In vivo studies of 111In-labelled canine lymphocytes.

[111In]oxine was used to label canine periperal blood lymphocytes from two normal animals and one animal with breast carcinoma. Using 150 microCi of [111In]oxine to label 50 X 10(6) cells yielded a labeling efficiency between 40 and 75% and a viability of about 95%. Gamma camera images obtained 24-44 h after injection showed uptake in the subcutaneous lymph nodes of the head, neck, axilla, pelvis and popliteal regions. Uptake in the liver, spleen, bowel, and bone marrow was also seen. In the animal bearing breast carcinoma, uptake occurred in the tumor, lungs and in a draining sinus in the left axilla. These results suggest that 111In labeled lymphocytes are useful in visualizing regional lymph nodes and may be valuable in assessing tumor recognition by these cells in vivo.

Semiquantitative gallium scintigraphy in patients with osteogenic sarcoma.

Sequential gallium scans were performed in 37 patients with newly diagnosed osteogenic sarcoma. High gallium uptake was found more often in males in the 10 to 19 age group and in femoral lesions. High uptake was also seen in patients who had predominantly osteoblastic or mixed changes on radiographs and in those who had a soft tissue mass. Following chemotherapy, significant decrease of tumor to nontumor ratio occurred in the patients who responded to treatment as shown by a Grade III or IV response on histologic examinations at the time of en bloc resection. It is concluded that semiquantitative gallium scintigraphy is useful in monitoring therapeutic response in patients with osteogenic sarcoma.

Bone sarcomas in Paget disease: a study of 85 patients.

This is a comprehensive review of 85 patients who had bone sarcoma associated with Paget disease and who were seen at Memorial Sloan-Kettering Cancer Center between 1927 and 1982. There was an almost equal distribution of tumors in the axial and the appendicular skeletons. The pelvis, humerus, femur, and skull were the tumor sites in 80% of cases. The tumors were bulky large soft tissue masses. Lytic lesions were more common than sclerotic lesions. Mixed lytic and sclerotic lesions were much less common than either single type. Periosteal reaction was uncommon and found in less than 7%. Methylene diphosphonate scans of the bone often showed a cold area that was associated with marked increase in uptake on the gallium scan. Angiography, which was performed in 13 patients, was useful, but CT was much more helpful in showing the soft tissue mass as well as the extent of bony disease. Only three patients in this study survived for five years. Present chemotherapy protocols were disappointing in the treatment of this highly lethal tumor.

Gallium scans in Paget's sarcoma.

Bone and gallium scans were performed in twelve patients with Paget's sarcoma. Most of these patients had polyostotic Paget's disease. The lesions with sarcomatous changes were usually associated with less Tc-99m MDP uptake as compared with Pagetoid bone which was not sarcomatous. Gallium uptake in the tumor area was proportionally higher than the MDP uptake and appeared quite irregular. Follow-up scans were available in a limited number of patients. Temporary response to chemotherapy was reflected by the decreased gallium uptake in the tumor area. Gallium scans appear to be useful in confirming the diagnosis of Paget's sarcoma and may be valuable in monitoring therapeutic response.

Increased accumulation of 2-deoxy-2-[18F]Fluoro-D-glucose in liver metastases from colon carcinoma.

Three patients with liver metastases from colon carcinoma were studied with 2-deoxy-2-[F-18]fluoro-d-glucose (F-18-FDG) using positron emission tomography. The radioactivity in the metastatic tumor increased continuously following the injection of F-18-FDG, whereas it decreased in normal liver tissue. This resulted in the tumor to normal-liver ratio of 3.3-4.7 at 50 min after injection. The liver tumor was visualized as an increased accumulation of radioactivity in all patients, with the central area of the tumor showing less activity. These preliminary results suggest that F-18-FDG may be useful as a positive imaging agent for the detection and characterization of liver tumors.

Bone scans in bone metastases from functioning thyroid carcinoma.

Sixty percent of differentiated thyroid carcinoma bone metastases identified by local radioactive iodide uptake and radiographic changes were negative in the bone scans. Another 20% of the bony metastases showed only a minimal increased uptake of bone imaging radionuclides. It is concluded that the bone scan is not a useful tool in the work-up for metastatic thyroid cancer.

Gallium scan in recurrent Hodgkin's disease in children.

In 18 of 88 children with biopsy proven and previously untreated Hodgkin's disease, recurrence developed during a period from four to 53 months after therapy (median period, 22 months). In 16 patients in whom gallium scans were performed, 21 positive gallium scans were obtained during 26 episodes of recurrence. Abnormalities were noted in half of them during a period from one to 10 months prior to physical, laboratory, radiographic or histologic confirmation of recurrence (median period about 5 months). We have concluded that the gallium scan is very useful in initial workup and is sensitive in detecting early recurrence in children with Hodgkin's disease. Such scans are indicated when there is clinical suspicion of recurrence, when other modalities are unavailable or when the results of other studies are equivocal.

Nuclear medicine and cancer research in the People's Republic of China.

This article reports the experience of the author's recent visit to China where he witnessed nuclear medicine facilities. Among other things, the author observes that most imaging equipment and radionuclides are made in China and that many good textbooks of nuclear medicine are available in Chinese. Precautions on radiation safety are very much stressed. Chinese medicinal plants are always added in order to improve the therapeutic effects of classical chemotherapeutic agents or to lessen their possible side effects. Their scientific value cannot be evaluated, nor can it be for the "anti-cancer pills" that are sold freely. The author encourages studies of different patterns of cancer problems in different geographic localities, as he gave indications for a few geographic areas.

Labeling efficiency and stomach concentration in methylene diphosphonate bone imaging.

When Tc-99m-labeled methylene diphosphonate was used 4.75 hr after reconstitution, stomach concentration was seen in the majority of patients. The number of patients with stomach concentration increases with time and is associated with the increasing amount of product breakdown. The reconstituted labeled MDP should not be used after 4-5 hr if stomach concentration is to be avoided.

Gallium-67-citrate accumulation in the heart with tumor involvement.

Tumor involvement of the heart is often found in post-mortem examination of patients who died of widespread malignancy. Recently, we saw four patients who had intense gallium uptake in the region of the heart. All of them had some cardiac symptoms and abnormal physical findings. Tumor was found in the pericardium and myocardium in all three patients who received necropsy.

Abnormal gallium scintigraphy in Pneumocystis carinii pneumonia with a normal chest radiograph.

The authors describe a patient with malignant lymphoma in whom the use of a 67Ga citrate lung scan assisted in the diagnosis of P. carinii pneumonia. In immunodeficient patients presenting with unexplained fever, abnormal pulmonary function tests, a normal chest radiograph, and few chest symptoms, the 67Ga citrate scan may lead to the early detection and successful treatment of this potentially lethal infection.

Gallium and bleomycin scans in the clinical staging of testis tumor.

Twenty-one patients with testicular tumors had gallium scans prior to retroperitoneal lymph node dissection. Eleven of 14 patients found to have nodal involvement had positive scans, and 2 of 7 patients with negative nodes had false positive scans. Bleomycin scans were positive in 4 of 5 patients with nodal metastases. While these scans provide a simple, non-invasive and occasionally useful technique for the clinical staging of testis neoplasms, they do not, in our experience, significantly supplement other staging procedures.

Indications for liver and brain scans. Screening tests for patients with oat cell carcinoma of the lung.

From 1973 to 1975, scans of the liver and brain as routine screening procedures were performed on patients with oat cell carcinoma of the lung at the Memorial Sloan-Kettering Cancer Center. Normal test results were observed in 20 of 21 and 32 patients who showed no clinical evidence of involvement of the liver and brain. Only one scan in 21 and one in 35 yielded new information concerning possible organ involvement. We concluded that scans of the liver and brain are not useful methods of screening the patient with no clinical evidence of oat cell carcinoma.
