Inhibition by somatostatin of the growth hormone, but not corticotropin response to angiotensin II in normal men.

The effect of an i.v. infusion of somatostatin (SRIH) (4.1 micrograms/min/180 min) on angiotensin II (ANG II infusion for 60 min of successively increasing doses of 4, 8 and 16 ng/kg/min; each dose for 20 min)-stimulated growth hormone (GH) and corticotropin (ACTH) release was studied in 7 normal men. In addition, 7 additional normal subjects were tested with ANG II alone (as described above), GH-RH (0.1 microgram/kg body weight as an i.v. bolus) alone or the combination of GH-RH and ANG II. The ACTH response to ANG II was not modified by SRIH infusion; in contrast, the GH response to ANG II was significantly reduced by the concomitant treatment with SRIH. On the other hand, the administration of GH-RH together with ANG II produced peak GH levels comparable to the sum of the individual responses to ANG II and GH-RH, given alone. These findings provide evidence that the stimulatory effect of ANG II on GH, but not ACTH secretion, is under the inhibitory control of somatostatin, suggesting an interaction between ANG II and SRIH in regulation of GH secretion.

[Color Doppler echography in the study of obstruction-related lung collapse].

The typical US pattern of obstructive atelectasis consists in a triangular hypoechoic area with anechoic bands inside related to fluid-filled bronchial structures--the US fluid bronchogram sign. According to some authors, this US sign within a chest mass indicates pulmonary parenchyma disease. Furthermore, it suggests the diagnosis of lung collapse. Sixty-one patients with obstructive atelectasis confirmed with conventional radiography, conventional and computed tomography, and bronchoscopy were submitted to B-mode and color-Doppler US to assess the importance of the US fluid bronchogram sign in obstructive pulmonary atelectasis. In this condition, B-mode US showed tubular anechoic bands in 59/61 patients. Power Doppler venous sampling showed a Doppler spectrum with marked phase oscillations. Arterial sampling showed a Doppler spectrum with high distal impedance-with poor or totally absent diastolic component. To conclude, in the atelectasis area, B-mode US showed in 96% of patients some anechoic bands with no apparent pulsatility. Color-Doppler showed color flow in 100% of cases, which confirmed the vascular nature of the masses. Thus, the US fluid bronchogram, which is frequently described in the literature, was never observed in our series. Power Doppler spectral flow analysis can be useful in the diagnosis of obstructive atelectasis because it depicts the hemodynamics of atelectasis parenchyma. Indeed, the arterial spectrum with high distal resistance is consistent with the effects of hypoxia on intra-atelectatic blood vessels. Further research is necessary to assess the role of color-Doppler US in the hemodynamic study of intra-atelectatic vessels. However, our preliminary results open new perspectives for the acquisition of physiopathologic data on abnormal blood flow in obstructive atelectasis.

Clinical application of genetic polymorphism in neurofibromatosis type 1.

The authors report the study of DNA polymorphic sequences, 5 intragenic and 5 flanking the NF1 gene, in 87 Italian NF1 families for a total of 142 affected individuals and 204 non-affected relatives. All PCR-based analyses are easy and simple to perform, and require small amounts of DNA. The non radioactive method used is sensitive, rapid, and has low background. All subjects were informative for at least 2 markers. The use of linkage study to familial cases allowed us to exclude the diagnosis prenatally in two fetuses, and to confirm or exclude diagnosis in those relatives with clinical signs, but not fulfilling the international diagnostic criteria. Furthermore indirect analysis permitted the detection of large gene deletions by loss of heterozygosity of one or more DNA markers in three out of 47 sporadic cases.

Case report: finasteride-induced gynecomastia in a 62-year-old man.

The authors describe a case of bilateral (with left prevalence) gynecomastia in a 62-year-old man after finasteride treatment because of benign prostatic hypertrophy. Finasteride is an inhibitor of 5 alpha-reductase, the enzyme responsible for testosterone metabolism to dihydrotestosterone. In this patient, nonspecific endocrine alterations were found, except for a significant decrease in dihydrotestosterone levels. In addition, there were no pathologic conditions affecting other organs or pharmacologic treatments that could be responsible for gynecomastia. Drug withdrawal started a progressive reduction of the lumps until complete their disappearance. It is possible that gynecomastia was caused by alterations of estrogen/androgen ratio because of a finasteride-induced decrease in circulating dihydrotestosterone levels. In this article, the authors confirm finasteride antiandrogenic activity and recommend a close follow-up of long-term treatments with finasteride to find out other possible side effects.

766 cases of oral cleft in Italy. Data from Emilia Romagna (IMER) and northeast Italy (NEI) registers.

Epidemiological and genetic variables for oral clefts were analysed for the years 1981-1989 in a case-control study of congenital malformations in the Emilia Romagna, Veneto, and Friuli regions, and in the Trento and Bolzano hospitals. Birth prevalence for all cases of cleft lip with or without cleft palate (CL(P)) was 8.2 per 10,000 births, and that for cleft palate only (CP) was 6.1 per 10,000. Coexisting abnormalities were found in 23% of CL(P) cases and in 43% of CP. No clusters in time or space were detected. For isolated clefts, a predominance of males among CL(P) and of females among CP was found; epilepsy was the only maternal risk factor correlated with clefts, and an association between clefting and consanguinity was found. Empirical recurrence risks were calculated in both isolated CL(P) and CP.

[The percutaneous treatment of breast cysts. A proposed new method].

Percutaneous needle aspiration of symptomatic breast cysts has nowadays replaced surgery. The medical literature reports on the accidental breaking of breast cysts during mammography. The authors report their personal experience with the percutaneous treatment of symptomatic breast cysts by cyst wall breaking. Twenty symptomatic breast cysts were treated with US-guided needle aspiration and 6-7 times more air than the aspirated fluid was injected through the same needle left in situ to break the cyst wall. US follow-up scans were performed 3, 6 and 12 months after the percutaneous treatment. The biggest cyst only recurred, while the extant 19 cysts did not. Our treatment exhibited no risks and US and mammographic patterns normalized over time. Our results (95% recovery rate) and the complete absence of any complication prove the effectiveness of this method.

Somatostatin receptor imaging in small cell lung cancer using 111In-DTPA-octreotide: a preliminary study.

Small cell lung cancer is a common and aggressive disease. Combined multiagent chemotherapy and radiotherapy can improve short-term prognosis, but long-term prognosis remains dim. Somatostatin receptors have been identified on the cellular surface of subsets of this cancer and may be associated with less aggressive evolution. Moreover, medical therapy with somatostatin analogues holds promise for neoplastic growth control. Planar scintigraphy has been performed in 15 patients with histologically proven small cell lung cancer at 4 and 24 h after the intravenous (i.v.) injection of 185 MBq 111In-octreotide (Octreoscan, BYK-Gulden). No short-term adverse effects were recorded; tumour uptake of the radiopharmaceutical was observed in 13 patients at 4 h and in 12 patients at 24 h suggesting more extensive disease than apparent by computed tomography (CT). It is highly likely that the 24 h uptake reflects the presence of somatostatin receptors on the tumour. Previous chemotherapy does not seem to play a key role in tumour visualization. 111In-octreotide is a suitable radiopharmaceutical for in vivo evaluation of somatostatin receptor status of small cell lung cancer. Quantitative scintigraphic methods are needed to investigate nonspecific binding and receptor kinetics.

[The clinical and echocardiographic parameters at rest and during mental arithmetic stress in preadolescent subjects with a hypertensive parent].

Previous studies have showed that a genetic predisposition and/or a family history of essential hypertension may play a significant role in the development of cardiovascular changes in borderline hypertensive patients. Moreover, mental stress has been identified as contributing to the development of cardiovascular disease. The aim of this study was to compare the possible changes in blood pressure and Doppler echocardiographic parameters in resting and during mental arithmetic stress in 21 children with a hypertensive parent (Group I) and 21 age-matched children with normotensive parents (Group II), similar for body mass index, alimentary and life habits, and not differing in resting blood pressure, heart rate and Doppler echocardiographic parameters. Mass index was slightly but not significantly high in Group I with respect to controls. During mental stress only in the children with a family history of hypertension there was a significant increase (p < 0.02) in systolic blood pressure. A positive relation between the increase in systolic blood pressure and end-diastolic thickness of interventricular septum and posterior wall was observed in the same group. In addition a close relation (p < 0.05) between mass index and end-systolic left ventricular stress at rest was evidenced, whereas a negative relation (p < 0.03) between the increase in systolic blood pressure on one side and the early peak diastolic filling velocity (Vmax E) and the ratio of early to atrial peak diastolic filling velocity (Vmax E/Vmax A), on the other side, were found.(ABSTRACT TRUNCATED AT 250 WORDS)

Familial adult amyotrophic lateral sclerosis: report of cases.

We examined 8 cases of familial ALS (amyotrophic lateral sclerosis) in three different families from our province, admitted to our hospital between 1970 and 1989. Clinical criteria for diagnosis were satisfied in all cases; EMG was performed in 6 out of 8 patients. 4 cases showed classical onset and 4 cases bulbar onset. The average age at onset was 65.7 + 10.6 years. The average survival was 19.1 + 9.2 months. In two families two generations were affected, in the other only one. The mode of transmission was found to be autosomal dominant with variable penetrance. Neither environmental nor toxic factors seemed to be involved in the development of the illness. Genetic investigations may help to elucidate the pathogenesis of familial ALS.

Crossed cerebello-cerebral diaschisis: a SPECT study.

A scintigraphic single photon emission computed tomographic (SPECT) evaluation of frontal perfusion alteration was performed in five patients with known cerebellar lesions but with normal supratentorial computed tomographic (CT) or magnetic resonance (MR) scans. A clearly evident asymmetry was found in prefrontal areas in the four subjects with acquired cerebellar damage. The fifth subject, who had congenital left cerebellar hypoplasia, did not show any frontal flow asymmetry. The data support the growing clinical evidence that the cerebellum contributes to the cognitive processes of the frontal lobes and suggest a possible role for SPECT examination in the assessment of functional cognitive impairment in patients with acquired cerebellar lesions.

Comparison of the antihypertensive effect of urapidil and metoprolol in hypertension.

The hypertensive effect of urapidil, a new antihypertensive agent that acts via central and peripheral alpha-adrenoceptors, has been compared with that of metoprolol in 40 patients with mild essential hypertension. Blood pressure was significantly reduced by both drugs, while the heart rate was reduced only after metoprolol. The increases in systolic blood pressure and heart rate caused by three progressive work loads of bicycle exercise were not affected during urapidil, whereas both were reduced by metoprolol. A slight reduction in forced expiratory volume was observed in some patients during treatment with the beta-blocker. There was no case of orthostatic hypotension during urapidil administration, despite its alpha1-blocking action. Side-effects were rare and negligible with both drugs.

[Activity of fenfluramine in obese subjects. Relation between dosage, plasma concentration of the drug and its effect on weight loss].

Twenty-one patients have been submitted to hypocaloric diet and anorexigenic treatment with fenfluramine at variable doses. The weight loss, the plasma level concentration of fenfluramine (F) and norfenfluramine (NF) have been evaluated during a period of about 9 weeks. Statistical analysis demonstrates a significant weight loss and a highly significant correlation between the dose of administered drug and plasma level concentration of F and NF. No correlation instead has been found between weight loss and the dose administered of F, nor between weight loss and the dose administered of F, nor between weight loss and F and/or NF plasma level concentration.

Carpal tunnel syndrome: the role of tridimensional computerized tomography.

The authors evaluated the possibility of obtaining tridimensional images of the fibrotendinous structures of the carpal tunnel and interpreting their diagnostic meaning starting from high resolution axial images of computerized tomography. In all of the cases proposed 3D-CT allowed for a representation of the tendons of the flexor muscles and the transverse ligament of the carpus (when present) in a complete and totally original manner. Of particular importance is the choice of a correct threshold value in order to obtain diagnostic imaging.

De Quervain's disease: diagnostic imaging.

De Quervain's disease constitutes the most frequently occurring stenotic tenosynovitis. The authors have studied which imaging technique allows for the best representation of anatomopathologic modifications. Ultrasound examination appeared to be the most appropriate and advantageous method as it reveals involvement of the sheaths and tendons in the simplest, most complete, and least expensive manner.
