[The role of the traditional radiological methods in conservative therapy and endodontics].

The traditional radiographic examinations used in restorative dentistry and endodontics are: intraoral radiograph performed in the beta-wing or the paralleling technique, the partial extraoral radiograph (rotational narrow beam), the panoramic radiography and the periapical radiographs with the bisecting technique which is particularly suitable for visualization of the apex. Radiology is a valuable diagnostic means to evaluate the extension of primary caries, to identify secondary and interproximal initial decays and if required let the therapist measure the biological width. Several factors influence the radiographic interpretation of caries: cervical burnout, mach band effect, internal and external resorption, restorative materials for fillings and sub-bases, abrasions and/or erosions. Radiology allows the diagnosis of developmental and acquired abnormalities of the teeth which can have an influence on the treatment itself. Examples are: variations in the shape of the crown and root, dens in dente, enamel hypoplasia, dentinogenesis imperfecta. Radiograms are of major importance in the evaluation of restorative dentistry results: precision of the margins, congruous contact points, fractures. Moreover, it provides the endodontic procedures with useful diagnostic data and permits the measurements during the treatment, supplying the immediate and long term checks too.

[Radiologic features of osteosarcoma of the jaw].

Osteosarcoma of the jaw differs from that in other sites because it is less aggressive, rarely metastasizes and appears later in life. The literature lacking a systematic treatise on the radiologic features of this disease, the authors decided to collect the case histories of 9 patients, 5 men and 4 women, 24 to 48 years old, suffering from osteosarcoma of the jaw. Both conventional radiography and CT were used to examine all patients. The accurate study of radiologic findings made it possible to establish 3 basic lesion patterns. The first and very rare one, is characterized by the total absence of neoplastic tissue ossification processes; in this case, the radiologic diagnosis is extremely difficult, even with CT support. The second and more common pattern exhibits evident tumor neo-ossification, the most representative cases present several bone lamellae radiating from the lesion, with a sunburst-like appearance. The disease can be diagnosed with conventional radiography alone, especially in the most advanced cases; CT permits the identification of early forms, missed with conventional methods. The third pattern is characterized by amorphous lesion ossification: there are small areas of structureless ossification and a patchy appearance, separated by non-ossified neoplastic tissue. CT shows tumor ossification missed at conventional radiography. Finally, the authors observed an extremely rare case of parosteal sarcoma of the jaw, considered a variety of osteosarcoma by some pathologists. The lesion presented uniform neo-ossification which was highly sclerotic and separated from cortical bone by a thin layer of radiolucent tissue: both findings were clearly visible with CT. In conclusion, the systemic use of CT has led to a marked improvement in the radiologic diagnosis of osteosarcoma of the jaw.

Radiological visibility of small artificial periapical bone lesions.

Previous studies have indicated that the visibility of periapical lesions on a radiograph is determined by erosion of either the cortex or the junction area between cortex and spongiosa. In the present study, artificial lesions, varying in diameter from 1.6 to 4.5 mm, were produced with a round dental bur at 32 sites in 27 blocks from four dried human mandibles embedded in paraffin and radiographed under standardized conditions. The radiographs were evaluated by three observers with differing degrees of radiological experience, and the results expressed as Mean Visibility Index (MVI) and Mean Difference Index (MDI). Six parameters were measured at each site, thickness of the bone, of the cortex and of the spongiosa, density of the spongiosa, cortex/spongiosa ratio, and erosion of the cortex, and their correlation with the two indices determined by stepwise and multiple regression. It was found that MVI was significantly associated not only with the erosion of the cortex but also the density of the spongiosa and the diameter of the lesion. MDI was found to be associated mainly with cortex erosion.

Primary impaction of primary teeth: a review and report of three cases.

Primary impaction of primary teeth is rare. Only ten cases have been found in the world literature. It affects almost always second molars and the finding is usually occasional. Three cases of this abnormality, two of the upper jaw and one of the mandible, are presented. The analysis of the characteristics of these cases and the cases in the literature, suggests that ankylosis has probably a leading role in the etiopathogenesis. The following criteria for a correct diagnosis is presented: 1) deep retention into the bone; 2) absence of caries, restorations and resorptions; 3) frequent passing of the corresponding permanent premolar; and possible retention of the corresponding adjacent tooth. More attention is recommended in the study of the lesion for the future.

[The surgical verification of the periodontal probe and endoral radiography in assessing the bone substance loss in periodontal disease. A proposal for optimizing the methods].

It is difficult to compare levels of bone loss in periodontal disease measured by periodontal probe and X-ray and to verify them versus surgical techniques. In fact, there are relatively few published reports on the subject. To this purpose the Authors describe their personal experience using plates equipped with guide tracks and center-film boxes with personalized bites. The suitability of the two methods (probe, intraoral X-ray) to determine levels of bone loss similar to those revealed by surgical verification was assessed on the basis of an analysis of the respective bone loss in 44 sites surrounding 19 dental elements. This method, which is accurate but time-consuming, demonstrated an broadly analogous underestimate (approx. 20%) using both preoperative probe and X-ray techniques in comparison to the true level of bone loss measured during surgery.

Desmoplastic fibroma of the mandible: a case with an unusual clinical presentation.

A new case of desmoplastic fibroma of the mandible is reported. The patient was a 29-year-old woman who presented with a 5-year course of abscess-like lesions simulating a chronic apical infection. Radiographically, a nonspecific, small osteolytic area was observed. A wide resection of the tumor and the neighboring bone was performed.

The computed tomographic appearances of cherubism.

The computed tomographic (CT) aspects of six cases of cherubism are described. Through its optimal representation of the lesions, CT enabled certain characteristics of this disease to be substantiated, namely, limitation of the process to the bones of the jaw and primarily superficial development of the mandibular lesions, coupled with an unusual mandibular condylar impairment. Moreover, the composition of this series permitted investigation of the condition in its several stages of progression.

[Radiologic aspects of myxoma of the jaws].

Four cases of jaw bones myxoma are described, laying special stress on the radiological appearance. Conventional radiogram showed a typical multilocular osteolytic lesion thinning and expanding the cortical bone. This allowed the authors to correctly diagnose the affection. On the contrary CT, showing a unilocular lesion, offered a far less typical representation of the tumor in the two patients who underwent this examination.

Comparison between pituitary computed tomographic findings and tests of hypothalamo-pituitary function in 72 patients with hyperprolactinaemia.

To verify the diagnostic capacity of some dynamic tests of the prolactin (Prl) secretion, the findings obtained by high-resolution computed tomography (CT) were compared with results obtained from tests using nomifensine (NOM) domperidone (DOM) and thyrotrophin-releasing hormone (TRH) in 72 patients with pathological hyperprolactinaemia. None of the patients with tumours had a positive Prl response to NOM or to DOM administration; however, a positive response to these tests was present in only 24 and 41%, respectively, of the patients with normal CT picture. The results of TRH testing were similar to those obtained with DOM. Different neuroendocrine patterns were disclosed by comparing pituitary Prl and thyrotrophin (TSH) responses to DOM: 1) some subjects had a reduced Prl response together with an exaggerated or normal TSH response to DOM; they comprised patients with tumour, empty sella, and normal CT picture; 2) other patients with normal CT picture had a positive Prl and a normal TSH response to DOM. These results demonstrate that a negative Prl response to NOM and DOM characterizes all patients with adenoma; however, the tumour-like responses in patients with no visible tumours seem to reduce the diagnostic value of these tests, unless the latter may predate the radiological appearance of an adenoma. On the other hand, a positive Prl and a normal TSH response to DOM exclude the presence of a pituitary tumour. This diagnostic finding is strengthened by the positive response also to NOM. Whatever may be the diagnostic validity of dynamic tests, they provide sound information on the functional state of dopamine neurotransmission.

[Computerized tomography of bladder neoplasms].

A comparison was made between the pelvic CT picture and clinical and instrumental findings in 89 patients with bladder neoplasm. A close critical assessment was also made of CT as a reliable way of determining the TNM stage, and its effective aid in the choice of a therapeutic strategy. The advantages and limits of CT are discussed. The conclusion is drawn that it is of assistance in overall TNM rating, and may sometimes provide useful informations concerning the involvement of surrounding structures. Particularly as far as the choice of radical management is concerned, however, it is prone to by no means negligible staging errors, though these are open to correction in the light of non-radiological criteria.

[Sequential endovenous angiourography (AUS) with image subtraction for the study of the arterial phase of urography. Comparison with nephroangiotomography (NAT)].

Two urographical techniques employed in the endovenous study of renal arteries are compared. They are nephroangiotomography (NAT) using synchroplan and sequential angiourography (SAV) with image subtraction. 64 patients were studied, 32 with NAT and 32 with AVS. Both techniques were shown to be useful, but AVS through infrequently used was cleary more efficient than Nat in the study of the vascular arterial phase.

Nephroangiotomographic diagnosis of retroperitoneal spleen. Report of a case.

A case of retroperitoneal spleen diagnosed by nephroangiotomography is described. The embryologic background of this rare abnormality is explained. Stress is laid on the importance of nephroangiotomography in the correct evaluation of those alterations that normally pose difficult problems of interpretation.

[Renal tubular damage after rapid intravenous contrast study with iopamidol].

Reference is made to results obtained in 53 nephropathics in the study of urinary elimination of alphaglycosidase and lysozyme before and after rapid contrastography with sodium and meglumine diatrizoate and with iopamidol. A statistically significant difference in alphaglycosidase was evident with both media, whereas no change was noted for lysozyme. As far as the evaluation of alphaglycosidase was concerned, the tubular alteration induced by each medium was the same in terms of statistical comparison of the mean differences before and after urography.
