Cervical status as a predictor of preterm labour.

A study was conducted on 140 pregnant women at and after 28 weeks of pregnancy for finding out the relationship of cervical length and position with onset of preterm labour. Incidence of preterm labour was significantly higher (p < 0.001) in women with central position of cervix (54.67%) than that with posterior position of cervix (10.77%). Onset of preterm delivery was closely related to cervical length of 2.5 cm or less irrespective of central (82.75%) or posterior (20%) position of cervix (p < 0.001).

Amphiphilic and hydrophilic domains of human sperm membrane antigens recognized by immunoinfertile sera.

Selected sera from married couples with immunological infertility were used to identify antigens on hydrophilic and amphiphilic domains of human sperm membrane. Out of eight sera, six recognized proteins from the hydrophilic as well as amphiphilic regions of the sperm membrane. Sera were either reactive to acrosome or to equator and tail of human sperms in indirect immunofluorescence assay.

Caesarean sections in developing and developed countries.

A clinical study of 1086 cases of caesareans done in the period 1987-1991 in a rural institute is presented with a review of the latest data from India and elsewhere. Of the women, 86.19% were between 20 and 30 years; 43.83% were primigravidas. The most common indications were obstructed labour and malpresentations. 17.76% of the emergency and 14.64% of registered obstetric admissions resulted in caesarean delivery. The status of caesarean births is very different in developed and developing countries though both face the problem of its very high incidence.

Midtrimester termination of pregnancy--search for a better method continues.

Every day a new method of termination of second trimester pregnancy clearly indicates that we have still not found a simple, safe, effective and economic method of termination of pregnancy in second trimester. Present study of 855 cases aims at searching out something better from available modalities. Age old hypertonic saline and ethacridine lactate were used with adjuvants like hyaluronidase and a preparation containing isapgol husk to reduce injection abortion interval and failures. Life threatening dangers of hypertonic saline are known. Ethacridine lactate seems to be safe. By giving it intra-amniotically with these adjuvants its major disadvantages could be minimised. There was no mortality. However, there was morbidity in the series.

Immune factor and infertility.

Infertility is a common problem seen in gynaecological practice. In a significant number of cases of infertility an immune factor may be operative. In our study of 200 cases, the presence of sperm antibodies was studied in sera of both males and females. The tests used were the gelatin and tube slide agglutination tests. In the infertile group with unexplained infertility (50 couples), 16% of males and 30% of females were positive by the gelatin agglutination test, and 18% of males and 30% of females were positive by the tube slide agglutination test. There was complete absence of positive test (by both methods) in male controls, but females had positive tests for antibodies by both methods in significant numbers.

Changes in respiratory function tests during pregnancy.

Anatomical, physiological and biochemical adaptations that occur during pregnancy are profound. Changes in respiratory physiology are a part of the same process. In the present study of 70 selected women, 50 pregnant and nonpregnant control, it was found that out of seven parameters studied five showed changes. There were changes in respiratory frequency, tidal volume, vital capacity, inspiratory capacity and expiratory reserve volume. Maximum voluntary ventilation and timed vital capacity did not change. RF, VT, VC and IC rose significantly while ERV had a significant fall. These changes may be affecting ante-intranatal behaviour of pregnant women and their pregnancy outcome.

Medical termination of pregnancy and concurrent contraceptive adoption in rural India.

This study reports on 2,755 women seeking medical termination of pregnancy (MTP), and concurrent contraceptive acceptance, at a clinic in rural India from 1976 to 1987. The level of contraceptive acceptance among married women seeking MTP between 1976 and 1987 was 88.2 percent. Among married women seeking MTP in their first trimester of pregnancy, 43.4 percent accepted the IUD as a method of contraception and 41.8 percent accepted sterilization. By contrast, only 11.5 percent of women in their second trimester accepted the IUD, but 70.2 percent accepted sterilization. Seventy-two percent of the unmarried women and 43 percent of the married women seeking MTP were in their second trimester. Recommendations are made to: (1) combine contraceptive services and counseling with MTP whenever possible, (2) examine the consequences of policies that exclude unmarried women from contraceptive services, (3) investigate the reasons why so many women in this study sought MTP so late in pregnancy, and (4) obtain information on the determinants of contraceptive acceptance among women who seek MTP.
