[Local immunomodulation in atopic and nonatopic children with nonspecific respiratory tract diseases].

The authors report their own results of local immunomodulation with IRS 19 in 57 atopic and nonatopic children having nonspecific chronical diseases of the respiratory tract. The results were obtained on the basis of complex diagnostical procedures before and after local immunomodulation. These results are demonstrated by tables, figures and graphs. By comparing the results obtained with local immunomodulation, it was found that prophylactic and therapeutic efficiency of IRS 19 is better in nonatopic children than in atopic ones. The detailed analyses showed that the local immunomodulation caused not only an increased concentration of the immunoglobulins IgG, IgM, IgA but also of the IgE.

[Immunologic response to microorganisms from a polluted ventilation system in a working environment].

UNLABELLED: Authors point out the morbidity of employees working in ventilation systems contaminated with various microorganisms. They analysed 96 workers exposed to air conditioning system (Group A), and 71 workers (Group B) breathing normal ambient air. The workers of both groups were subjected clinically by functionally and immunologically. Preparation of antigens "MMM" (Monday morning miseries) was used as an original method by Ajello et al. for producing antigens from systemic mycotic agents and subsequently modified. The aim of the present study is to evaluate the possibility of using ELISA in clinical practice for respiratory allergy diagnosis, and especially Hypersensitivity pneumonitis. Atopic status was determined by skin prick tests with common airborne allergens including Dermatophagoides pteronyssinus, ragweed, grasses and Aspergillus fumigatus., by Enzygnost--IgE (Behringwerke AG, Marburg) and for specific IgE by RAST technique (Pharmacia, Uppsala). The skin prick tests were performed with "MMM"-antigens. PEFR (Peak Expiratory Flow-Rate) was measured by using a Wright's peak flow meter. PEFR was recorded on Monday (first day at work) and Friday (the end of the working week). Measured values of PEFR in both groups of employees from Monday to Friday were elaborated by the Wilcoxon test. RESULTS AND DISCUSSION: Culture of scrapings from air conditioning vents were obtained and water from the humidifier system also cultured. They were grown: T. vulgaris, Aspergillus fumigatus, Thermoactinomyces vulgaris and others. Results of questionnaires, clinical evaluation and diagnostical procedures in employees of Group A and B are as follows: Thirty eight workers in Group A had a positive clinical history of "Monday illness". In the symptomatic Group A we found in 8 cases abnormal chest roentgenogram. Further, there was no correlation between the presence of antibodies (ELISA) against MMM and pulmonary function abnormalities, as measured by either spirometry or DLCO. Further, we found good agreement between ELISA and prick test results with antigen MMM. Wilcoxon test showed a statistically significant difference between the two groups (0.01). The median or central value of PEFR reduction in Group A is 10.23 per cent, and in Group B 1.49 per cent. A 30 per cent reduction of PEFR was observed in 5.21 per cent of subjects in Group A. Exposure to ventilation systems contaminated with Thermophilic actinomyces may be responsible for increased morbidity and reduced performance of employees working in air conditioning systems. Particularly the main filter should be checked regularly. Moreover, regular microbiologic examinations of dust and water from air preventing chronic obstructive lung diseases in employees working in areas served by contaminated air conditioning systems.

Immunologic and clinical responses to "Monday morning miseries" antigens.

Authors analysed 96 workers exposed to air conditioning system (Group A), and 71 workers (Group B) breathing normal ambient air. 38 workers in group A had a positive clinical history of "Monday morning miseries". Eight cases with the diagnosis hypersensitivity pneumonitis, acute and chronic form was based on environmental history, clinical investigations, physical examination, Chest-X-ray examination, immunological test "in vivo" and "in vitro" with common allergens and antigen "Monday morning miseries", ELISA, spirometry and PEFR (Peak Expiratory Flow-Rate) measurements. Exposure to contaminated air may be responsible for morbidity and reduced performance of workers.

Scanning electron microscopic studies of the nasal mucosa in normal rabbit and rabbit exposed to alcalase.

The authors investigated the epithelium of the nasal mucous membrane with SEM to enable critical evaluation of alterations induced by the allergic reactions following endonasal application of Alcalase in rabbits. Experimental animals were previously submitted to 6-week intraperitoneal Alcalase sensitization, Biopsy specimens of the mucous membrane of the nasal septum were studied. Our study had confirmed alterations occurring in the epithelium following anaphylactic reaction to Alcalase. The significant morphological changes observed were formation of cytoplasm protuberances in the epithelial cells, and irregular epithelium with widened intercellular spaces, ladin leading to destruction of the respiratory epithelium.

Immune response of rabbits immunized with proteolytic enzymes of B. subtilis.

This investigation was carried out to determine antigenic potency of the proteolytic enzymes: Alcalase, Maxatase and crystalline subtilisin type VII. To proof the antigenicity of these enzymes the precipitating antibodies in serum of immunized rabbits were determined. The titre of precipitating antibodies depended on the mode of immunization, preparation and concentration of antigens. The authors compare the effect of the immunization by subcutaneous and intraperitoneal injections of proteolytic enzymes on the production of serum precipitins in rabbits.

Relationship between the house dust and mite allergen by immunochemical and clinical evidence.

This investigation was carried out to determine antigenic potency in extracts of house dust, D. pteronyssinus and D. farinae and Aspergillus fumigatus with observation of precipiting antibodies in the serum of immunised rabbits. The titre of precipiting antibodies dependens on the mode of immunisation, preparation and concentration of antigens. Our investigations revealed that the house dust antigen comprises antigen of D. pteronyssinus, which can clarify the cross-reaction between the antiserum to the mite and antigens of the mite and house dust. The house dust allergen component is most probably present in second peak obtained by fractionation on Sephadex G 200, which was confirmed by double immunodiffusion with two precipitation lines. Fraction 2 of house dust is observed to bring about more pronounced skin reaction than fraction 1.

Evoked cerebral potential audiometry and hearing threshold.

Audiometric testing by means of the nonspecific evoked cerebral potential (NECP) was performed in 20 children (11 with normal hearing, 6 with conductive and 3 with sensorineural hearing loss). The threshold as determined by this method was 20-30 dB higher than the threshold of the subjective tonal audiograms in the majority of the cases. Only in two cases the difference was up to 70 dB. There were no significant differences between the results of the methods in the three groups of children. The possible reasons for the described discrepancy between the thresholds of the ECP audiometry and the subjective tonal audiometry are discussed. It is pointed out that either of the two methods explores one of two different functional systems: the nonspecific activating system of the brain stem and the auditory pathway. It is suggested that the observed discrepancy between the results of the two methods may mainly be due to different thresholds of the two systems. To avoid false negative results of the ECP audiometry in subjects with 'constitutionally' low amplitude of the NECP it is proposed to routinely determine the amplitude of the NECP produced by somatosensory stimuli. Careful evaluation of the recorded data can improve the diagnostic value of ECP audiometry.

[Epidemiology, diagnosis and treatment of the house-dust-mite allergy in asthmatic children].

Authors introduced as the first in Yugoslavia the method of isolation and classifying Dermatophagoides pteronyssinus out of house dust using the first the help of Spieksma, Cunnington and Araujo, later on by themselves in new established laboratory. In house dust of 290 houses of asthmatic children in 88 (30,8%) through of cases mites were found, mostly Dermatophagoides pteronyssinus. 212 asthmati children who did not react well to specific hyposensitisation with standard house dust extract showed an allergy to house dust mites in 76 (35,8%), who were successful treated with allergen extract Dermatophagoides pteronyssinus with the cutaneous method of hyposensitisation. House dust mites allergy has clinical importance in asthmatic children for pathogenesis of asthma bronchiale in childhood as well in adults. IgE values in the serum after the radioimmunological method in asthmatic and healthy children has been estimated. In asthmatic children we found statistical significant higher of IgE values in the serum as in healthy children, this is probably caused by the infestation from Ascaris lumbricoides. Asthmatic children were significant stengher infesteds the healthy children.

Correlation between the concentration of immunoglobulins in human serum and secretions.

The authors investigated the serum and secretions of 53 patients who underwent surgery for endonasal polyposis. They were distributed in allergic and nonallergic groups, according to their etiopathogenesis. Immunoglobulin concentrations obtained in these groups were compared to our reference standard values (control group). Patients with chronic hyperplastic rhinosinusitis showed a statistically significant increase in concentrations of immunoglobulin A in their saliva and immunoglobulin G and A in their nasal secretion. The authors failed to detect immunoglobulins in the saliva and nasal secretion of patients with endonasal polyposis owing to mucoviscidosis. There was no correlation between the immunoglobulin concentrations in the serum, saliva and nasal secretion of the above patients and controls, but a negative correlation between immunoglobulin M levels in the serum and nasal secretion.
