Long-term results of topical glyceryl trinitrate ointment in children with anal fissure.

BACKGROUND/AIM: Although topical glyceryl trinitrate ointment (GTN) has become a popular treatment for anal fissure in adults, its use in children is still limited. We aimed to determine the effectiveness and safety of topical GTN in the long-term management of anal fissure in children, which has not yet been reported. METHODS: Thirty-one children with anal fissure who received topical 0.2% GTN treatment between 1997 and 1998 were evaluated in 2004. RESULTS: Ten patients had one or more relapses after initial treatment with 0.2% GTN, all within 1 year of first onset of anal fissure. No further recurrences were diagnosed during the 4-year follow-up period. CONCLUSION: Although early recurrence can occur, topical GTN ointment is effective in healing chronic anal fissures in children.

Multilocular cervical thymic cyst: an unusual neck mass in children.

Thymic cysts are usually considered uncommon lesions in the differential diagnosis of neck masses. Approximately 100 cases have been reported to date and most of these cases have occurred asymptomatically. Herein, the authors report a 4-year-old boy with cervical thymic cyst, which was misdiagnosed preoperatively as conglomerated lymph nodes. The correct diagnosis was made after surgical excision and through determination of specific histopathological findings of the thymic cyst. The etiologic theories, differential diagnosis and the treatment of thymic cysts are discussed.

The effects of chemical sympathectomy on testicular injury in varicocele.

OBJECTIVE: To determine whether the sympathetic nervous system plays a role in the contralateral testicular deterioration encountered in varicocele. MATERIALS AND METHODS: Forty male Sprague-Dawley albino rats (28 days old) were divided equally into four treatment groups, i.e. (1) sham operation, (2) with varicocele, (3) treated by chemical sympathectomy plus varicocele, and (4) chemical sympathectomy only. Chemical sympathectomy was induced by administering intraperitoneal 6-OH dopamine (100 microg/g for 5 days) in groups 3 and 4; groups 1 and 2 received equal volumes of physiological saline by the same route. All rats underwent laparotomy and part of the left renal vein (distal to the spermatic vein confluence) was isolated and encircled with a 4/0 silk suture. The suture was left untied in group 2, and tied around a 24 F peripheral venous cannula in groups 3 and 4. The testes were then excised when the rats were 70 days old; malondialdehyde (MDA) in the testicular tissue was assayed by the thiobarbituric acid-reactive substances method, and superoxide dismutase (SOD) and glutathione peroxidase (GSH-Px) levels were determined by spectrophotometric analysis. RESULTS: Varicocele resulted in a significant increase in MDA levels in both testes and chemical sympathectomy prevented this effect. SOD and GSH-Px values were significantly decreased in both testes in group 2; chemical sympathectomy also prevented this effect. CONCLUSION: An induced unilateral varicocele significantly increases the biochemical indicators of tissue hypoxia in both testes. As this increase was prevented by chemical sympathectomy, the sympathetic nervous system may play a role in the testicular degeneration associated with varicocele.

Unusual presentation of tuberculosis reactivation in childhood: an anterior neck mass.

Although extrapulmonary tuberculosis has a broad spectrum of clinical manifestations, involvement of the thyroid gland in children has been reported very rarely. The authors report a case of an 11-year-old girl with a nontender nodular swelling of the thyroid, whose symptoms, tomographic and scintigraphic features, mimicked a nodule with a cystic component. Although seldom observed, tuberculosis should be considered in the differential diagnosis of nodular lesions of the thyroid in children, especially in the patient with known history of exposure to tuberculosis.

An alternative approach to bladder autoaugmentation.

This article describes in detail the alternative technique of bladder autoaugmentation. The authors applied a different autoaugmentation method to reliably create a diffuse circular bulge or diverticulum by incising detrusor muscle while leaving bladder mucosa intact. The results were assessed by urodynamic, radiological, and histopathological methods.

Lipid secreting breast carcinoma in childhood: a case report.

A ten-year-old girl was admitted to our hospital with a mass in the left breast after a local recurrence following a resection done in a district hospital three months earlier. A modified radical mastectomy with axillary lymph node dissection was performed in our department. Histological examination revealed a lipid secreting carcinoma (lipid-rich carcinoma) of the breast. To our knowledge she is the first case in childhood in the literature and is therefore reported.

Congenital megalourethra.

Megalourethra, a rare congenital disorder involving the anterior urethra, is subdivided into two types: Fusiform and scaphoid. Two cases of scaphoid type megalourethra are reported. The first patient, a 5-weeks-old infant diagnosed at birth as having the prune belly syndrome was admitted to the hospital with vomiting and failure to thrive. Intravenous pyelogram revealed marked dilatation of the left ureter and a bladder diverticulum. A penile urethrostomy was performed. The second patient, born to healthy parents after an uneventful pregnancy and delivery, was found to have an enlarged and deformed penis. The baby voided with a poor stream and a concomitant swelling of the penis was noted. Retrograde uretrography showed a sac-like dilatation of the penile urethra. Surgical revision was carried out a 2 stage procedure and was completed (Nesbitt's operation) after 4 months. The patient did well postoperatively and voided with a normal stream without any abnormality of the penile shaft.

Effects of phototherapy on newborn rat testicles.

Phototherapy is the most widespread treatment for lowering bilirubin concentration in neonates. In the routine, phototherapy has some side effects including skin eruption, fluid loss, abdominal distention, mild hemolysis and mild thrombocytopenia. The aim of the study was to investigate the possible mutagenic and gametocidal side effects of 72 h continuous phototherapy on the rat testicle. We observed decreases in spermatogonia numbers per tubule (S/T values), tubular fertilization index (TFI) and sperm sertoli cell index (SSCI), which are the most reliable methods in estimating future fertility potential, due to sensitivity to phototherapy. The differences between study and control groups for S/T, TFI and SSCI values were statistically significant (p = 0.008, p = 0.02 and p = 0.004, respectively). There were significant differences in seminiferous tubule diameters between the control and study groups (p < 0.005), but no significant difference in DNA index values between the control (0.66 +/- 0.12) and study (0.59 +/- 0.05) groups (p > 0.05). As a conclusion, phototherapy seems to have some side effects on the newborn rat testicle. Further studies with larger groups, designed for investigation of the effects of phototherapy on seminiferous tubules, may give more beneficial results.

Effects of testicular fixation on oxidative stress-related parameters in prepubertal rat testis.

The goal of orchiopexy is to provide adequate scrotal fixation to prevent recurrent torsion of the testis and the spermatic cord or ascent of the testis, to be achieved with minimal trauma to the testis. Forty-two prepubertal male Sprague-Dawley rats were equally distributed into 6 groups (1 SHAM + 5 Operation). In operational groups, two lateral sutures were placed near the upper and lower pole of the testis passing through the tunica albuginea with 4-zero polyglactin suture in the transverse axis of the testis. At the 1st-3rd-7th-15th and 30th postoperative days, bilateral orchiectomy was performed, then malondialdehyde (MDA), nitrite, superoxide dismutase (SOD) and glutathion peroxidase (GPx) values were measured in testicular tissue homogenates. MDA and nitrite levels increased, while GPx and SOD levels diminished, statistically significant in both ipsilateral and contralateral testicular tissue homogenates. Prepubertal testicular fixation causes overproduction of the lipid peroxidation end product and free radicals while heavy consuming anti-oxidant reserve systems in both ipsilateral and contralateral testicular tissue. These results clearly reveal the extent of the testicular deterioration due to suture fixation technique.

Testicular blood flow alterations and flow cytometric analysis in prepubertal rats with experimentally induced unilateral varicocele.

Unilateral testicular pathologies have been accused of causing contralateral damage through a decrease in testicular blood flow. However, the contralateral testicular blood flow in unilateral varicocele has not been studied in detail. Therefore, the present study has been designed to evaluate the effects of a unilateral varicocele on the ipsilateral and on the contralateral testicular blood flow and the flow cytometric alterations in prepubertal rats. Experimental stenosis of the left renal vein in prepubertal rats causes dilation of the testicular vessels after puberty. Partial stenosis of the ipsilateral renal vein during the prepubertal period has no effect on ipsilateral or contralateral testicular blood flow, but does induce significant testicular damage, as determined by flow cytometry.
