No evidence of mutations in the P450 aromatase gene in patients with polycystic ovary syndrome.

BACKGROUND: Etiology and inheritance pattern in polycystic ovary syndrome (PCOS) remain uncertain. Granulosa cells from follicles of women with PCOS have little, if any, aromatase (encoded by the CYP19 gene) activity; follicles contain low levels of estradiol, P450arom mRNA and aromatase stimulating bioactivity. Mice with targeted disruption of the CYP19 gene present cystic follicles. It has been proposed that chronic exposure to high levels of LH, because of aromatase deficiency, determines the development of ovarian cysts. Herein, we investigated if mutations in the CYP19 gene and/or its ovary promoter are causal in patients with PCOS. METHODS: Twenty-five patients with PCOS and 50 control women were studied. PCR analysis of genomic DNA and complete sequence of all exons of the aromatase gene and its ovary promoter were performed. RESULTS: No heterozygous or homozygous mutant alleles were present in any of the patients studied. CONCLUSIONS: In the population studied, mutations of the P450arom gene or its promoter are not the cause of PCOS. However, these findings do not preclude the possible importance of an aromatase disorder in PCOS etiology. Variations in aromatase complex function could play a role in PCOS etiology, but the determinants of such variations might be located in other genes.

A de novo phe671eu mutation in the SRY gene in a patient with complete 46,XY gonadal dysgenesis.

The sex-determining region of the Y chromosome (SRY) gene initiates the process of male sex differentiation in mammalians. In humans, mutations in the SRY gene have been reported to account for 10-15% of the XY sex reversal cases. In this report we describe the clinical, endocrinological and molecular data of a patient with complete 46,XY gonadal dysgenesis caused by a de novo mutation affecting SRY amino acid phenylalanine at position 67 (F67L), located within the highly conserved high mobility group (HMG) box coding region of the gene.

46, XY agonadism associated with adrenal adenoma/myelolipoma: report of a case analyzing Y-chromosome-specific sequences.

A unique patient with 46, XY agonadism associated with adrenal adenoma/myelolipoma is described. The patient was an 18-year-old female with primary amenorrhea, lack of secondary sexual development and an aldosterone-producing adrenocortical adenoma associated with foci of myelolipoma. Molecular analyses of Y-chromosome-specific regions, including automated sequencing of the entire coding region of SRY, the Y-linked testis-determining gene, were performed. Our results excluded the possibility that a mutation in SRY was responsible for this unusual clinical combination.

[Comparative study between estradiol valerate combined with cyproterone acetate, and conjugated equine estrogens combined with medroxyprogesterone acetate as hormone replacement therapy in climacteric].

OBJECTIVE: To compare the effectiveness in the control of climacteric symptoms, the tolerability and the uterine bleeding pattern between two types of hormone replacement therapy. MATERIAL AND METHODS: 81 postmenopausal women between 40 and 60 years age with vasomotor symptoms were studied. Randomly it was administered: I. Conjugated equine estrogens (CEE)-Medroxyprogesterone acetate (MPA): 21 tablets with 0.625 mg of CEE and 10 tablets with 5 mg of MPA which were taken with the last ten tablets of CEE in each cycle (n = 3 7). II. E2V-CPA: 11 tablets with 2 mg of E2V and 10 tablets with 2 mg of E2V and 1 mg of CPA (n = 44). Each cycle comprised 28 days. The treatments were compared with squared Chi and Mann-Whitney U test. RESULTS: The hot flushes presence was significantly greater in the group with CEE-MPA (p < 0.03). Severe throbs at the end of the study had a trend to be more frequent in the group with EC-MPA (p < 0.06). At the end of the study there was a greater frequency of breast tenderness in the group with CEE-MPA (P = 0.009). CONCLUSIONS: Both treatments have good effectiveness, tolerability and same bleeding pattern. Only the frequency of hot flushes, the presence of severe throbs and breast tenderness were lower in the group with E2V-CPA, that makes this preparation an option that can have a better acceptance by the women.

Estrogen therapy for depression in postmenopausal women.

OBJECTIVE: To look at the modification in depressive mood in postmenopausal depressed women after estrogen replacement therapy (ERT). METHOD: Twelve depressed patients divided into two groups of six women each were studied. One group received conjugated equine estrogens (CEE) 0.625 mg/day; the other did not receive any treatment (control group). Mood was assessed in all the subjects at baseline and at 6 months with Hamilton Rating Scale score and considered as depression when it was > 15. Differences between groups were determined by Mann-Whitney U-test, and in each group between baseline and 6-month values with Wilcoxon test. RESULTS: The ERT group had a statistically significant decrease in depressive mood (21 vs. 13 points, P < 0.03), while in the control group no significant change was found. Final Hamilton scale scores were significantly lower (P < 0.05) in those under ERT, when compared with those in the control group. CONCLUSION: Depressive mood decreased after 6 months with CEE, so the prescription of ERT can be useful in postmenopausal women with depressive mood.

Neovagina with cultured allogenic epidermal sheets.

Several materials have been used in neovagina construction, but most of them show some kind of drawback. This is the report of a case in which cultured allogenic epidermal sheets were used instead of skin grafting for a McIndoe procedure.

[Epidemiologic variables in postmenopausal women].

The objective was to know the characteristics of presentation, its clinical aspects and the modification in the diagnostic tools in a selected population of Mexican women with spontaneous menopause. The age at menarche, age at menopause, marital status, age at marriage, number of pregnancies and occupation of 1,099 women with spontaneous menopause were studied. In 619 women which were not receiving nor had received hormone replacement therapy the clinical, laboratory such as glucose, lipids, hormones, bone remodeling biochemistry; and X ray studies such as densitometry and mammography were analyzed. The age average of menarche presentation was at 13 years, and that of spontaneous menopause at 48.1 years 78% were married, with an age at marriage of 23 years, 66% had home duties. The screening tests showed that 30% of patients required cardiovascular evaluation, 40% showed alterations on lipids levels and at least 40% had some alteration on bone remodeling biochemistry or in densitometry. The mammography was normal in 81%. This study showed that most of the data in these group of women were similar to those of other populations, and many of them need intensive surveillance and adequate therapeutic prescriptions to diminish risk factors.

Effect of clomiphene citrate on ovulation after treatment withdrawal.

The effect of clomiphene citrate (CC) on ovarian function in cycles subsequent to treatment withdrawal was studied. Thirty two out of 45 patients with anovulation due to polycystic ovarian syndrome (PCOS) and with no other factor affecting fertility who got pregnant were included. All patients received CC to induce ovulation. Pregnancies in CC treated cycles and after treatment were recorded. Twenty pregnancies were achieved during CC treated cycles and 12 in the cycles after it was withdrawn. In the latter group, eight pregnancies were achieved in the cycle following CC therapy, and four in the second subsequent cycle. In relation to pregnancy complications no significant statistical differences were found between the group of patients who became pregnant during the stimulation cycles and in the cycles after CC.

Prolactin secretion in molar and normal pregnancy.

OBJECTIVE: To describe the relation between PRL and hCG, T3, T4, fT4, TSH, E2 and P4 levels in normal pregnancies and those complicated with GTD. METHOD: Twenty women, 10 with normal pregnancy and 10 with molar pregnancy were studied. The differences between them and the relation between prolactin (PRL) and human chorionic gonadotropin (hCG), estradiol and thyroid hormones, thyroid stimulating hormone (TSH) and progesterone, was determined by Mann-Whitney U-test and Pearson's correlation coefficient, respectively. RESULTS: HCG, estradiol and thyroid hormones were higher in molar pregnancy, but PRL and progesterone were similar and TSH was lower in molar pregnancy. A negative correlation was found between PRL level and all the hormones in normal pregnancy, with the exception of TSH, and in molar pregnancy there was a positive correlation of PRL with all hormones. CONCLUSION: HCG, E2, T3, T4, fT4 levels were higher in the group with molar pregnancy. Differences in correlation analysis suggest different endocrine regulation mechanisms in molar and normal pregnancies.

Endometrial changes according to hormone replacement therapy schedule.

OBJECTIVE: The purpose of this study was to assess differences in endometrial thickness, ultrasonographic characteristics, and histological and bleeding patterns in two groups of women according to the type and length of hormone replacement therapy (HRT) administered. DESIGN: Twenty-seven women were divided into two groups. Group I received oral conjugated estrogens 0.625 mg/day for 21 days, plus chlormadinone 2 mg the last 12 days, for a median length of 6.5 months' time. Group II received oral conjugated estrogens 0.625 mg/day plus chlormadinone 1 mg/day, both continuous and uninterrupted (n = 13), for a median length of 3 months' time. Using transvaginal ultrasound, endometrial thickness, refringence, and the presence of liquid in the uterine cavity were analyzed. An endometrial biopsy was performed the same day and the histological and bleeding patterns were described. Statistical analysis was performed with Mann-Whitney U test, Fisher's exact test, and Spearman correlation coefficient. RESULTS: Results of these tests showed that liquid in the uterine cavity and secretory endometrium were frequently found in those with sequential schedule (Group I); regular uterine bleeding was also frequent in this group. Negative correlation coefficients were found in this group between duration of HRT and endometrial thickness and uterine bleeding patterns, and in the continuous schedule group, between the duration of HRT and uterine bleeding pattern. CONCLUSIONS: We conclude that endometrial and ultrasonographic changes depend on the type of HRT schedule and the duration of therapy.
