Case report: asymmetrical development of the gyri recti presenting as a suprasellar mass: case report and description of six further cases.

Asymmetrical appearances of normal anatomical structures can sometimes mimic pathology. We present a case where asymmetry of the gyri recti was interpreted as a suprasellar mass on axial CT scanning. The true nature of the apparent abnormality was revealed by coronal magnetic resonance imaging (MRI). We also present six further cases where asymmetrical gyri recti were noted as incidental findings on coronal MRI scans. This is a common, but important, normal anatomical variant, which should not be mistaken for pathology.

Odd chromosome movement and inaccurate chromosome distribution in mitosis and meiosis after treatment with protein kinase inhibitors.

Errors in chromosome orientation in mitosis and meiosis are inevitable, but normally they are quickly corrected. We find that such errors usually are not corrected in cells treated with protein kinase inhibitors. Highly inaccurate chromosome distribution is the result. When grasshopper spermatocytes were treated with the kinase inhibitor 6-dimethylaminopurine (DMAP), 84% of maloriented chromosomes failed to reorient; in anaphase, both partner chromosomes were distributed to the same daughter cell. These chromosomes were observed for a total of over 60 h, and not a single reorientation was seen. In contrast, in untreated cells, maloriented chromosomes invariably reoriented, and quickly: in 10 min, on average. A second protein kinase inhibitor, genistein, had exactly the same effect as DMAP. DMAP affected PtK1 cells in mitosis as it did spermatocytes in meiosis: improper chromosome orientations persisted, leading to frequent errors in distribution. We micromanipulated chromosomes in spermatocytes treated with DMAP to learn why maloriented chromosomes often fail to reorient. Reorientation requires the loss of improper microtubule attachments and the acquisition of new, properly directed kinetochore microtubules. Micromanipulation experiments disclose that neither the loss of old nor the acquisition of new microtubules is sufficiently affected by DMAP to account for the indefinite persistence of malorientations. Drug treatment causes a novel form of chromosome movement in which one kinetochore moves toward another kinetochore. Two kinetochores in the same chromosome or in different chromosomes can participate, producing varied, dance-like movements executed by one or two chromosomes. These kinetochore-kinetochore interactions evidently are at the expense of kinetochore-spindle interactions. We propose that malorientations persist in treated cells because the kinetochores have numerous, short microtubules with a free end that can be captured by a second kinetochore. Kinetochores capture each other's kinetochore microtubules, leaving too few sites available for the efficient capture of spindle microtubules. Since the efficient capture of spindle microtubules is essential for the correction of errors, failure of capture allows malorientations to persist. Whether the effects of DMAP actually are due to protein kinase inhibition remains to be seen. In any case, DMAP reveals interactions of one kinetochore with another, which, though ordinarily suppressed, have implications for normal mitosis.

Case report: metastatic transitional cell carcinoma presenting in a urinoma.

We describe a case of transitional cell carcinoma (TCC) metastasising into a urinoma cavity in a patient where the primary tumour arose at the ipsilateral ureterovesical junction. Seeding of transitional cell carcinoma into contiguous structures following surgical intervention has been reported, but to our knowledge, this is the first report of spread into a distant urinoma.

Early repair of an external sphincter ani muscle and rectal mucosal dehiscence.

Little information is available to assist in the management of a dehiscence subsequent to a fourth degree episiotomy repair. Delayed repair of the external sphincter ani muscle and rectal mucosa may result in weeks or months of disability and inconvenience for these women. A series of eight women is presented whose fourth degree episiotomy repair dehiscence was managed by early puerperal repair. The details of wound and bowel preparation, the operative technique, and postoperative care are described. Because all eight women had a successful repair, the authors believe that early surgical correction is an acceptable treatment for this puerperal complication.

Case report of ECT during high-risk pregnancy.

The authors present the case of a depressed woman given ECT during pregnancy and list administrative and monitoring techniques for protection of both fetus and mother when ECT is administered during pregnancy.

Pregnancy and acute optic disc edema of juvenile-onset diabetes.

Acute optic disc edema is a recently recognized, relatively benign manifestation of juvenile-onset diabetes mellitus. Twenty-three cases have been reported in the ophthalmology literature, two occurring during pregnancy. The authors present an additional case recognized in a pregnant patient and review the literature. The disorder manifests physical findings similar to those of papilledema, proliferative retinopathy with optic disc edema, inflammatory papillitis, and ischemic optic neuropathy, but has a benign course that requires no treatment and is not adversely affected by pregnancy. It is important to recognize this syndrome because failure to make the correct diagnosis in pregnancy may lead to inappropriate treatment, including therapeutic abortion.

Subnasal alveolar morphology and the systematic position of Sivapithecus.

Recent collecting in the Potwar Plateau of Pakistan has produced several new maxillae attributable to Sivapithecus. Since the subnasal region is preserved in most of these specimens, comparisons with early Miocene hominoid and Pliocene hominid maxillae become possible. On the basis of these comparisons, it has become clear that subnasal/premaxillary morphology distinguishes Asian and African hominoids. Ramapithecus and Sivapithecus share with Pongo an "Asian" subnasal pattern. The Proconsul species from the early Miocene of western Kenya and Australopithecus afarensis from the Hadar Formation of Ethiopia present two subsets of an "African" subnasal pattern. We think it likely that Ramapithecus and Sivapithecus represent a lineage that postdates the last common ancestor of African and Asian hominoids.

Experimental stress analysis of topographic diversity in early hominid gnathic morphology.

Reconstructing the biomechanics of early hominid mastication is a key element in most models of hominid differentiation. Traditionally, osteological features marking muscle attachment surfaces have served as a reference system from which the vector geometry of the masticatory force system and resultant force distributions could be predicted. To augment traditional morphological and computational approaches, we developed a simulation system capable of replicating human and non-human primate chewing motions. The forces of occlusion are recorded as photoelastic fringes in a urethane alveolar process. Simulation experiments evaluating the functional correlates of topographic diversity in zygomatic root position and mandibular ramus height in early hominids indicated that the mandibles and dentitions of robust australopithecines are well adapted to sustain high magnitude, low gradient load distributions.

Correlates between locomotor anatomy and behavior in two sympatric of Lemur.

Sympatric populations of Lemur catta and Lemur fulvus show major differences in their occupation of canopy forest levels. Lemur catta is usually found in the lowest levels of the forest, and uses the ground for travel over 65% of the time. Lemur fulvus uses the upper levels of the canopy forest and is observed on the ground in less than 2% of all observations. Comparison of hindlimb musculoskeletal anatomy show statistically significant differences in the lengths of the long bones and the height of the talus. Differences also occur in ranges of tibio-talar and mid-talar joint mobility. The details of insertion of the biceps femoris muscle are distinct in the two species as is the relative mass of muscle associated with flexion of the pedal digits, and flexion of th tibio-talar joint. The metric and morphological differences in the hindlimb anatomy of Lemur catta and Lemur fulvus are consistent with differences in habitat and substrate utilization.

Mineral metabolism and microstructural defects in primate teeth.

There are numerous structural defects that occur in primate teeth due to variable calcification during certain growth stages. These interruptions are usually areas of hypomineralization in enamel and dentin which are referred to as Striae of Retzius and Hunter Shreger bands in the enamel or Incremental Lines of von Ebner and Contour Lines of Owen in the case of the dentin. The frequency of occurrence of these variations in mineralization can be related to such factors as growth tempo, dietary quality, state of health, and past disease episodes. Another structure that appears in the dentin is a region that fails to calcify and is referred to as Inter-globular Dentin. Our studies have shown that the microstructural quality of different species' dentition varies widely. Samples obtained from certain fre-ranging cercopithcoids show that these species have the least hypomineralizations while man has the most. Other primate species range between these two extremems with the pongids nearer to man in these characteristics, as previously noted by Sognnaes. Additionally, out initial study shows a great deal of diversity between prehistoric human populations in the microstructural quality of their teeth. We offer the tentative explanation that this variation is due to differences in the calcitying properties of the diet and hence a difference in the general state of their health.
