IgA associated glomerulonephritis.

Immunohistological analysis of 1146 renal biopsies revealed IgA associated glomerulonephritis (IgAGN) in 83 (7.24%) patients (33 children, 50 adults). Clinical features were unusually severe in a high proportion. Nephrotic syndrome (NS) responding poorly to prednisolone was found in 24%, hypertension (HT) in 39%, and azotemia in 34% of patients. NS was slightly more frequent in children than in adults, but HT and azotemia occurred twice as often in adults as in children. Histologically, extensive glomerular crescents and sclerosis were prominent. In addition, moderate arteriolitis and arteriolosclerosis and marked tubulointerstitial nephropathy were notable features. Thus, a low incidence and marked severity characterized IgAGN in this study.

The initiation and organization of microtubules in germinating pear (Pyrus communis L.) pollen.

To study microtubule organization in germinating pear (Pyrus communis L., cv., Bartlett) pollen, we removed the pollen wall by freeze-fracturing before treating the resultant pollen protoplasts by conventional immunofluorescence procedures. Results reveal that axial bundles of microtubules are present in the generative cell of both inactivated and activated pollen grains. Microtubules are not present in the vegetative cells of inactivated pollen, but they are present in the vegetative cells of activated pollen grains. Microtubule nucleation occurs in the vegetative cell cortex. Subsequently, the microtubules grow as branching arrays through most of the vegetative cell cortex except at the apertures where they form localized converging or criss-cross patterns. Eventually, in a germinated pollen grain, the microtubules form network-like arrays through most of the pollen grain and a collar of short arrays at the base of the pollen tube. It is suggested that the role of vegetative cell microtubules in pollen germination is indirect through their mediation of the conformational changes in actin organization that are essential for pollen germination.

Clinical and laboratory profile of systemic sclerosis in northern India.

The clinical and laboratory features of 87 patients with systemic sclerosis (SSc) are described. A large majority of patients presented with diffuse disease with extensive ulcerations and digital resorption. No case of CREST syndrome was seen. The pattern of systemic involvement was generally similar to that reported in the Western literature. Renal involvement appeared to be less aggressive and chronic in its course. A possible aetiology was identified in three patients.

Incidence of reflux nephropathy (RN) in ESRD and effect of nephrectomy in RN on blood pressure and haematocrit following renal transplantation.

One hundred and fifty cases of end stage renal disease (ESRD) on pretransplant workup showed vesicoureteric reflux (VUR) in 21 (14%). Of these, 15 were primary reflux nephropathy (PRN) whereas 6 were secondary VUR. All patients in PRN group showed grade III to IV reflux while secondary VUR ranged between grade I to II. In the PRN group 38.5% had severe, 23% moderate and 23% mild hypertension; 53.3% were nonoliguric and 46.7% were oliguric. Mean plasma renin activity (PRA) was 6.6 and 2.85 ng/ml/hr in PRN and secondary VUR groups respectively. All cases of PRN and 2 from secondary VUR underwent nephrectomy and a month later renal transplantation. Following surgery blood pressure normalised without therapy in 50% and another 50% required lesser drugs in comparison to age matched non nephrectomised controls who showed hypertension in 100% cases, 60% requiring 3 to 4 drugs combination (p less than 0.05). Risk of low haematocrit in nephrectomised patients was insignificant.

Relationship of immunoglobulins with the number & duration of schizophrenic episodes.

The relationship of serum and CSF immunoglobulins with the number and duration of schizophrenic episodes was assessed in 40 schizophrenics. The mean IgA was increased in patients having 3 or more episodes as compared with those having the first episode (P less than 0.001). IgM showed a moderate increase in patients with the second episode and those having 3 or more episodes. IgG/TP per cent (IgG/total protein %) was found to be significantly increased in CSF of patients having the first episode, as compared with those having 3 or more episodes (P less than 0.001). IgG was increased (although insignificantly) in the CSF of patients with illness of five years duration as compared to patients having illness of one year duration or for illness between 1-5 yr. IgG/TP per cent was found to be significantly increased in patients having illness for 5 or more years when compared to patients having illness for less than 1 yr (P less than 0.001) and for 1-5 yr (P less than 0.01).

Systemic lupus erythematosus in North Indian Asians. A prospective analysis of clinical and immunological features.

One hundred and one patients with systemic lupus erythematosus (SLE) from North Indian stock are presented. The clinical manifestations, laboratory parameters, causes of death, and survival are compared and contrasted with the other major reported series. SLE of North Indian Asians has several features comparable to those reported from the West, but other features are more similar to the SLE seen in Mongoloid races.
