Pancytopenia with hypocellular bone marrow due to miliary tuberculosis: an unusual presentation.

In a patient who presented with fever, weight loss and abdominal swelling, blood counts revealed pancytopenia. Broad-spectrum empirical antibiotic treatment was started because of neutropenic fever, but without effect. The bone marrow was hypocellular with an increase in fat droplets. Peritoneoscopy was performed to evaluate ascites; in the liver biopsy specimen caseous granulomas were seen and the patient was found to suffer from miliary tuberculosis. Antituberculous therapy was begun and the patient recovered completely and the haematological parameters returned to normal.

Effect of desmopressin (DDAVP) on protein C and protein C inhibitors in uremia.

The effect of desmopressin (DDAVP) on protein C (PC) and PC inhibitors was investigated in 7 uremic predialysis patients, 7 hemodialysis patients and 7 controls. Significant decrease in PC activity was observed in all groups after DDAVP administration. Desmopressin did not influence PC antigen and two well-known PC inhibitors, plasminogen activator inhibitor-3 and alpha 1-antitrypsin. Thus, it is suggested that DDAVP-induced decrease in PC activity is not related to the changes in PC inhibitors; further studies are needed to clarify the precise mechanisms of the effect of DDAVP on PC in uremia.

Activated protein C levels in patients with gynecologic tumors.

Activated protein C (APC) levels were measured in the plasma of patients with benign and malignant gynecologic tumors preoperatively, and on postoperative days 1 and 7. Preoperative levels of APC were found to be significantly low in both groups of patients compared to a group of healthy subjects. On postoperative day 1, levels of APC decreased significantly in both groups of patients compared to preoperative levels. By the 7th postoperative day, levels of APC in patients with benign tumors had increased significantly compared to postoperative day 1; levels in patients with malignant tumors, however, remained low. Our findings imply that there is a substantive correlation between gynecologic tumors, gynecologic surgery and the coagulation inhibitor, APC, which, in turn, can presumably be used to identify gynecologic patients who are at high risk for venous thrombosis.

Evidence for the role of dialysis hypoxemia in the pathogenesis of hemodialysis-induced rise in tissue-type plasminogen activator.

It is well known that hemodialysis (HD) causes a rise in plasma tissue-type plasminogen activator (t-PA). Although there have been several suggested mechanisms responsible for this effect of HD, the precise cause has not been well understood yet. Another complication of HD, when performed with acetate-containing dialysate, is hypoxemia, which is commonly observed during the first hour of the session. The purpose of this study was to investigate the relationship between dialysis hypoxemia and HD-induced t-PA changes during the first two hours of HD. HD caused significant increase in plasma t-PA antigen levels. When individual t-PA profiles versus time were examined, two patterns were observed. Whilst ten subjects (%56) experienced minimal or no increase, t-PA antigen level of the remaining eight subjects began to rise at 30 minutes and continued at that level up to 90 minutes, when the last samples were drawn. The courses of pO2 were also different; whilst the former group had "early-onset and short-term" hypoxemia, the latter had "late-onset and prolonged" hypoxemia. The amount of increase in t-PA antigen and the amount of decrease in pO2 were correlated at 60 and 90 minutes of the HD session. Thus, it is concluded that dialysis hypoxemia may contribute to HD-induced rise in plasma t-PA levels. Further studies comparing different dialysates and dialyser membranes are required to confirm this hypothesis.

The acute effect of smoking on platelet and endothelial release reaction is suppressed in chronic smokers.

Whilst increased plasma beta-thromboglobulin (beta TG), platelet factor 4 and thrombospondin levels are regarded as reflecting the release reaction of platelets, tissue-type plasminogen activator (t-PA) and plasminogen activator inhibitor-1 (PAI-1) levels represent endothelial release reaction and/or damage. In this study including 12 smokers and 9 nonsmokers, we investigated the acute and chronic effect of smoking on these parameters and antithrombin III, protein S and fibrinogen, as well. Nonsmokers were found to have somewhat higher plasma levels of beta TG, t-PA and PAI-1 than chronic smokers, and 30 minutes after smoking two cigarettes, these levels and protein S levels of nonsmokers showed more prominent increases than of chronic smokers. It is speculated that chronic exposure to cigarette smoke may cause "exhaustion" or "receptor down-regulation" of platelets and endothelial cells, resulting with diminished release reaction of platelets and endothelium in response to acute smoking.

Cyclosporin in steroid-resistant auto-immune haemolytic anaemia.

A case of Evans' syndrome which was refractory to conventional and high dose steroid treatment and to splenectomy was treated with cyclosporin, beginning with the dose of 10 mg/kg/day and then gradually tapering to 4 mg/kg/day. The patient experienced excellent benefit from cyclosporin; his haematological parameters were completely normal at the 12th month of therapy, without any side effect of the drug.

Beta-thromboglobulin and platelet factor 4 levels in pregnancy and preeclampsia.

The plasma levels of beta-thromboglobulin (BTG) and platelet factor 4 (PF-4) were measured in patients during the normal obstetric period and in preeclampsia. A significant increase was observed in the two proteins with respect to the nonpregnant control group. Both proteins increased significantly with preeclampsia and a further increment was observed with the severity of preeclampsia though no significant difference in PF-4 levels was encountered between pregnant and mildly preeclamptic women. There was no correlation between BTG, PF-4 and thrombocyte counts.
