[Hypertension programs and the risk of ischemic cardiopathy].

OBJECTIVE: To find the risk of ischaemic cardiopathy in a sample of hypertension patients. DESIGN: Crossover study. SETTING: Almanjáyar and Cartuja Health Centres (Granada). PATIENTS AND OTHERS TAKING PART: 202 Hypertension patients included in the programme and belonging to three practices in the above-mentioned centres. MEASUREMENTS AND MAIN RESULTS: We determined the variable of age, gender, overall cholesterol, cholesterol-HDL, TAS, HVI in ECG and we recorded the number of check-ups over the last year, tobacco consumption and diabetes. The 47% of men and 46% of women presented a RCI higher than that of the population as a whole for their age and gender group, in spite of treatment and intervention on these risk factors. Men between 45 and 64 presented higher RCI than women (p < 0.001) and had fewer check-ups. CONCLUSION: We consider that our programme did not sufficiently affect the prevention of ischaemic cardiopathy. It was especially deficient in the monitoring of men between 45 and 64 years. We think that the maintenance of programmes oriented round particular pathologies, instead of integrated programmes, is not sufficient to reduce ischaemic cardiopathy, which clearly has a multiple-cause origin.

Thirty years' experience with biliary tract reconstruction by hepaticoenterostomy and transhepatic T tube.

This study shows long-term results of biliary tract reconstruction in cases of high stenosis of the hepatic duct. Hepaticoenterostomy was performed with transhepatic insertion of a T tube following an original technique. A portion of the horizontal limb of the T tube is placed in the left hepatic duct, the other portion in the duodenum or jejunum, and the vertical limb extends into the right hepatic duct and the right lobe of the liver and is brought out through the abdominal wall. This tube is irrigated daily and removed after 12 months in order to keep the stent in place for a sufficient duration to prevent stenosis. This technique was used in 45 patients who underwent operations from 1954 to 1984. There were 6 postoperative deaths. Of the 39 patients discharged from the hospital, 37 were followed up. Excellent results were obtained in 30 patients who remained asymptomatic after observation from 1 to 14 years. In three patients, results were good, but there were occasional episodes of fever that subsided with antibiotics; one of these patients was observed for 17 years. Results were poor in four patients who developed a recurrent stenosis; three of them underwent reoperations, with satisfactory results. Long-term observation has shown that the described surgical procedure is useful in cases of high stenosis of the hepatic duct, because a stent can be left in place for a year and usually will prevent stenosis at the biliary intestinal anastomosis.

[Total proctocolectomy with ileo-anal reservoir in "J". Experience at the Salvador Zubirán National Institute of Nutrition].

BACKGROUND: Total proctocolectomy with ileal pouch-anal anastomosis (TPCIAA) is the procedure of choice for patients with Ulcerative Colitis and Familial Adenomatous Polyposis. The frequency of presentation of both diseases is low in Mexico, therefore the experience with the surgical procedure is limited. OBJECTIVE: To analyze the operative morbidity and mortality and long-term functional results in a series of patients operated upon with the TPCIAA in a referral center in Mexico. MATERIAL AND METHODS: Retrospective analysis of 44 consecutive patients operated upon from 1987 through 1997. The operation included resection of the anal transitional zone, handsewn anastomosis of a "J" pouch, and diverting ileostomy in all cases. Operative morbidity and mortality, and long-term functional results and complications were determined. RESULTS: Mean age was 33 +/- 15 years. There were 52% women and 48% men. Diagnoses were ulcerative colitis in 59% and familial polyposis in 36%. Global morbidity was 39%, and mortality 2%. With a mean follow-up of 24 months, mean number of bowel movements was 4, 10% of patients had diurnal spotting, 30% nocturnal spotting, and no patient had gross incontinence. Three patients presented pouchitis with adequate response to antibiotics. Two patients presented long-term pouch fistulas that did not require pouch excision. CONCLUSIONS: The TPCIAA is a feasible operation with acceptable rates of morbidity and mortality and satisfactory functional results.

[Carcinoid tumor of the cecal appendix].

In connection with carcinoid tumor of the appendix as an incidental finding in appendectomies, we carried out revision in a period of 5 years, from 1983 through 1987, in the General Hospital of Venezuelan Institute of Social Security in San Felix, Bolivar State. We found two cases of carcinoid tumor of the appendix in 1433 appendixes that were examined by the Service of Pathology. Both cases were males patients who were admitted due to acute appendicitis. They represented 0.14% of the total appendectomies. We review on the subject and we find that the frequency is like to that published up to now. We agree with others authors in that simple appendectomy in the carcinoid tumor of the appendix is curative in tumors < 2 cm.

[Management of the perineal wound in abdominoperineal resection or in proctocilectomy].

The methods of pelvic and perineal drainage and its complications, in 105 cases with proctocolectomy or abdominoperineal resection are analyzed. Those cases with an open perineal wound or partly closed wound using soft or rigid drains showed an average incidence rate of wound infection of 38 per cent (30 to 50 per cent). In the other hand when primary perineal closure and suction drainage were performed, the morbidity rate was 13 per cent. The low incidence of local septic complications diminish the hospitalization time, assure the primary wound healing and there is a low rate of discomfort during the early post-operative period. At the end some technical considerations are made.

[Surgical alternatives in the treatment of chronic unspecific ulcerative colitis].

BACKGROUND: Newer techniques for surgical treatment of chronic ulcerative colitis have been developed. PURPOSE: This review article analyzes the current surgical alternatives, attempting to define their role in the overall management of patients with chronic ulcerative colitis. RESULTS: Non-restorative total proctocolectomy with permanent ileostomy remains as the gold-standard operation for chronic ulcerative colitis. Restorative total proctocolectomy, with or without excision of the anal transitional zone, is becoming one of the most widely used procedures for this disease. It avoids a permanent ileostomy, and permits good functional results in the majority of patients. Subtotal proctocolectomy, with a Hartmann Pouch may be an adequate procedure in patients operated upon on emergency basis, to permit an ileo-anal pouch in a subsequent operation. Colectomy with ileo-rectal anastomosis, and creation of a continent ileostomy are procedures that should be limited to selected candidates. CONCLUSIONS: Currently, total proctocolectomy with ileo-anal pouch seems to be the operation of choice in the majority of patients, but the preoperative general status is important to define the best surgical alternative for each patient.
