[Treatment of duodenal stress ulcer by means of chemical sclerosis of the gastric mucosa induced with a 60 percent glucose solution (results after 1--5 years)].

Sclerosing of the gastric mucosa with a 60% fresh sterile solution of glucose was applied in 60 selected patients with chronic conflictual duodenal ulcers, hyperacidic gastritis by vagal neurogenic hyper-reactivity, associated with ischaemic gastropathy developed on an sympathico-adrenergic background. The immediate operatory results were good. The late results (between 1 and 5 years) were as follows: the clinical, radiological and metabolic results were good in 57 patients. Gastric chemical analysis, the values of the hourly basal output remained high in 16,6% of the patients, and those of acidity stimulated by histamine administration remained high in 13,3% of the patients. No peptic recidive was recorded. Unsatisfactory late results were noted in 3 patients (of which forced indication of the method in one patient and non-association of drainage in two patients).

[Rapid method of testing microbiol sensitivity to antibiotics].

The conventional technique for determining the sensitivity of bacteria to antibiotics is slow and a more rapid method becomes mandatory. The authors tested microbial sensitivity to antibiotics by measuring turbidity of the cultures in fluid medium. The method offers an orientation in the choice of an elective antibiotic within at most 4 hours.

[Polyps; rectocolic polyposis. Etiopathogenic and anatomoclinical data. Results].

The authors statistic concerns 370 interventions for adenomatous polyps and rectocolic polyposis (105 segmental exereses, 32 total or sub-total rectocolectomies, 233 direct, endo-anal or laparocolotomy electroresections). The necessity is stressed of the early detection of isolated, or of multiple polyps, or of potentially malignant polyposis, either genetically inherited or acquired, and on their radical extirpation from the beginning, an attitude that improved late oncologic prognosis (98% survivals after 5 years).

[Severe arterial hypertension due to chronic constitutional hyperadrenalism. Splanchnic sympathectomy and left adrenal medulla sclerosis with 40 percent sodium salicylate (result after 3 years--genetic chemosurgery)].

The case is presented of a male patient hospitalized for severe arterial hypertension determined by chronic constitutional hyperadrenalism. Splanchnic-sympathectomy was performed, associated to medullar sclerosis of the left adrenal with 40% sodium salicylate (genetic chemical surgery). The results after 3 years were good, both clinically and paraclinically. The constitutional background was normo-hypoadrenergic and the ecologic balance of the patient became normal after surgery.

Morphophysiological studies in experimental myocardial aggressions with isoproterenol. Note I. Morphological aspects.

The morphopathogenesis of lesions induced by high doses of isoproterenol injected for 1-7 days and 1-3 months is dynamically studied in groups of Wistar rats and guinea pigs. The disseminated myocardial necroses induced were of a hypoxic nature. By cicatricial substitutions, lesions of cardiosclerosis similar to those observed in pheochromocytoma cases were obtained.

Morphophysiological studies in experimental myocardial stress induced by isoproterenol. Note II. The myocardioprotector effect of magnesium ascorbate.

The efficiency of magnesium ascorbate, a new Romanian cardioprotector drug, was studied in guinea pigs and Wistar rats, both normal and with scurvy, within the framework of morphophysiological investigations of the pathogenesis of myocardial disease induced by large doses of beta-adrenergic substances of the isoproterenol type. The preventively treated animals presented only disseminated myocardial ultrastructural lesions within reversible limits. Biochemically, as compared with controls, a return to normal levels of ATP and Mg2+ and a decrease in the uptake of 45CaCl2 by the myocardium were observed. The pharmacological mechanisms of action of magnesium ascorbate are discussed.

Myocardial lesions in experimental acute heart failure.

The present study concerns the morphological and biochemical lesional picture of the myocardium in cases of acute heart failure induced by various experimental models: ligature of the coronary artery, direct electric stimulation of the heart by catheterism, lethal hemorrhage, pneumothorax, beta-adrenergic shock. Worthy of note was the similitude of the lesional myocardium pictures characterized electron microscopically by a wide range of lesions, from reversible to focal cytolysis, and biochemically by decrease of mitochondrial enzymes, ATP, Mg2-1 X K+ and increase of Na+ X H2O, Ca2+. Problems linked to the pathogenesis, reversibility of the lesions and efficiency of certain therapeutical means are discussed.

Cytochrome-oxidase and succinate dehydrogenase activity in the rat kidney preserved by cold storage, correlated with the study of mitochondrial suspension.

Succinate dehydrogenase (SDH) and cytochrome-oxidase (CO) activities were determined biochemically in the white rat kidneys after 24, 48, 72 hours preservation by cold storage. The investigations also included the study of infrastructural alterations in mitochondrial suspensions, separately for the medulla and the cortex. The comparison of the enzymatic activity curves showed a mirror-like aspect with a maximum at 24 hours for CO and a minimum for SDH, and a constantly more intense activity in the medulla.

Experimental investigations in prolonged myocardial ischaemia. Note I. Biology of the myocardial fiber after transient myocardial fiber after transient myocardial ischaemia.

The first ten days' evolution of post-ischaemic lesions of the premonitory or angina pectoris syndrome type was experimentally studied by the challenge of a short-term (10 and 15 min) ischaemia, of an adaptation to ischaemia and an adaptation followed by prolonged ischaemia (20 and 35 min). Worthy of note was the persistence of reversible lesions after short-term ischaemia and adaptation, and the progressive evolution towards cytolysis and cicatrization of some pancicellular foci after adaptation followed by prolonged ischaemia. The role of mitochondrial lesions, of lysosomal hydrolases, the inefficiency of renewed circulation, as well as problems of diagnosis are discussed.

Experimental investigations in prolonged myocardial ischaemia. Note II. Biology of the myocardial fiber during permanent ischaemia.

The dynamics of morphologic and biochemical lesions of the myocardial fiber during the first ten days after an experimental myocardial infarction showed an unequal evolution of the lesional picture in the ischaemic area, the intervention of lysosomal hydrolases and the reactions of inflammatory fibrocollagen substitution. Morphologic aspects in the areas neighbouring the infarction and other myocardial areas are discussed.

[Chemical medullosclerosis with 40-percent sodium salicylate, combined with splanchnicosympathectomy, with or without contralateral adrenalectomy, in severe essential arterial hypertension. II. Clinical, paraclinical and technical studies. Results for a period of 2 to 20 years (original concepts and therapy)].

Unilateral medullosclerosis associated with splanchnicosympathectomy was applied in 60 cases of severe essential arterial hypertension. In 21 patients this was associated with contralateral adrenalectomy. No deaths were recorded. The late results (2 to 20 years) varied in terms of the anatomoclinical stage of the disease when surgery was performed. Systolic pressures fell to 30-50 mm Hg, hypertensive oscillations disappeared, mean adrenalinaemia values decreased by 47.9% after unilateral medullosclerosis and slanchnicosympathectomy and by 72.2% after bilateral interventions in two stage. The constitutional hyperadrenergic psychosomatic background was modified and became normohypoadrenergic. There was no case of adrenocortical insufficiency, the secretion of corticoids being insured by activity of the hexose-6-mono-phosphate dehydrogenase shunt.
