Some particularities of hepatitis B-antigen subdeterminants.

From 200 acute viral hepatitis patients serum samples were collected successively, at various intervals. In 192 patients one single hepatitis B-antigen (HB-Ag) subdeterminant ("ad" in 37.6% and "ay" in 62.4% of the cases) occurred and persisted during the entire course of the disease. The remained 8 patients (4%) showed an alternation of HB-Ag subdeterminants. The possible causes of this alternation are discussed in detail.

Quantitative and qualitative studies on hepatitis B antigen. Titer and subtypes in acute virual hepatitis patients and blood donors.

Hepatitis B antigen (HB Ag) titer and subtype were determined in blood samples obtained from 270 HB Ag-positive acute viral hepatitis (AVH) patients, as well as in 100 HB Ag-positive blood donors. The percentage distribution of the HB Ag titer shows a shift towards lower titers in the case of "healthy carriers". In both AVH patients and blood donors the "ay" antigenic subtype was found to be predominant.

Investigations of HBe antigen. Note I. Prevalence of HBeAg and anti-HBe in the stationary phase of HBsAg-positive acute viral hepatitis.

The presence of HBeAg and anti-HBe was investigated in 1,088 serum samples from 436 patients with HBsAg-positive acute viral hepatitis. The prevalence of HBeAg in the initial serum samples (collected upon hospital admission) was of 37.47%. In view of the results obtained with serial serum samples collected at 1-week-intervals the patients could be grouped into 4 categories: 1) seroconversion; 2) persistence of HBeAg; 3) presence of anti-HBe and 4) other serological sequences, irregular as compared with the former ones. Some serological characteristics point to a similar pathogenic behaviour in the first 3 categories, distinct from that of the patients belonging to the 4th category; the difference is ascribed to deficiencies in the immune response to hepatitis B virus antigens.

Investigations on HBe antigen. Note II. Dynamics of HBsAg in acute viral hepatitis, according to HBeAg persistence or seroconversion.

Serum samples collected at 10-day intervals from 146 patients with acute viral hepatitis were tested for the presence of HBsAg, HBeAg and anti-HBe. During an observation period of 4 months HBe system seroconversion occurred in 70.5% of the the patients; a persistence of HBeAG was recorded in the remaining cases. Lower values of HBsAg prevalence and a shorter persistence of this antigen were recorded in the patients with HBe system seroconversion as compared with those showing HBeAg persistence, and in female as against male patients.

Investigations on HBe antigen. Note III. HBe system and sex-dependent clearance of HBsAg subtypes.

The presence of HBeAg and anti-HBe was tested in 876 serum samples collected at 10-day intervals from 146 HBsAg-positive acute viral hepatitis patients. The clearance of HBsAg was followed up in the patients grouped according to HBsAg subtypes and to HBe system seroconversion or HBeAg persistence. The clearance of HBsAg was found to be more rapid in patients with HBe system seroconversion, in the case of HBsAg/ay and in female patients.

Some remarks on acute viral hepatitis features.

1086 patients with acute viral hepatitis (AVH) including both children and adults were followed up clinically and biologically for 1-1.6 years after onset. Length of hospitalization and time intervals until return of normal laboratory test values were correlated with the presence of HB8Ag. The evolution of HB8Ag positive AVH was found to be more severe and protracted, especially in the case of HB8Ag persistence. The data point to the fact that ambulatory medical surveyance is recommendable in AVH, necessary in AVH type B and compulsory in the case of HB8Ag persistence.
