[Hereditary (dysplastic) diseases of the joints].

Using the intentional procedures to reveal systemic semantic interrelations between different phenomena that form the causative-sequential chain in joint diseases of dysplasia genesis and assuming the pathology theory laws as a basis, applying the methodologies of different interdisciplinary sciences and the technology of cognitive simulation, the conceptual model "joint dysplasia--dysplasia arthrosis" was developed. This model depicts the characteristic features of joint dysplasia. The use of the model permitted one to base and describe a group of inherited (depressional) diseases of joints as an independent scientific orthopedo-arthrological problem. The application of the conceptual model confirmed its high cognitive and reforming role. Using this model on a knee joint new syndromes of dysplasia have been described; the system of roentgenometric diagnosis of dysplasia anomalies and new methods of surgical intervention have been elaborated; the foundation has been laid of prophylactic surgery aimed at the exclusion or at the minimizing of the risk of gonarthrosis in persons suffering from different inherited syndromes.

[Corrective osteotomy of the femur in the treatment of the syndromes of dysplastic instability of the hip joint].

On the basis of analysis of short-term outcomes (1-5 years) of treatment of 139 patients (150 joints) with congenital dislocation and oluzation of the thigh, conclusion has been drawn that the application of corrective femur osteotomy with these patients should be limited with the cases of formation of pathology loading of the joint under conditions of negative intercomponental trophism. Restoration of correct anatomical interrelations between the components of the joint can be accompanied by the clinical decompensation of the hip joint state. Position of the cotyloid cavity and epiphysis of the head of the femur is of fundamental importance for further formation of the hip joint and its neck and diaphysis position is of minor importance. When positing indications for corrective femur osteotomy and its efficiency prognostication it is necessary to be orientated towards epidiaphysar and not cervicodiaphysar correlations. Removal of antetorsion of the head of the femur without due regard for the grade of its proximal epiphysis retroversion can lead to the development of external rotary contraction in hip joint.

[Surgical treatment of scoliosis by the Armstrong method].

The authors share their know-how of surgical treatment of severe forms of scoliosis by the method of Cartel Armstrong. Evaluation of the results within 2 years (the period of the greatest loss of correction) demonstrated greater efficiency of this method as compared with operation after Harrington with application of one fixing device or distractor with lateral traction.

[Late treatment results in patients with bilateral congenital hip dislocations].

In the present work, from the position of final outcomes, have been analyzed the results of treatment of bilateral dysplastic femur dislocation with application of different methods of femur setting without correction of space position of proximal section of femur and cotyloid cavity. The best long-term results are observed with the patients, subjected to single-step closed or sparing open hip setting. Age-comparable groups of patients have similar characteristics of the final outcome structure. Open femur setting, following the pattern of arthroplasty, as traumatic intervention shouldn't be applied for bilateral hip dislocation treatment.

[Lengthening and correction of deformation of the femur in children and adolescents using rod and bar-rod apparatus of external fixation].

55 patients with thigh shortening and deformations have been subjected to 65 operations with the use of the rod-based apparatus with outer fixation. Original arrangement diagrams and indications for application of apparatus with external fixation of different versions are provided. Planned lengthening has been achieved with 54 patients. Decrease of the apparatus mass and dimensions, reduction of operative intervention time period, improvement of conditions of remedial gymnastics carrying out and patient self-servicing are considered as advantages of the rod-based apparatus with external fixation.

[Hip reduction in its congenital dislocation and the evaluation of long-term results].

Having analysed the outcomes of the treatment of 57 patients with congenital dislocation of the hip for over 20 years, the authors give an evaluation of closed reduction after Lorenz, according to the method of M. S. Sitenko and open reduction like arthroplasty. Doing this they evaluated the results from the points of view of stabilization of the joint function, development of dysplastic arthrosis and equal lengths of the legs. The outcomes of treatment after Lorenz were the best ones.

[Genetically determined diseases of the hip joint (a conceptual model)].

The authors consider the problem of pathology of the hip joint from the point of view of the conception "Dysplasia-coxarthrosis". A single system of the ideas concerning dysplastis pathology is presented, which will allow to unify the methods of diagnosis and treatment of dysplasia of the hip joint in children, adolescents and adults with the aim of prevention of crippling diseases of the joint.
