Aphasia and neglect after subcortical stroke. A clinical/cerebral perfusion correlation study.

Sixteen patients with unilateral subcortical haemorrhagic or ischaemic stroke, confirmed by CT, were evaluated for the presence of aphasia and neglect. Compared with patients without neuropsychological deficits, left brain-damaged aphasic and right brain-damaged neglect patients showed a significantly greater reduction of cortical perfusion on N,N,N1-trimethyl-N1-(2)-hydroxy-3-methyl-5-(I-123) iodobenzyl-1,3-propanediamine 2 HCl I-123 (HIPDM) and single photon emission computerized tomography (SPECT). These results suggest that major cortical derangement is the crucial factor for the appearance of aphasia or neglect after a subcortical stroke. These remote effects, which are related to the size of the subcortical lesion, are interpreted in terms of interruption of neural connections (diaschisis).

Neurological complications of cerebral angiography.

We report a prospective study of 218 consecutive patients undergoing cerebral diagnostical angiography, before during and 24 hours after the procedure, to identify the neurological complication rate and the risk factors related to the patients and to the procedure. We observed 15 neurologic accidents (6.9%) with permanent sequelae in one case (0.4%). Two risk factors proved to correlate significantly with accidents, i.e. the time that the catheter remained within a vessel and difficulty in performing the procedure.

Syndromes of abnormal muscular activity: overlap between continuous muscle fibre activity and the stiff man syndrome.

Four patients with muscular pain, fasciculations, contractures or cramps are presented. Evidence of peripheral nerve involvement was revealed by electromyography and nerve conduction studies. Muscle biopsy showed mild signs of denervation and reinnervation and, at electron microscopy, dilatations of terminal cisternae were found. All patients showed a remarkable improvement after therapy with diphenylhydantoin or carbamazepine. These clinical, neurophysiological and morphological data underline the role of peripheral nerve pathology in various syndromes of abnormal continuous muscular activity.

A very delayed case of post-irradiation myelopathy with unusual features.

A 48 year old man, irradiated 30 years before for a left laterocervical, lymphadenopathy, developed a progressive weakness of the left arm of two years duration. Myelography and other examinations excluded a compressive spinal process. The clinical and neurophysiological data point to a lesion in the spinal gray matter, probably due to the previous radiation treatment.

Number-Stroop performance in normal aging and Alzheimer's-type dementia.

The number-Stroop paradigm was used to investigate changes in the inhibitory system and in numerical processing in healthy elderly and individuals with dementia of Alzheimer's type (DAT). The size-congruity effect (i.e., relative to neutral trials, incongruent pairs interfere and/or congruent pairs facilitate either numerical or physical comparison) was found in all groups, though the pattern of interference and facilitation varied across them. Overall, the selective attention breakdown was reflected by the increase in interference shown by the older group and the DAT group. On the other hand, the observation of a standard laterality effect andof automatic numerical processing in all groups suggests that access and retrieval of numerical information is relatively resistant to cognitive deterioration.
