Controlling intraoperative and postoperative nasal bleeding.

An indwelling nasal suction tampon is described that effectively controls both intraoperative and postoperative nasal hemorrhage. The significant benefits of this device are better intraoperative control for the surgeon and less complicated postoperative recovery. I do not have any financial involvement in this device, but I would like other nasal surgeons to benefit from its particular advantages. Increasing patient complicance in preparing for barium enema examination.

Since feces may mimic important lesions of the colon, adequate cleansing is required for a good diagnostic study. An experiment was designed to assess the effect of advance oral and written communication on patient compliance with instruction for preparing the colon for the barium enema. These communication methods were compared against a control group in the extent to which they would encourage patients to more adequately follow cleansing instructions. It was found that each of these increased the quality of the preparation to a similar degree and when combined resulted in the most optimal level of cleansing.

Limitations of mammography.

The aim of mammography is to aid in the discovery of cancers of the breast at an early stage and to supply information about breast changes. It is not 100% reliable, and in most circumstances the decision to biopsy should not be based solely on the radiographic report. A relevant example is presented to reaffirm the need for a definitive surgical diagnosis when clinically indicated.

Arthrography in penetrating injuries.

Arthrography in cases of trauma to joints may provide valuable information about joint disruption as well as the location of foreign bodies. Allergy to contrast agents and bleeding diathesis are the only contraindications. Four cases are presented that demonstrate the usefulness of this technique.

The detection of glass foreign bodies.

Roentgenography is a reliable method for detecting glass foreign bodies resulting from trauma. The density of glass in comparison to surrounding tissue almost always provides sufficient contrast for roentgenographic differentiation. This method is slightly compromised by the relative size of the foreign body and the body part being examined, but setting the roentgen ray beam for bone technic and having the patient in different positions facilitate detection of such a foreign body.
