.7 cycles of M-VAC was given for complete remissions and 3.5 +/- 1.4 cycles in partial remissions. An over-all survival rates after one and two years were 28.125% and 5.859%, respectively. Two of the five patients who had once marked complete remissions died after a mean survival time of 14.5 +/- 4.5 months and three survived with a mean duration of 20.3 +/- 5.4 months. Meanwhile, all of the patients who had achieved partial remissions died after a mean survival of 9.1 +/- 5.4 months except for one patient who survived more than 12 months. These results indicate that M-VAC combination chemotherapy for advanced urothelial cancer is extremely efficacious in initial responses.(ABSTRACT TRUNCATED AT 250 WORDS)

[Inhibitory effect of a new proglumide derivative, loxiglumide, on CCK action in isolated rat pancreatic acini].

The effect of a new proglumide derivative, loxiglumide (DL-4-(3,4-dichloro-benzoyl-amino)-5-(N-3-methoxy-propyl-pentylamino+ ++)-5-oxo-pentanic acid; CR 1505), on binding of 125I-CCK-8 and amylase release stimulated by CCK-8 was investigated in isolated rat pancreatic acini. Loxiglumide inhibited CCK-8-stimulated amylase release and binding of 125I-CCK-8 to rat pancreatic acini in a dose-dependent manner. Loxiglumide caused a concentration-dependent rightward shift of the dose-response curve for CCK-8-stimulated amylase release without altering the maximal response. Schild plots showed a slope of 0.82 and pA2 value of 7.05. The inhibitory effect of loxiglumide on amylase release was reversible. Loxiglumide significantly inhibited amylase release in response to CCK-8, caerulein and gastrin-I. However, loxiglumide had no effect on amylase release stimulated by other receptor secretagogues (bombesin, carbamylcholine, secretion and vasoactive intestinal polypeptide) or by agents bypassing receptors (A23187 and TPA). These results indicate that loxiglumide acts as a potent, competitive and specific CCK antagonist on the pancreatic acini.

Bioassay of plasma cholecystokinin in rat and human: inhibition of protein synthesis prevents the decrease in the sensitivity and responsiveness of isolated rat pancreatic acini to CCK-8.

Isolated rat pancreatic acini were most sensitive and responsive when stimulated directly with cholecystokinin octapeptide (CCK-8) without preincubation. Both the responsiveness and sensitivity of acini to CCK-8 decreased time dependently with prolonged preincubation. When acini were stimulated with CCK-8 following pulse labeling with radioactive leucine, old protein was discharged together with newly synthesized (labeled) protein. However, the sensitivity and responsiveness of these acini for release of labeled protein were primarily reduced with preincubation, indicating that newly synthesized protein was contributing to the time-dependent loss of sensitivity and responsiveness. Then isolated acini were treated with 300 microM cycloheximide for 2 h. This treatment prevented the decreases in the sensitivity and responsiveness of amylase release to CCK-8 stimulation and made these alterations in one series of experiments negligible. Using these acini, a sensitive and specific bioassay for the measurement of CCK in human and rat plasma was developed.

[Retrograde prostatic urethroplasty with balloon catheter].

Retrograde prostatic urethroplasty with balloon catheter was first described by Castaneda et al. in 1987. This procedure is performed using local anesthesia with modified angioplasty balloon catheter. We applied this technique in 5 cases, including benign prostatic hypertrophy in 4 and prostatic cancer with urethral stricture in 1. Improvement in clinical symptoms was seen in 4 patients. Uroflowmetry was performed in 3 patients and flow was improved. Urinary retention recurred a few days after the procedure in one other patient. This was though to be related to the predominant hypertrophy of the middle prostatic lobe. A strong urge to void is commonly present at the time of balloon dilatation. Transient hematuria is seen after the procedure. To prevent the risk of postprocedural incontinence, it is very important to avoid dilating the external sphincter. Further investigation is necessary for the evaluation of long-term results, but this procedure can be performed in poor risk patients since it is less invasive. Hence, retrograde prostatic urethroplasty with balloon catheter is thought to be useful treatment for urethral stricture in poor risk patients.

Clinical application of hair protein glycation in the assessment of blood glucose control and diabetic neuropathy.

Glycation of hair protein was assessed in diabetic patients by the measurement of furosine, which is derived from fructose-lysine, a glycated lysine residue in protein. The level of furosine in 12-cm-long hair which grew over the course of one year was significantly better correlated with the mean values of four determinations of fasting plasma glucose (FPG) and four determinations of hemoglobin A1c, respectively, at the time of hair sampling. The level of glycation in hair, which corresponds to the time taken for hair growth, may represent the mean level of blood glucose during the time corresponding to the growth period. The values of motor nerve conduction velocity and sensory nerve conduction velocity were better correlated with the level of furosine in hair corresponding in the length to 1 year's growth than the levels of FPG and hemoglobin A1c at the time of the determination of nerve conduction velocity. These results suggest that hair glycation may serve as a valuable indicator both of long-term blood glucose trends and of the relationship between diabetic complications and blood glucose.

The effect of fructose on collagen glycation.

The effect of fructose on the formation of advanced Maillard reaction products which have fluorescence and cross-links was investigated. Type I collagen was added to various concentrations of glucose and fructose which were then incubated at 37 C for 4 weeks. Both the level of furosine and the fluorescence intensity increased in direct proportion to glucose and fructose levels and to the duration of incubation. Incubation with fructose produced less furosine but more intense fluorescence than incubation with glucose. These results suggest that fructose in the polyol pathway plays an important role in the formation of advanced Maillard products.

[Evaluation of the efficacy of ceftriaxone in acute suppurative otitis media and acute exacerbation of chronic suppurative otitis media. A comparative study with cefotiam as the control].

In order to objectively evaluate the efficacy and the safety of ceftriaxone (CTRX) using once daily administration of 1 g to cases of acute suppurative otitis media and acute exacerbation of chronic suppurative otitis media, a group comparison study by the envelope method was conducted using cefotiam (CTM) as the control drug (2 g twice daily). The results obtained are summarized as follows. 1. Clinical efficacies evaluated by the committee were 71% in the CTRX group and 86% in the CTM group for acute suppurative otitis media, and 63% and 60%, respectively, for chronic suppurative otitis media. When all cases were considered both groups evidenced a clinical efficacy of 64%, and no significant difference was observed between the 2 groups. 2. Clinical efficacies evaluated by the physician in charge were 65% in the CTRX group and 86% in the CTM group for acute suppurative otitis media, and 72% and 60%, respectively, for chronic suppurative otitis media. When all cases were considered efficacies were, respectively, 70% and 64%, showing no significant difference between the 2 groups. 3. Bacteriological efficacies were 88% in the CTRX group and 86% in the CTM group for acute suppurative otitis media, and 74% and 62%, respectively, for chronic suppurative otitis media. With all cases bacterial eradication rates were, respectively, 76% and 67%. Bacterial eradication rates were always higher for the CTRX group than for the CTM group, but the difference was not significant between the 2 groups. 4. Against infections caused by Staphylococcus aureus alone, CTRX showed equal clinical and bacteriological efficacies to CTM. 5. As side effects, dermatitis, vomiting, and malaise were observed in 5 cases (4%) of the CTRX group and 3 cases (3%) of the CTM group. As clinical testing abnormalities, elevations of GOT, GPT, and Al-P, and thrombocytopenia were noted only in 3 cases (5%) of the CTRX group. Furthermore, all of these abnormalities were temporary and of moderate degree or mild, thus the safety of either drug was considered high. 6. Clinical utilities were 71% in the CTRX group and 86% in the CTM group for acute suppurative otitis media, and 72% and 62%, respectively, for chronic suppurative otitis media. When all cases were included, they were 72% and 66%, respectively, and there was no significant difference between the 2 groups. It is concluded from the above results that CTRX is a highly useful drug with once daily administration of 1 g in the treatment of suppurative otitis media.

Skin blood flow in diabetic patients during cold loading.

We measured periflux blood flow (PBF) in people's fingers and its response to cold loading with a laser Doppler flowmeter. As an index in the cold loading test we used cold recovery time (CRT). Among normal people, CRT increased with age. Diabetics had significantly longer CRTs than did age-matched controls. CRT seems to be useful for evaluating the skin microcirculation in diabetics and the aged.

[Effect of palmitate and phorbol ester on synthesis of phosphatidylcholine in isolated guinea pig gastric glands].

To better understand the mechanism of phosphatidylcholine synthesis in the stomach, [3H] choline incorporation into phosphatidylcholine in response to agents which have been shown to induce phosphatidylcholine biosynthesis in other tissues was examined using isolated guinea pig gastric glands. Palmitate and 12-O-tetradecanoyl phorbol 13-acetate (TPA) which has been shown to activate protein kinase C directly, stimulated [3H] choline incorporation into phosphatidylcholine in gastric glands, by 189 +/- 12.9%, and 129 +/- 10.4% of control, respectively (n = 4, p less than 0.01, p less than 0.05). On the other hand, dibutyryl cyclic AMP had no effect on the incorporation. When the glands were pulsed with [3H] choline followed by incubation in the presence of palmitate and TPA for 180 min to see the effects of the agents on the limiting step of the phosphatidylcholine synthesis, phosphatidyl-[3H] choline was increased to 167 +/- 7.5% and 142 +/- 7.5% of control respectively (n = 4, p less than 0.01, p less than 0.05). In parallel to the increase in phosphatidylcholine synthesis, phosphoryl-[3H] choline in the glands incubated with palmitate and TPA was decreased as compared with control. These results suggest that palmitate or TPA may stimulate phosphatidylcholine biosynthesis through the activation of cytidylyltransferase in the stomach.

Effect of aminoguanidine on the glycation.

3-Deoxyglucosone, a carbonyl intermediate compound in the Maillard reaction, acts on bovine serum albumin to increase its fluorescence. Aminoguanidine inhibited the increase of fluorescence intensity formed by bovine serum albumin and 3-deoxyglucosone when 3-deoxyglucosone had been preincubated with aminoguanidine. These results suggested that aminoguanidine inhibits the action of 3-deoxyglucosone in the Maillard reaction.

Effect of hyperglycemia on hepatic extraction of insulin in totally pancreatectomized dogs.

Using the glucose clamp technique, we investigated the effect of blood glucose levels on hepatic extraction of insulin in pancreatectomized dogs. A primed continuous infusion of regular insulin was administered at a rate 1.5 mU/kg body wt./min into the portal or peripheral vein and the blood glucose level was clamped at hyperglycemia (over 200 mg/dl) or euglycemia (below 100 mg/dl) by a variable infusion of glucose through the saphenous or portal vein. Plasma insulin concentrations were measured during portal or peripheral insulin infusion, as were the metabolic clearance rates of insulin during insulin infusion via each route. Hepatic extraction of insulin was calculated from the two metabolic clearance rates using the insulin kinetic model (Pilo' method). Hepatic extraction of insulin during the hyperglycemic clamp was 45.8 +/- 2.5%, significantly higher than that during the euglycemic clamp (35.7 +/- 2.8%: p less than 0.01). When glucose was infused into the portal instead of the peripheral vein during the euglycemic clamp, hepatic extraction of insulin increased (44.7 +/- 3.4 vs 37.6 +/- 2.9%: p less than 0.05). It was concluded that hyperglycemia increases the hepatic extraction of insulin. Moreover, portal hyperglycemia during the euglycemic clamp also increases the hepatic extraction of insulin. It is suggested that portal glucose concentration may affect the hepatic extraction of insulin.

[Evaluation of allergen-specific IgE antibodies by a newly developed mast allergy system].

MAST, which stand for multiple antigen simultaneous test, uses enzyme-linked anti-human IgE and chemiluminogenic substate to determine IgE. This system is characterized by simultaneous analysis of multiple allergen items, up to 35, together with total IgE determination. We evaluated usefulness of this MAST system using 191 serum samples obtained from patients with bronchial asthma, allergic rhinitis and/or atopic dermatitis. It was found that there were statistically significant correlations between IgE antibody quantification by MAST and RAST in 24 out of 35 allergen items, with correlation coefficients more than r = 0.60. These included Dermatophagoides farinae and pteronyssinus, Japanese cedar pollen, orchard grass, Alternaria, Candida as inhalant allergens; egg white, milk, soybeans, wheat and rice as food allergens. It was also evaluated how many allergen-specific IgE antibodies could be detected in one serum sample. More than six allergen-specific IgE antibodies were simultaneously detected in 33% of 191 cases, indicating the importance of multiple-allergen analysis. These results indicate the clinical usefulness of the MAST allergy system in detecting IgE antibodies in allergic subjects.

[Effect of continuous epidural infusion of morphine for postoperative analgesia on pituitary-adrenocortical function].

Plasma levels of cortisol, ACTH and beta-endorphin like immunoreactivity (beta-ELI) were measured to evaluate postoperative pain relief with epidural morphine and systemic analgesics in conjunction with endocrine functions in 16 patients who underwent gastrectomy. Eight of these patients (epidural morphine group) obtained postoperative analgesia with continuous epidural infusion of morphine with a pump as in our previous report. A bolus of epidural morphine was administered through an indwelling thoracic (Th8,9) catheter at 3 hrs prior to the proposed end of the surgery, which was followed with continuous epidural infusion of morphine at a rate of 0.167-0.042 mg.hr-1 with a pump (CADD-PCA, Model 5200P, Pharmacia) during and after anesthesia and surgery with gradual decrease in dose until the third postoperative day. The remaining eight patients (systemic analgesics group) repeatedly received systemic pentazocine and buprenorphine when needed. Plasma cortisol levels increased significantly at the end of surgery and after in both groups. However plasma concentrations of cortisol in the epidural morphine group were significantly lower than those in the systemic analgesics group on the first and second postoperative days. Plasma levels of ACTH and beta-ELI increased significantly at the end of surgery but returned to levels of the previous day in both groups postoperatively. Our study suggests that continuous epidural infusion of morphine is adequate for postoperative pain relief and has suppressing effect on plasma cortisol levels as compared with systemic analgesics regimen.

3-Deoxyglucosone, an intermediate product of the Maillard reaction.

The Maillard reactions between proteins and glucose or fructose, in 0.2M phosphate buffer (pH 7.4) at 37 degrees C, were investigated and the following results were obtained: 1) 3-deoxyglucosone (3DG) is formed as the major carbonyl intermediate; 2) fructose has a higher reactivity than glucose and 3DG is formed also from fructose without involvement of amino groups; 3) 3DG is a crosslinker responsible for polymerization of proteins; 4) 3DG is involved in the formation of a fluorescent advanced-stage product named "Peak L1" in vitro and in vivo.

[Antitumor effect of IL-2 entrapped in liposomes on rat hepatoma, AH-66].

The human recombinant interleukin 2 (IL-2) was entrapped in liposomes, and the therapeutic effects of the liposomes containing IL-2 (Lip-IL-2) were experimentally studied using the rat hepatoma strain, AH-66, maintained in donryu rats and challenged subcutaneously in the inguinal region. The peri-tumor injections of Lip-IL-2 (15 X 10(4)/kg units for the IL-2 dose) significantly inhibited the tumor growth as determined from the relative mean tumor weight, although no therapeutic effects were observed when the unentrapped IL-2 or liposomes containing saline was administered rats in the same way as the injections of Lip-IL-2. They also prolonged the survival time of rats. The studies of serum IL-2 values after i.v. or s.c. injections of Lip-IL-2 revealed that IL-2 was released gradually from the liposomes containing IL-2 into the circulation. As the result of the tumor tissue staining of the immunoperoxidase 18 hrs after the peri-tumor injection of IL-2, it was shown that a number of macrophages infiltrated into the tumor tissue and degenerated tumor cells were observed adjacent to those macrophages. It is suggested that Lip-IL-2 is useful as an antineoplastic agent in the immunotherapy and that the therapeutic effects of Lip-IL-2 would be related to both the slow release of IL-2 and the cytotoxicity on the tumor cells mediated by the macrophages.

Familial hypercholinesterasemia.

A case of familial hypercholinesterasemia was presented. Cholinesterase isoenzyme study revealed the extra C5 band nearer to the cathode than C4 on the gradient polyacrylamide gel electrophoresis in 6 out of 8 subjects in the family. No significant difference in the effect of inhibitors and activator on serum cholinesterase activity of the family with hypercholinesterasemia was found compared to that of the normal control. It was considered to be clinically useful for differential diagnosis of patients with elevated serum cholinesterase to find subjects with familial hypercholinesterasemia.
