Radiotherapy for desmoplastic fibroma of bone: a case report.

Desmoplastic fibroma is a rare benign tumour of bone. Diagnosis is not easy and is often made by excluding other tumours. Histopathological diagnosis of this tumour is also sometimes not easy. The treatment modalities for this tumour are non-uniform and often controversial. In the present case surgical options were left aside because the patient did not consent to surgery, so radiotherapy was used, with success at 3-year follow-up. This case is presented here along with a review of relevant literature.

Surgical management of the severely displaced supracondylar fracture of the humerus in children.

We reviewed 44 children with a widely displaced supracondylar fracture of the humerus (Gartland grade III) treated with primary open reduction and cross pinning. The average age was 8 years and the mean delay in presentation was 34 h. Comminution of the medial supracondylar pillar was seen in 57% of the cases. After treatment, the range of the elbow motion was restricted in eight patients. Cubitus varus was not seen. There was no deep infection or myositis ossificans. Post-operatively, five children had a temporary nerve palsy. According to Flynns' criteria, 42 patients had a satisfactory outcome.

Elbow arthropathy in hemophilia.

Twenty-one affected elbows in 14 known hemophilic patients undergoing treatment at the Hemophilia Clinic of our institution between 1994 and 1997 were evaluated using the clinical evaluation score of the Orthopaedic Advisory Committee of the World Federation of Hemophilia and radiological examination using the Pettersson score. The mean age of patients was 16.2 years (range 10-25 years), and all patients except 1 had severe hemophilia with factor VIII levels less than 1% of normal. The mean duration of disease at the time of presentation was 12.8 years. The mean clinical score was 6, while the mean radiological score was 4.9. There was a positive correlation between the clinical and radiological scores (P < 0.0001). However, the clinical and radiological scores did not correlate with either duration of disease or joint bleeding score. Two new radiological signs of obliteration of the capitello-lateral epicondyle groove with rounding off of the lateral aspect of the distal humerus and lipping of the medial trochlear surface were also recognized in hemophilic arthropathy of the elbow.

Subtalar arthrodesis for flexible hindfoot deformities in children.

Flexible hindfoot valgus deformities in 16 patients (20 feet) with poliomyelitis and 10 patients (16 feet) with cerebral palsy were surgically treated by a modified Dennyson-Fulford operation using a fibular dowel and a screw. The correction achieved was assessed clinically and radiologically after a mean follow-up of 3.6 years. Overall good results were achieved in 86%. There were no cases of under- or over-correction, pseudarthrosis or screw failure. The procedure is suitable even for spastic planovalgus deformities. Complications related to screw position can be avoided by following the recommendations given here. Use of fluoroscopy and cannulated screws facilitate quick and easy completion of the procedure.

Bipolar hip arthroplasty in ankylosing spondylitis.

Nineteen severely affected hips in 12 young patients with ankylosing spondylitis were treated with bipolar hip arthroplasty followed by a comprehensive postoperative rehabilitation programme. Additional operative procedures of adductor tenotomy, knee flexion release, supracondylar femoral osteotomy for genu valgum and tendo Achillis lengthening for equinus contracture were required in 13 limbs. After a mean follow-up of 45.8 months, results were good in all cases with respect to relief from pain, and there was significant improvement in posture and function. All the patients resumed employment. The average gain in sum total range of hip movements was 194 deg, and the mean preoperative Harris hip score of 27.8 improved to 82.6 after bipolar arthroplasty. Ectopic ossification did not develop in any case. Bipolar hip arthroplasty appears to be ideally suited for young patients with ankylosing spondylitis.

A radiological sign in pulled elbows.

A new radiological sign of distal shift of the radius compared to the ulna is described in the condition of pulled elbow. This measurement has been termed the proximal radial length and was altered in 21 out of 25 cases of pulled elbow diagnosed clinically. The normal relationship was restored after manipulation.

Bipolar hemiarthroplasty in femoral neck fractures.

Thirty-two elderly patients with a femoral neck fracture treated by bipolar hemiarthroplasty and 36 patients (matched for age) with an Austin-Moore hemiarthroplasty were followed-up and compared. Bipolar replacement resulted in a higher percentage of satisfactory results, less postoperative pain, greater range of movement, more rapid return to unassisted activity, fewer unsatisfactory results and no acetabular erosion. The device functioned as bipolar in all the cases studied for inner-bearing motion.

Percutaneous pinning for radial neck fractures.

Eight patients with angulated radial neck fractures were successfully treated by percutaneous reduction and pinning. The operative technique is simple and is not associated with any important complications. This technique may however not be suitable for radial neck fractures with major translocation.

Classification and treatment of volar Barton fractures.

Results of treatment in 78 patients with volar Barton's fractures have been analysed. On radiological assessment these fractures were divided into three distinct categories and this classification is related to the results of closed manipulation. Results are best when this fracture is treated with open reduction and internal fixation. In non-operatively treated cases results are much better if the articular step is less than 2 mm.

Rotatory dislocation of the second carpometacarpal joint: case report.

Isolated dorsal dislocation and rotatory dislocation of the second carpometacarpal joint is rare. A case of rotatory dislocation of this joint is presented, where reduction was blocked by entrapment of the extensor carpi radialis longus tendon in the joint. A roentgenographic technique for evaluating dislocation of carpometacarpal joints is also described.
