Two-dimensional spectrophotometry and pseudo-color representation of chemical reaction patterns.

The formation of spatio-temporal patterns in layers of chemical and biochemical media is observed and quantified by a two-dimensional spectrophotometer based on a computerized video equipment with high spatial, temporal, and intensity resolution. Images are shown, to some extent, in pseudo-colors. The use of pseudo-colors and three-dimensional perspective representations enhances the perception of the information contained in an image and the inherent order of the spatial structures.

Social stress increases [14C]2-deoxyglucose incorporation in three rostral forebrain areas of the young chick.

The 2-deoxyglucose method and EEG-recordings were used to compare forebrain activities between socially reared chicks, one remaining in the group the other being separated with preserved visual and auditory contact. During 30% of the test period, separated chicks produced distress calls and showed a typical EEG arousal pattern. The 9 Hz band of the EEG spectrum and the duration of distress calling were found to be significantly different from grouped chicks. Increased 2-deoxyglucose incorporation were found in 3 rostral forebrain areas of separated chicks, namely the hyperstriatum accessorium/hyperstriatum dorsale, the lateral neostriatum/hyperstriatum ventrale and the medial neostriatum/hyperstriatum ventrale. These same areas were previously reported to increase metabolic activity in imprinted Guinea fowl chicks, when they heared the imprinting stimulus during the 2-deoxyglucose experiment. It is argued that high metabolic activity in these areas reflects arousal and attention to relevant auditory or visual stimuli.

Selective sacral nerve blockade for the treatment of unstable bladders.

38 patients with severe urge or urge incontinence, who did not respond to conservative therapy, were treated with selective sacral nerve blockade using a local anesthetic (bupivacaine). 6 patients of this group had definite selective sacral denervation with phenol. In 31 patients a urodynamic study was done previous to the sacral nerve block as well as 10 and 90 min after the injection. Within the first 2-7 weeks the success rate was about 70% in regard to bladder capacity and mean volume at first desire to void. On long-term follow-up (greater than 7 months), the success rate decreased to about 16%. Only 1 patient of the phenol group still has complete detrusor areflexia for now more than 2 years. 4 male patients with advanced or locally recurrent bladder tumors had the sacral block because of severe perineal or suprapubic pain. Subjectively their response seemed to be better. According to our experience nonoperative central bladder denervation with selective sacral nerve blockade using local anesthetics or phenol shows no convincing results on long-term follow-up. Since it is a minimal invasive technique, which can be repeated several times, it may be helpful for some patients and offers a chance to bypass major surgical denervation procedures.

Myelolipoma of the adrenal gland. Review of diagnostic problems and surgical intervention.

Myelolipoma is a benign, rare, and hormone-inactive tumor of the adrenal gland. Its characteristic appearance on sonography and computed tomography enables preoperative diagnosis with a high degree of safety and accuracy. Symptomatic or large tumors have to be removed since there is a high risk of spontaneous hemorrhage. Common findings in patients with myelolipoma are reviewed, especially in those with surgically removed tumors.

Blind-ending bifid ureter with an intravesical ectopic orifice.

Blind-ending bifid ureters, as one type of duplications, are a rare congenital anomaly of the urinary tract. Occasionally they present with unspecific symptoms and require surgical intervention. This case of a blind-ending bifid ureter was associated with an intravesical ectopic orifice. Recurrent urinary tract infection necessitated excision of the blind branch and ureteroneocystostomy using the psoas-hitch technique.

[Gastrectomy--interposition of the jejunum. Results, function and histologic changes of the substitute stomach].

Of 31 patients with gastrectomy and isoperistaltic interposition of a long jejunal loop, a follow-up examination was possible in 13 cases. Gastroscopy and biopsy were performed in 10 of these patients. The time between operation and follow-up examination was 3 months to 2 1/2 years. At the time of operation 15 patients were older than 70 years (48.4%) and the 3 postoperative deaths belong to this age-group (9.6%). 10 patients died (32.3%). The various complaints were registered with a questionnaire and show the good function of the substitute-stomach. Once we saw a leakage of the oesophago-jejunal anastomosis and once reflux oesophagitis (3.2%). The laboratory findings showed no remarkable changes. In X-ray studies a distension of the transposed jejunal segment was never seen and the insignificant histological changes do not correlate to the time after operation. Neither muscular wall hypertrophy nor an increase in goblet cells was observed.

[Total stomach wall phlegmon].

The localized gastric abscess and the diffuse stomach phlegmon are called phlegmonous gastritis. This very rare disease can hardly be diagnosed preoperatively. Resection is the method of choice for the stomach abscess. Gastrectomy might be the safest therapeutical concept for total gastric phlegmon.

Percutaneous penile venoablation for treatment of impotence.

Percutaneous penile venoablation with sclerosing agents and coils was performed in 58 patients. Of 104 ablations attempted, 44 of 51 (86%) transpenile, and 46 of 53 (86.7%) retrograde interventions were technically successful. In 40 patients (69%) erectile function improved (intercourse was possible in 21 without additional measures). Initially improved erectile function deteriorated in 24 patients during follow-up (in an average of 6 months); in 15 of these, intercourse was still possible with injection of vasoactive drugs. In 7 patients, repeat venoablation improved erectile function again. Hot contrast medium was slightly superior to sodium morrhuate as a sclerosing agent. In successfully treated patients, the average decrease in venous leakage was 30 ml/min vs 13 ml/min in treatment failures, with considerable overlap between both groups. Only minor complications were observed. We conclude that percutaneous penile venoablation is technically feasible, and safe. Whether the procedure will have a definite role in the treatment of venogenic impotence, however, still has to be determined.

[Bardet-Biedl syndrome: aspects of nephro-urology and human genetics].

Bardet-Biedl syndrome is a genetically heterogeneous autosomal recessive complex of features in which five gene loci have been described up to now. The diagnosis of this rare syndrome is based on the main manifestations hypogonadism, age-dependent increasing obesity and reduction of renal function, age-dependent progressive retinal degeneration with blindness as well as postaxial polydactyly and mental retardation. The life expectancy is short. Problems of early diagnostics, secondary hyperparathyroidism as well as surgical reconstruction of the genitals and kidney replacement therapy are discussed.

Limitations of detection of bone-marrow micrometastasis in prostate carcinoma patients by CK18/PSA immunocytochemistry and PSA RT-PCR.

BACKGROUND: The aim of this study was to prove the reliability of cytokeratin 18/prostate specific antigen (CK18/PSA) immunocytochemistry to detect tumor cells in the bone marrow of patients with prostate cancer (CaP) compared with a control group without CaP. PATIENTS AND METHODS: In bone marrow aspirates of 34 patients with CaP and 11 control patients without CaP, CK18/PSA immunocytochemistry was performed and compared with PSA RT-PCR in bone marrow and blood. RESULTS: 12% of tumors showed a positive staining with PSA and and 17% with CK 18 immunocytochemistry. There was no correlation with the stage or grade of the CaP. False positive results occurred in 2 out of 6 males without CaP and in 2 out of 5 women. 35% of the samples were found to have PSA-mRNA signals in RT-PCR which were neither associated with the histological stage or grade nor the PSA staining in immunocytochemistry. CONCLUSION: False positive staining results were obtained in control patients. Detection of PSA-mRNA was not associated with immunocytochemistry. Neither immunocytochemical nor PCR results were associated with the histological stage of the CaP. Thus, detection of micrometastasis using immunocytochemical methods has to be interpreted with caution and cannot be recommended as a clinical staging procedure.

Spatial attractors in aggregation patterns of Dictyostelium discoideum.

Chemotactic cell motion in aggregation patterns of the slime mould Dictyostelium discoideum is analyzed by a computerized cross-correlation method. In this excitable medium the movement of amoebae is selforganized by signals of cAMP that propagate as rotating spirals or expanding concentric circles (target patterns). A vortex-like rotation of cells is found close to the core of spiral waves, with a maximum velocity of 15 microns/min. Cell motion and spiral tip orbiting follow an opposite sense of rotation. The calculation of streamlines of the most probable trajectories reveals the existence of a closed curve (radius approximately 130 microns) corresponding to the boundary of the spiral core. This spatial-limit cycle attracts the amoebae on rotational paths and leads to the formation of a cell-free disk in the centre of the pattern. In contrast, pacemakers of target patterns organize cell movement in a radial, star-shaped motion, leading directly to the formation of central mounds.
