Connatal (fetal) hydrocephalus: an acquired pathology?

We investigated the etiology of hydrocephalus present at birth, i.e. of fetal hydrocephalus. Both inherited and dysraphic major malformations are very rare. Intraventricular hemorrhages and viral infections during pregnancy are among the proposed etiologies; they are supported more by anatomical, physiopathological and experimental findings than by clinical evidence. Cases of fetal intraventricular hemorrhages cited in the literature are anecdotical, and the reports fail to identify maternal or fetal predisposing factors. The role of viruses in the etiology of connatal hydrocephalus has been postulated on the basis of epidemiological considerations in human pathology and of a considerable amount of experimental studies in animals. Investigations were generally focused on aqueduct ependyma, but research should also address other structures involved in the genesis of hydrocephalus (choroid plexus, extraventricular CSF pathways, including arachnoid villi). Furthermore, experimental evidence has emerged concerning a number of toxins and of drugs administered during pregnancy, which are thought to be involved in the genesis of hydrocephalus: once more, the conclusions reached in these experimental trials lend further credence to the human epidemiological data linking pregnancy disturbances with fetal hydrocephalus. Since most of these toxic agents are also thought to induce major malformations, we could assume the degree of their effect to depend on the embryonal stage affected: the earlier the action, the worse the malformation.

Lectin binding pattern of Schwann cells and macrophages in 2,5-hexanedione-induced axonal degeneration in rats.

The lectin binding pattern of both Schwann cells and macrophages has been studied during axonal degeneration induced in the rat sciatic nerve by chronic administration of 2,5-hexanedione (0.8 ml/kg per day i.p. for 20 days). In particular, the present study aimed to establish a possible relationship between macrophage activation and expression of lectin binding sites. To identify and distinguish between Schwann cells and macrophages, electron microscopy was combined with the lectin staining method. On 2,5-hexanedione injury, a drastic disorganization of both axon and myelin sheath occurred and nerve fibers were replaced by a chain of ovoids. Besides the well-established concept that Schwann cells and macrophages cooperate in the removal of the myelin debris during axonal degeneration, evidence is presented that expression of binding sites to lectins is closely related to macrophage activation. Monocytes occasionally present in control nerves were labelled only by Con A and sialidase-peanut sequence; in 2,5-hexanedione degeneration monocytes, prephagocytes (macrophages with minute bubbles) and phagocytes (macrophages with large bubbles) were labelled also by peanut, wheat germ and BSA I-B4; moreover, phagocytes were labelled by soybean as well, thus showing a clearly differentiation-dependent binding pattern. Since changes in lectin binding pattern may reflect changes in complex carbohydrate structures, the results show that the expression of certain glycoproteins may be closely related to activation of macrophages in response to toxic injuries.

Peripheral aneurysms of the anterior inferior cerebellar artery. Case report and review of literature.

Distal anterior inferior cerebellar artery aneurysms are rare. We recently treated one of these in a 46-year-old woman suffering from SAH and right facial and auditory deficit. The aneurysm was trapped 3 weeks after haemorrhage. Review of the pertinent literature revealed 36 similar cases which we analysed in detail. Guidelines for the proper management of these rare lesions are outlined.

Influence of ofloxacin, norfloxacin, nalidixic acid, pyromidic acid and pipemidic acid on human gamma-interferon production and blastogenesis.

Several new quinolone derivatives were investigated for their influence on human lymphocyte blastogenesis and gamma-interferon production following concanavalin A stimulation. All the antimicrobials induced inhibition of lymphocyte DNA synthesis. The gamma-interferon measurements showed that nalidixic acid and norfloxacin have a negative influence on lymphokine production and release.

The effect of NPT 15392, 9-(erythro-2-hydroxy-3-nonyl)-6-hydroxypurine, on the phytohemagglutinin of OKT3+, OKT4+, OKT8+, and OKM1+ cell-depleted and undepleted peripheral blood mononuclear cells.

NPT 15392, 9-(erythro-2-hydroxy-3-nonyl)-6-hydroxypurine, has been reported to influence immunological responses involving different cell types. Herein data are obtained by studying the influence of NPT 15392 on the phytohemagglutinin (PHA)-induced proliferative responses of unfractionated peripheral blood lymphocytes (PBLs) and of cell suspensions, which have been depleted of the cell subsets recognized by the monoclonal antibodies OKT3, OKT4, OKT8, and OKM1, in an attempt to identify which cell types respond to NPT 15392 in the PHA-driven blast transformation assay. The proliferative responses of unfractionated peripheral blood lymphocytes are potentiated when the drug is employed at the concentration of 0.1 microgram/ml and inhibited when NPT 15392 is added to the cell suspensions at concentrations over 5 micrograms/ml. The data reported here suggest that this phenomenon is a composite effect, made up of a combination of the counteracting effects caused by the OKT4+ cells on the one hand, and the OKT8+ and OKM1+ cells on the other.

Light and electron microscopic localization of acetylcholinesterase activity in the rat renal nerves.

Acetylcholinesterase activity is shown in the renal nerves of the rat with the technique of Karnovsky and Roots. By light microscopy, the acetylcholinesterase-positive nerves are seen in association with blood vessels, including the glomerular arterioles, and occasionally with renal tubules. By electron microscopy the precipitate appears extracellularly around axons and varicosities. DFP inhibits the deposition of precipitate. Previous demonstration by serial section electron microscopy in the rat revealed that all nerves around the glomerular arterioles contain small dense-cored vesicles characteristic of adrenergic nerves, indicating that the acetylcholinesterase-positive nerves demonstrated here are likely to be adrenergic nerves containing acetylcholinesterase.

Diabetic rats treated by low molecular weight heparin OP 2123/parnaparin: morphological changes in the kidney and heart.

AIM: Early morphological alterations in the rat kidney and heart due to experimentally induced diabetes are described in order to evaluate the possible therapeutic role of low molecular weight heparin (LMWH; OP 2123/parnaparin). METHODS: Our findings concern the alterations observed in the rat kidney and heart because these are the organs (together with the retina) mainly involved in the early morphological angiopathic modifications associated with diabetic damage of organs and tissues. In diabetic animals treated with LMWH, the Periodic Acid-Schiff (PAS) reaction showed a slight decrease when compared with the diabetic control group. Photographs were submitted to the quantitative analysis of images using a Quantimet 500 Image Analyzer (Leica) equipped with specific software. The following parameters were measured: (1) total area occupied by alkaline phosphatase (AP)-positive capillaries; (2) number and diameter of AP-positive capillaries; (3) distribution and total area occupied by PAS-positive structures (related to the intensity of the reaction resulting from the different amount of mucopolysaccharides). RESULTS: LMWH treatment is efficient in preventing these modifications, above all in the kidney. The histological study of the heart and kidney shows no significant, relevant alterations. However, the histological study of the mucopolysaccharides in diabetic animals highlighted a tendency for the heart to accumulate these substances. LMWH treatment only modestly reduced this accumulation. CONCLUSIONS: Previous evidence demonstrating a beneficial effect of therapy based on heparan sulphate proteoglycans and/or other heparin-like substances in insulin-dependent diabetes mellitus seems to be confirmed by our experimental results in different organs of adult rats. In fact, parnaparin treatment is effective (in our experience) for ameliorating the morphological pattern observed early in some diabetic tissues of rats and, above all, in the kidney.

Up-regulation of polysialic acid in peripheral myelinated axons of rat chronically exposed to 2,5-hexanedione.

Myelinated nerve fibres isolated from Wistar rats chronically exposed to 2,5-hexanedione (0.8 ml/kg/day, intraperitoneally) over a period of 20 days, were stained with lectin-horseradish peroxidase conjugates. The lectins with high affinity for terminal D-galactopyranosyl residues, Bandeiraea simplicifolia-B4 (BSA I-B4) and peanut agglutinin (PNA), showed glycoconjugates in the control nodes of Ranvier. In the treated animals, application of PNA-HRP caused weak reactivity to the node of Ranvier; digestion with sialidase prior to the application of PNA-HRP conjugate enhanced reactivity, thus revealing the presence of a sialoglycoprotein. The results indicate that glycoconjugates of the Ranvier node undergo a rearrangement during exposure to 2,5-hexanedione. In particular, neutral glycoproteins with terminal galactose are replaced by sialoglycoproteins. These findings are consistent with the proposed role of polysialic acid as a regulator of axonal behaviour during regeneration.

Anatomical variations of the ulnar and median nerves in the upper limb.

The aim of our study was the evaluation of the anatomy of ulnar and median nerves in the upper limb in order to ameliorate knowledge on the clinical anatomy of these nerves. In fact, further information on this topic may be useful owing to its possible clinical relevance when planning surgical anatomy and reconstructive surgery in tumor affected and injured patients. The relationships between ulnar and median nerve and neighbouring anatomical structures have been examined, together with the course and ramification of the ulnar and median nerves in six fresh cadavers. Moreover, we have performed a review of the literature. Four specific aspects were evaluated during dissection: 1) division modality of the ulnar nerve at the wrist; 2) anatomical details of the medial humeral epicondyle; 3) anatomical relationships between median nerve and retinaculum flexorum; 4) median-ulnar nerves anastomosis. Our results show that: the medial humeral epicondyle shows specific anatomical details in relation to the ulnar nerve; the relationships between the median nerve and the transverse carpal ligament may be characterized by one or two nerve trunks (two cases of bifid median nerve in our experience); median-ulnar nerve anastomosis may be also found at various levels. Comparing our results with those of the available literature we can conclude that anatomical variations of ulnar and median nerve in the upper limb are not an infrequent finding and their clinical, diagnostic and surgical relevance should be considered.

Effect of 5-fluorouracil on carcinoembryonic antigen expression and shedding at clonal level in colon cancer cells.

BACKGROUND: The carcinoembryonic antigen (CEA) is a tumor marker largely utilized for the detection of minimal disease or as a target of immunotherapeutic approaches. In preclinical models CEA has been found to be up-regulated after exposure of cancer cells to 5-fluorouracil (5-FU). In the present study, the clonal distribution of CEA and its regulation by 5-FU at clonal level was investigated using human HT-29 colon cancer cells. MATERIALS AND METHODS: The extent of CEA expression was measured in terms of: (a) antigen levels on plasma membrane, by flow cytometry; (b) cytoplasm and membrane protein, by Western blot analysis: (c) transcript, by Northern blot analysis; (d) CEA shedding by radioimmunossay. RESULTS: CEA protein and gene transcript were variably expressed among different clones. In all cases 5-FU was able to increase the percentage of CEA-positive cells, the amount of antigen, either in the membrane or cytosolic fractions, and the corresponding transcript. Moreover, a marked increase of CEA shedding was found in drug-treated cells with respect to that of controls. The increase of CEA induced by the antimetabolite was not the result of a selection mechanism based on preferential killing of CEA negative cells. The antimetabolite was capable of enhancing antigen expression also in other CEA-positive tumor cell lines with different basal levels of the marker. CONCLUSIONS: The present findings could be of potential value to increase the sensitivity of diagnostic procedures based on detection of CEA positive tumor cells. Moreover, the antimetabolite might be included in immunotherapeutic protocols to facilitate recognition of CEA-positive cancer cells by immune responses.

[The lung in AIDS].

The aim of this study was to underline the important role played by respiratory tract pathologies in HIV-infected subjects and to evaluate the incidence of respiratory pathologies in HIV-positive subjects in the province of Cosenza. After examining recently reported data, the authors analyse the patients studied by the Infectious Disease and Pneumology Divisions of the United Hospitals of Cosenza with the collaboration of the Virology Unit over the two-year period 1992-1994. As well as the lower incidence of HIV infection in this area compared to the rest of Italy, the paper highlights the reduced percentage of concomitant respiratory pathologies. The authors attempt to interpret the significance of these results.

[Update on HIV infection in the province of Cosenza. HIV infections and respiratory disease].

The aim of the present study was to evaluate the incidence of HIV infection and concomitant pathologies, paying special attention to those involving the respiratory tract, in the Province of Cosenza through a retrospective study of patients monitored by the Divisions of Infectious Diseases and Pneumology with the collaboration of the Virology Unit of the United Hospitals of Cosenza. The results obtained show that there is a lower incidence of AIDS and HIV-correlated respiratory pathologies in this area compared to the rest of Italy but, in spite of this, the authors highlight the need in Calabria for those centres and structures that have been planned but never completely realised (Law 135/90; DPR 7/4/94; etc.) which would enable infected subjects to be identified and the necessary measures for prevention, diagnosis and treatment to be implemented.
