Blepharoclonus provoked by voluntary eye closure.

A patient of 23 years of age is presented who, on recovery following a severe head injury, developed abnormal involuntary movements of the eyelids. This consisted of intermittent blepharoclonus which recurred repetitively on voluntary, but not forced, eye closure. The possible genesis of this disorder will be discussed.

Traumatic neuropathy of second cervical spinal nerves.

The second cervical spinal nerves are unduly vulnerable to forcible approximation of the arches of the atlas and axis and to excessive rotation of the atlas on the axis. Sequelae of such injury include sensory aberrations ranging from loss of feeling to severe neuralgia and disorders of balance. Diagnosis of second cervical neuropathy may be difficult when there are multiple injuries to the cervical spine, but most cases clear up spontaneously within one to three years.

Migraine as a sequela of blunt head injury.

Evidence that a blunt head injury can engender a migrainous state in susceptible individuals is provided by a series of 13 patients who developed recurring attacks of classic migraine after suffering such injury. Among a group of patients with post-traumatic headache of more than one year's standing, the preponderant type of headache was found to be of the migrainous variety. The significance of these findings is discussed.

Hostility to kith and kin.

Ten male patients who shared a similar behaviour disorder have been seen. Their behaviour was characterized by extreme hostility towards close members of the family without aggressiveness towards others. The first manifestations of an increase in hostility may be acts aimed to injure of denigrate the spouse or close members of the family. The dynamics of Capgras's syndrome may be similar. This behaviour disorder must be recognized as one of many causes of conjugal conflict.
