Reliability of procalcitonin as a severity marker in critically ill patients with inflammatory response.

Procalcitonin (PCT) is increasingly recognised as an important diagnostic parameter in clinical evaluation of the critically ill. This prospective study was designed to investigate PCT as a diagnostic marker of infection in critically ill patients with sepsis. Eighty-five adult ICU patients were studied. Four groups were defined on the basis of clinical, laboratory and bacteriologic findings as systemic inflammatory response syndrome (SIRS) (n = 10), sepsis (n = 16), severe sepsis (n = 18) and septic shock (n = 41). Data were collected including C-reactive protein (CRP), PCT levels and Sequential Organ Failure Assessment and Acute Physiology and Chronic Health Evaluation II scores on each ICU day. PCT levels were significantly higher in patients with severe sepsis and septic shock (19.25 +/- 43.08 and 37.15 +/- 61.39 ng/ml) than patients with SIRS (0.73 +/- 1.37 ng/ml) (P < 0.05 for each comparison). As compared with SIRS patients, plasma PCT levels were significantly higher in infected patients (21.9 +/- 47.8 ng/ml), regardless of the degree of sepsis (P < 0.001). PCT showed a higher sensitivity (73% versus 35%) and specificity (83% versus 42%) compared to CRP in identifying infection as a cause of the inflammatory response. Best cut-off levels were 1.31 ng/ml for PCT and 13.9 mg/dl for CRP. We suggest that PCT is a more reliable marker than CRP in defining infection as a cause of systemic inflammatory response.

Meropenem in neonatal severe infections due to multiresistant gram-negative bacteria.

Recently, new broad spectrum carbapenem has been investigated on a world-wide scale for the treatment of moderate to severe infections. In the neonatal intensive care units the extensive use of third generation cephalosporins for therapy of neonatal sepsis may lead to rapid emergence of multiresistant gram-negative organisms. We report the use of meropenem in 35 infants with severe infections due to Acinetobacter baumanii and Klebsiella pneumoniae. All gram negative bacteria were resistant to ampicillin, amoxicillin, ticarcilin, cefazoline, cefotaxime, ceftazidime, ceftriaxone and aminoglycosides. Eighty two percent of the cases (29/35) were born prematurely. Assisted ventilation was needed in 85.7% (30/35). All infants deteriorated during their conventional treatment and were changed to meropenem monotherapy. Six percent (2/35) died. The incidence of drug-related adverse events (mostly a slight increase in liver enzymes) was 8.5%. No adverse effects such as diarrhea, vomiting, rash, glossitis, oral or diaper area moniliasis, thrombocytosis, thrombocytopenia, eosinophilia and seizures were observed. At the end of therapy, overall satisfactory clinical and bacterial response was obtained in 33/35 (94.3%) of the newborns treated with meropenem. Clinical and bacterial response rates for meropenem were 100% for sepsis and 87.5% for nosocomial pneumonia. This report suggests that meropenem may be a useful antimicrobial agent in neonatal infections caused by multiresistant gram negative bacilli. Further studies are needed to confirm these results: Meropenem, newborn, sepsis and nosocomial infection.

Amyopathic dermatomyositis associated with cervical cancer.

We here report a 44-year-old female patient who is the first documented case of cervical carcinoma with amyopathic dermatomysitis (ADM). The association of ADM with malignancy is clearly stated in the literature. This patient first presented with classic cutaneous findings of dermatomyositis with no signs and symptoms of muscle involvement for the first 7 months following the diagnosis. When this case has been worked up for malignancy by invasive procedures such as cervical cone biopsy, an increase in muscle enzymes and exacerbation of skin findings were detected.

Color Doppler ultrasonography in ocular Behçet's disease.

PURPOSE: The aim of this prospective study was to assess the ocular hemodynamic changes in patients with Behcet's disease who had involvement of the posterior segment by color Doppler ultrasonography. METHODS: The present study was carried out in 32 eyes of 24 patients with ocular Behcet's disease and 42 eyes of 21 persons as a healthy age-matched control group. In order to detect the hemodynamic changes in ocular Behcet's disease, peak systolic and end-diastolic velocities (PSV, EDV; cm/sec), resistive and pulsatile indices (RI, PI), and PSV/EDV ratio of central retinal artery (CRA), short posterior ciliary artery (PCA) and ophthalmic artery (OA) were determined. RESULTS: The mean PSV and EDV (8.1+/-2.6 and 2.7+/-0.1 cm/sec, respectively) in the CRA were significantly lower in patients with Behcet's disease than in healthy controls (11.5+/-2.2 and 3.5+/-1.0 cm/sec, respectively, p<0.001). For the PCA, these values (12.7+/-4.2 and 4.3+/-1.1 cm/sec, respectively) were significantly lower in the patient group than in the control the group (18.2+/-4.2 and 6.1+/-2.5 cm/sec, respectively, p<0.001). In the patient group, a statistically significant decrease compared to the control group was detected in the mean PSV of the OA (31.8+/-8.2 and 35.8+/-0.6 cm/sec, respectively, p<0.02). However, no significant difference in the other parameters of these vessels was observed between groups. CONCLUSIONS: These results demonstrate the presence of some circulatory changes in the flow velocities of the CRA, PCA and OA in patients with Behcet's disease when compared with control subjects.

Effect of Octreotide on experimental corneal neovascularization.

PURPOSE: To examine the ability of subcutaneously administered Octreotide ( a long acting somatostatin analoque) to serve as an inhibitory agent for corneal neovascularization in eyes of Wistar Albino rats. METHODS: Neovascular growth into the corneas of all the animals was induced by silver nitrate cauterization. Half of the animals which were randomly selected for the Octreotide group received 30 micrograms systemic Octreotide for 7 days. The treatment was initiated on the same day as chemical cauterization. The rest of the animals (control group) received no treatment. Slit lamp and histopathologic examination of the corneas of both groups were performed at the end of the study period. RESULTS: It was observed that the corneal neovascularization and histopatologic scores of the Octreotide group were significantly lower than those of the control group (p < 0.001, p < 0.01). CONCLUSION: Systemic administration of Octreotide inhibits the corneal neovascular response in a rat model.

The comparison of cyclopentolate and atropine in patients with refractive accommodative esotropia by means of retinoscopy, autorefractometry and biometric lens thickness.

PURPOSE: In this study, we aimed to compare the cycloplegic effect of cyclopentolate HCI 1% and atropine sulphate 1% in patients with refractive accommodative esotropia by means of retinoscopy, autorefractometer and the measurement of lens thickness by biometry. METHODS: Thirty-two patients with refractive accommodative esotropia aged from 5 to 10 (mean 6.8+/-1.4), had a deviation under 10 prism diopters, and underwent retinoscopic, autorefractometric and biometric study in dry and wet conditions. RESULTS: The retinoscopic, autorefractometric and biometric findings of the right eye were 5.10+/-1.21 diopter (D), 5.03+/-1.20 D, 3.43+/-0.16 mm with cyclopentolate, and 5.2+/-1.2 D, 5.2+/-1.2 D, 3.4+/-0.1 mm with atropine. In the left eye, the measurements were 5.2+/-1.4 D, 5.1+/-1.4 D, 3.5+/-0.2 mm with cyclopentolate, and 5.3+/-1.2 D, 5.20+/-1.3 D, 3.4+/-0.2 mm with atropine, respectively. When these obtained data were compared by the Student's t test no statistical significance was found (p > 0.05). CONCLUSIONS: We suggest that the cyclopentolate cycloplegia applied to the patients with refractive accommodative esotropia is sufficient to produce good cycloplegia, with an effect similar to atropine.

Penetration of oral and topical ciprofloxacin into human aqueous humor.

PURPOSE: This study was planned to investigate the penetration of ciprofloxacin into aqueous humor following oral and topical application as a prophylactic antimicrobial agent. METHODS: Forty-six patients undergoing cataract surgery were randomly divided into two groups. In the first group, the patients received 500 mg oral ciprofloxacin eight hours before surgery and in the second, 5 drops of 0.3% ciprofloxacin were applied to the patients every twenty minutes, starting 100 minutes before the surgery. By paracentesis, aqueous samples were taken just before the operation so the interval between the first topical application and paracentesis was 100 minutes. RESULTS: The mean concentration of ciprofloxacin in aqueous humor was 0.63+/-0.29 microg/ml in the first group. The concentration was 0.69+/-0.30 microg/ml in the second group. Both of these mean concentrations were higher than the levels of MIC90 of S.aureus , S. epidermidis, P. aeruginosa and Gram (-) bacteriae. CONCLUSION: As a result, both topically and orally applied ciprofloxacin achieved a significant aqueous concentration. Each route studied might be suitable for surgical prophylaxis or treatment of infections.

Anthrax as the cause of preseptal cellulitis.

Anthrax is an infectious disease caused by Bacillus anthracis. It is primarily a disease of domestic animals such as cattle, goats, and sheep; but humans can rarely be infected by contact with infected animals or contaminated animal products. Our case is a 4-year-old boy who was initially diagnosed as preseptal cellulitis, but later he showed the characteristic anthrax lesions with a black necrotic eschar. Scrapings from the necrotic tissue showed gram positive rods and culture grew Bacillus anthracis. The patient responded to intravenous administration of penicillin G, and the lesions resolved, leaving a scar on the right upper eyelid. Eyelid involvement of anthrax is rarely seen in clinical practice, but should be considered in differential diagnosis.

Effect of topical apraclonidine on flow properties of central retinal and ophthalmic arteries.

To investigate the acute effects of topically administered 1% apraclonidine hydrochloride on blood velocity and resistance parameters of central retinal and ophthalmic arteries, the vessels were studied by color Doppler ultrasonography during baseline and at the time of peak effect of apraclonidine in 20 healthy volunteers. After medication, significant decrease in peak systolic, peak diastolic, mean flow velocities and increase in resistive and pulsatility indices of ophthalmic artery were observed. Blood velocity and resistance parameters of central retinal artery didn't change significantly. In conclusion, topically used apraclonidine was thought to change the blood velocity and resistance parameters of ophthalmic artery as a result of its local vasoconstrictory effect on the anterior segment branches of ophthalmic artery.

[The prevalence of hepatitis B in prostitutes working at the Erzurum brothel].

In this study, 101 prostitutes working at Erzurum brothel house, considered as risk group for HBV infection, in 51 (50.5%) of them Anti-HBs, in 9 (8.9%) of them HBsAg, in 4 (3.9%) of them Anti-HBs+HBsAg were found to be positive. The results in various age groups are as follows: In the 23-27 age group 9.5% HBsAg, 42.9% Anti-HBs and 9.5% HBsAg+Anti-HBs, in the 28-32 age group; 13.1% HBsAg and 50% Anti-HBs; In the 33-37 age group 4.3% HBsAg, 52.1% Anti-HBs and 4.3% HBsAg+Anti HBs; in the age group above 38, 5.2% HBsAg, 57.9% Anti HBs and 5.2% HBsAg-HBs were found to be positive.

[The local prevalence of group B streptococcus in pregnant women and newborn infants].

In this study, group B streptococcus (GBS) were investigated in vaginal swabs obtained from 76 pregnant women and in samples of newborn infants obtained from different areas of their bodies. GBS were determined in 5.2% of women and 3.96% of infants. GBS were recovered in 15.4% of the women in their first pregnancy and in 13.3% of women in their second pregnancy. GBS could not isolated from the women in their third and more pregnancies. All eight species of GBS were sensitive to AMP + Sulbactam, Clavulanic acid + Amox, Lincomycin, Clindamycin, Penicillin G and SMZ + TMP.

Low dose cyclosporin A treatment in generalized pustular psoriasis.

Generalized pustular psoriasis is a rare form of psoriasis, seldom seen in children. Three patients with generalized pustular psoriasis are presented, two of whom were a sister and brother and whose grandfather also had pustular psoriasis. Lesions consisted of pustular, erythematous, scaly, follicular papules located on the trunk, scalp, and extremities. The pustules in some areas coalesced to form lakes. Histologic examination of several biopsy specimens revealed the changes of pustular psoriasis, which were parakeratosis, elongation of the rete ridges, and deep spongioform pustules and Munro abscesses. All patients were treated with cyclosporin A for periods of 2-12 months. The doses ranged from 1 to 2 mg/kg/day. Clearance of psoriatic lesions occurred after 2-4 weeks of therapy.

Ocular findings in Sturge-Weber syndrome.

PURPOSE: We reviewed the rare ocular findings of Sturge-Weber syndrome (SWS) and the results of implantation of the Ahmed valve in cases associated with glaucoma. METHODS: Seven patients (range 18 to 52 years) diagnosed as SWS were reviewed as clinical findings over two years. An anterior chamber maintainer was placed in patients with glaucoma to maintain stable intraocular pressure and minimize the risk of intraocular hemorrhage due to sudden pressure changes during surgery, when an Ahmed valve is implanted. RESULTS: Episcleral venous vessels were prominent in all cases. Diffuse choroidal hemangiomas were seen in three cases. Nevus of Ota was observed in only one case. Three cases had juvenile glaucoma. One also had buphthalmos. No intraocular hemorrhage or choroidal effusion was observed intraoperatively in valve-implant patients. The surgical treatment of the patients with glaucoma gave favorable outcomes. CONCLUSIONS: Rare ocular findings such as choroidal hemangioma and nevus of Ota are sometimes seen in SWS. Drainage valve implantation, with an anterior chamber maintainer, is a good choice for treatment when surgery is done in cases with glaucoma. This method may reduce the risk of intraoperative suprachoroidal effusion and expulsive hemorrhage by stabilizing intraocular pressure within normal limits during the surgery.

Large bilateral lateral rectus recession in large angle divergence excess exotropia.

PURPOSE: Classic teaching suggests that surgery for intermittent exotropia should be based on distance/near differences. True divergence excess exotropia should be treated with symmetric lateral rectus recession. The aim of this study was to investigate the effect of large bilateral lateral rectus (LR) recession in large-angle intermittent exotropia. METHODS: Thirty-three consecutive patients with large-angle divergence excess exotropia ranging from 50 to 65 (mean 56.7 +/- 6.3) prism diopters were treated with 8.0 to 9.5 mm (mean 8.8 +/- 0.7 mm) recession of both LR muscles. RESULTS: Successful alignment was achieved in 25 cases (76%) while residual exotropia was seen in eight patients (24%) within the limit of 15 prism diopters. Mean follow-up time was 28.5 +/- 8.4 (range 13 to 38) months. Abduction deficit due to this procedure was not seen in any case. CONCLUSIONS: We conclude that large bilateral LR recession is an appropriate surgical method for large-angle divergence excess exotropia.

Palbebral anthrax.

PURPOSE: Anthrax is a rare infection transmitted to humans from animals or animal products. In cutaneous anthrax it may produce lesions of the eyelids which can lead to cicatricial ectropion. METHODS: We examined three women and five men with anthrax of the eyelids. Intravenous penicillin G was used alone as therapy. RESULTS: All patients presented with a localized itchy erythematous papule of the eyelid. A necrotising ulcer formed in each case, resulting in black eschar. The lesions resolved, with only mild cicatrization of the eyelid in all the patients. CONCLUSIONS: Anthrax is primarily an infectious disease of domestic animals but it is also seen in humans. Eyelids are rarely affected but cutaneous anthrax should be considered in any patient with a painless ulcer or black eschar who has a history of exposure to animals.

Photodisruptive Nd:YAG laser in the management of premacular subhyaloid hemorrhage.

PURPOSE: Premacular subhyaloid hemorrhage is usually a benign condition that generally improves spontaneously and rarely causes visual loss. However, because the hemorrhage may cause permanent macular changes before it resolves, Nd:YAG laser posterior hyaloidotomy may be indicated in selected cases. This study investigated the effects of drainage of premacular subhyaloid hemorrhage into the vitreous with Nd:YAG laser treatment. METHODS: This study was conducted between February 1996 and March 1999. Six patients had a circumscribed premacular hemorrhage in one eye and were treated with the Nd:YAG laser to drain the blood into the vitreous cavity. The hemorrhage originated from Valsalva retinopathy (2 cases), proliferative diabetic retinopathy (2 cases), central retinal vein occlusion (1 case), and blunt ocular trauma (1 case). The size of the hemorrhage is expressed in disc diameters. RESULTS: The mean pretreatment hemorrhage measured 5.7 disc diameters (range 3.5-8.0). Visual acuity in all cases before laser treatment was hand movement. After laser treatment, the hemorrhage instantly drained into the vitreous cavity, resulting in rapid improvement of vision. Drainage was complete within one week and visual acuity improved dramatically. The mean follow-up was 26.3 months (range 7-42 months). No retinal damage or rebleeding occurred due to the laser treatment, and vitrectomy was not required in any eye. CONCLUSIONS: Nd:YAG laser posterior hyaloidotomy may be useful for draining a premacular hemorrhage into the vitreous cavity in selected cases. To establish this as a routine procedure, a randomized prospective study is needed to compare observation, primary vitrectomy, and Nd:YAG laser treatment.

The protective effects of melatonin, vitamin E and octreotide on retinal edema during ischemia-reperfusion in the guinea pig retina.

PURPOSE: The purpose of this study is to provide evidence that free radical damage is a component of retinal ischemia-reperfusion (I/R) injury, and to determine whether melatonin, vitamin E and octreotide can protect retina from this injury. METHODS: The right eyes of 50 male guinea pigs weighing 500-600 g were used. The animals were randomly assigned to group 1 (control), group 2 (I/R), group 3 (melatonin + I/R), group 4 (vitamin E + I/R) and group 5 (octreotide + I/R). Groups 3, 4 and 5 received four subcutaneous injections at six-hour intervals for total dosage of 10 mg/kg melatonin, 150 mg/kg vitamin E and 22 microg/kg octreotide respectively. The first dose of each substance was administered 5 minutes before retinal ischemia. Retinal ischemia was induced for 1.5 hours, then followed by reperfusion for 24 hours. Infections of all three substances were repeated at 6, 12 and 18 hours during reperfusion. The animals were killed at 24 hours of reperfusion. Sagittal sections of 4 microm were cut and stained with hematoxylin and eosin for light microscopic evaluation. The average thickness (edema) of the inner plexiform layer for each eye was measured in sagittal sections near the optic nerve and expressed in microns. RESULTS: The efficacy of each compound had the following relationships: melatonin>vitamin E>octreotide in preventing retinal damage by ischemia-reperfusion. The mean thickness of the inner plexiform layer was 13.3 +/- 0.8 microm, 25.9 +/- 2. 0 microm, 20.0 +/- 0. 7 microm, 21.6 +/- 0.7 microm, 23.9 +/- 0.8 microm respectively in the control, I/R, I/R plus melatonin, I/R plus vitamin E and I/R plus octreotide groups. The thickness of the inner plexiform layer in group 1 (control) was significantly less than the other groups (p<0.001). The inner plexiform layer was thicker in the I/R group than with I/R plus melatonin, I/R plus vitamin E and I/R plus octreotide (all p < 0.01). The inner plexiform layer was thicker in the I/R plus octreotide group than the I/R plus vitamin E and I.R plus melatonin groups both (p < 0.05). Compared to the I/R plus melatonin group, the inner plexiform layer was significantly thicker in the I/R plus vitamin E group (p < 0. 05). CONCLUSIONS: This study demonstrates a protective effect of melatonin, vitamin E and octreotide on the retina during retinal ischemia-reperfusion injury.
