Perinatal hematological profile of newborn infants with candida antenatal infections.

We serially studied hematological profiles of 8 newborns with proven antenatal candida chorioamnionitis and funisitis. Candida albicans was cultured from various sources in the infants; blood and CSF cultures were all negative. Seven of the 8 were premature; 5 were sick. We statistically compared hematological data of the 'well' and 'sick' babies. The latter showed neutrophilia (mean 52,381 vs. 11,326/microliters, p less than 0.01) accompanied by significant elevation of nonsegmented granulocytes (NSG) and a NSG/neutrophil ratio greater than 0.3. Significant monocytosis (mean 7,836 vs. 2,869/microliters, p less than 0.01) and eosinophilia (mean 4,723 vs. 782/microliters, p less than 0.05) were also noted. Remarkable toxic granulations, vacuolization, and Döhle bodies were found in the neutrophils of the 'sick' newborns. Hematological profiles correlated with the infants' clinical condition.

Clinical and laboratory findings of spotted fever in Israeli children.

In a prospective study of 70 Israeli children with spotted fever the major clinical features were fever (100%), skin rash (98.5%), myalgia (54%) and vomiting (40%). Thrombocytopenia (75%) and hyponatremia (62.5%) were common, but were not associated with increased mortality. Antibodies to Rickettsia conorii were detected by the indirect immunofluorescent antibody assay. In one patient Rickettsia was grown from blood. Contacts with dogs were reported in 17 of 40 patients questioned, and in only 2 was a canine tick bite obvious. Hospitalization was required in 11 (16%) patients. There was 1 fatality. The rickettsia responsible for spotted fever in Israel appears to be an antigenic variant of R. conorii. Early recognition and treatment of this disease permits rapid eradication of the rickettsiae and facilitates complete recovery.

Nasopharyngeal carcinoma in Israel: epidemiology and Epstein-Barr virus-related serology.

Epidemiological, histological and serological characteristics of nasopharyngeal carcinoma (NPC) were investigated. Included were 25 patients aged 10-70 with male to female ratio 2:1. Among 23 Jewish patients, 18 were of Asian-African (AA) and five of European (Eur) descent; two were Arabs (Ar). The dominant histological type among AA patients was undifferentiated carcinoma (UCNT) and among Eur squamous cell carcinoma (SCC). Elevated IgG and IgA antibodies to Epstein-Barr (EBV) viral capsid, early and nuclear antigens were observed in patients, as compared to 34 healthy controls matched by age, sex and ethnic origin. Although not statistically significant, antibodies to EBV were elevated in AA, as compared to Eur patients. No significant differences in IgG and IgA antibodies to Herpes simplex, Cytomegalo and Varicella-zoster viruses were demonstrated among patients and controls. The study suggests that NPC in Israel, as elsewhere, is associated with EBV and genetic or environmental factors may influence the prevalence of NPC among certain ethnic groups.

Mucosal cell-mediated immunity to varicella zoster virus: role in protection against disease.

By indirect immunofluorescence, enzyme-linked immunosorbent assay, and in vitro lymphocyte proliferation, we studied the antibody and cell-mediated immune response to varicella zoster virus (VZV) in serum, peripheral blood lymphocytes, and tonsillar lymphocytes in 49 children before and after tonsillectomy and adenoidectomy. Among the naturally infected patients seropositive for VZV antibody, most demonstrated VZV-specific proliferation in the peripheral blood and tonsillar lymphocytes, with activity consistently higher in the tonsillar lymphocytes. Several patients seronegative for VZV antibody and without a prior history of clinical chickenpox also manifested VZV-specific proliferation in the tonsillar lymphocytes, and less frequently in peripheral blood lymphocytes. Of these, six children with high levels of activity in tonsillar lymphocytes, with or without high levels in the peripheral blood lymphocytes, failed to develop disease after intimate exposure to VZV in family settings. On the other hand, three other subjects with little or no VZV-specific proliferative activity in the tonsillar lymphocytes developed disease after similar exposure to VZV. These observations suggest the development of VZV-specific mucosal cellular immunity after overt or inapparent exposure to VZV. The appearance of such immunity appears to have a protective role against reinfection even in the absence of detectable serum antibody.

Clinical and virological observations in childhood Hodgkin's disease in Israel.

Antibody levels to Epstein-Barr virus (EBV), cytomegalovirus (CMV), herpes simplex virus (HSV) and varicella-zoster virus (VZV) were determined in children with Hodgkin's disease (HD) before therapy, as well as in healthy, age-matched controls. Included were 21 Israeli-born children, aged 2.5 to 14.5 years: 13 Jews whose parents were of African-Asian origin, 1 Jew whose parents were of European origin, and 7 Arabs. Most of them were from large families (average 5.5 children per family) and low socioeconomic class. Antibody levels were not statistically different between patient and control groups with respect to CMV, HSV and VZV. Geometric mean titer of antibody to EBV viral capsid antigen in HD patients was 100.0 compared with 10.8 in controls. Thirty-three percent of controls were seronegative to EBV, and none had titers greater than or equal to 1:160. All patients but one were seropositive to EBV, and 8 of 20 had titers greater than or equal to 1:160. Among patients, 13 experienced onset of symptoms in the cold season (October to March) and 8 in the hot season (April to September). Onset during the cold season was usually abrupt with acute symptoms, Stage III to IV, involving the mediastinum and neck. Onset during the hot season was insidious, Stage I to II, with the affected area frequently located below the diaphragm. It is suggested that children of Arab and African-Asian Jewish origin with high serum titers to EBV are at increased risk for HD and for seasonal onset-associated clinical presentation.

Antibody response to varicella-zoster virus after natural or vaccine-induced infection.

The development of serum and nasopharyngeal antibody responses to varicella-zoster virus (VZV) was studied in groups of children after naturally acquired varicella or after immunization with the Oka strain of live attenuated VZV vaccine administered in varying doses via respiratory inhalation or subcutaneous injection. Natural infection, subcutaneous immunization, and respiratory inhalation of large doses of VZV vaccine consistently resulted in the development of VZV-specific IgG antibody responses in serum. Although the serum IgG antibody responses persisted for at least eight to 12 months (to date) after either form of infection, the antibody activity appeared to be four- to eight-fold higher after natural infection than after immunization. Transient IgG antibody responses were observed in serum after respiratory inhalation of smaller doses of VZV vaccine. Natural infection, but not VZV vaccine, was associated with the development of serum and nasopharyngeal IgA responses to VZV in most subjects.

Development of serum and intestinal antibody response to rotavirus after naturally acquired rotavirus infection in man.

The temporal characteristics of the response of rotavirus specific IgM, IgG, IgA in serum and secretory antibody in feces to rotavirus were studied in 77 hospitalized patients with rotavirus induced gastroenteritis. The response in serum was characterized by the sequential appearance of rotavirus specific IgM, IgG, and IgA antibody. The IgM antibody appeared to be higher in the acute phase of the disease and was subsequently replaced by the IgG and IgA antibodies. However, the titers of IgG rotavirus antibody in convalescent specimens of serum were found to be statistically significantly lower in patients with severe or prolonged rotavirus infection than in specimens from subjects with mild or moderate disease. Most fecal specimens collected during both the acute and convalescent phase of illness contained virus specific secretory IgA. Higher concentrations of antibody were measured in convalescent samples from patients with prolonged diarrhea and virus shedding. These observations suggest a possible relationship between the severity of rotavirus infection and the nature of systemic and secretory antibody response.

Prevention of varicella zoster (VZ) with poly(I) . poly(C) given during the incubation period.

Poly I . Poly C (Poly I:C) was administered to six children with cancer who were exposed to varicella zoster (VZ) and who were under active treatment to determine if this agent could prevent the development of overt varicella zoster (chickenpox). Poly I:C was administered as a single injection during the incubation period, but beyond 72 hours, since zoster immunoglobulin (ZIG) is a proven effective agent in the first 72 hours. None of these patients had a prior history of VZ and neither ZIG nor any other known antiviral agent was administered to these patients. None of the children developed chickenpox. This appears significant but awaits confirmation in a larger control study.
