Smokeless tobacco use in adolescents: the Cardiovascular Health in Children (CHIC II) Study.

Smokeless tobacco has seen a resurgence of popularity among adolescents despite its association with oral cancer and altered cardiovascular function. This study examined age, gender, ethnicity, self-esteem, physical activity, parental smoking, and socioeconomic status as predictors of smokeless tobacco use among middle school children. Subjects included 1,211 youth (White (64%), Black (24%), Hispanic (6%), and Other (6%); age 12.2) participating in the Cardiovascular Health in Children and Youth (CHIC II) study. All data were collected by questionnaire. Factors related to ever using smokeless tobacco included older age (p < .001), being male (p < .001), lower self-esteem (p < .001), and having parents who currently (p = .02) or formerly (p = .05) smoked. Hispanics reported a higher current usage rate than other ethnic groups (p < .001). White youth in the lowest socioeconomic status were most likely to be experimental users (p = .007), while those in the high socioeconomic status were more likely to be current users (p = .006). Physical activity was not associated with smokeless tobacco use.

Cardiovascular disease risk factors and obesity of rural and urban elementary school children.

Previous studies on the influence of a rural/urban setting on the prevalence of cardiovascular disease risk factors in children have not sufficiently controlled for socioeconomic status, race, gender, and perhaps, may not have included a representative sample of rural and urban children. This study compared the cardiovascular disease risk factors and rate of obesity of children living in rural and urban settings. It also determined the magnitude of the effect of the rural/urban setting on cardiovascular disease risk factors and obesity when controlling for race, socioeconomic status, and gender. The subjects were 2,113 third- and fourth-grade children; 962 from an urban setting and 1,151 from a rural setting. Height, weight, skinfolds, resting blood pressure, and total cholesterol levels were measured. Aerobic power (pVO2max) was estimated from cycle ergometry. Physical activity and smoking history were obtained from a questionnaire. Clustering analyses using adjustment for sample error indicated that total cholesterol, blood pressure, smoking, and physical activity levels of rural and urban children were not different (P > 0.10); however, body mass index and sum of skinfolds was greater for rural youth (P < 0.004). Logistic regression indicated that rural children had a 54.7 percent increased risk of obesity (P = 0.0001). This study's results indicate that, in children, a rural setting is associated with obesity, but not with the major risk factors associated with cardiovascular disease.

[Polymorphisms of five short tandem repeat systems in Chinese Han population in Chengdu].

OBJECTIVE: To get preliminary genotype and allele frequency distributions of D6S366, D6S477, D12S375, D12S391 and PLA2A loci in Chinese Han population in Chengdu area and to validate more short Tandem repeat (STR) systems for forensic application. METHODS: The polymerase chain reaction (PCR) was used. Five STR systems (D6S366, D6S477, D12S375, D12S391 and PLA2A) were amplified on DNA samples, which were extracted from 108 unrelated individuals EDTA-blood by Phenol/Chloroform method. The PCR products were analyzed by PAG vertical electrophoresis. RESULTS: Seven alleles were found at D6S366 locus, nine alleles at D6S477 locus, five alleles at D12S375 locus, nine alleles at D12S391 locus and seven alleles at PLA2A locus. No deviations from Hardy-Weinberg equilibrium were observed. The heterozygosities observed were 0.69, 0.78, 0.81, 0.68 and 0.79 for D6S366, D6S477, D12S375, D12S391 and PLA2A respectively. The chances of exclusion were 0.3621, 0.6004, 0.4581, 0.7014 and 0.5589 and the discriminating powers were 0.79, 0.93, 0.86, 0.96 and 0.91. CONCLUSION: All of the five loci in this study were useful markers for individual identification and paternity test and for genetics purposes.

[Fiberoptic bronchoscopy in diagnosis and treatment of inhalation injury].

OBJECTIVE: To explore the morphological standard of diagnosing inhalation injury, the feasibility of early diagnosis and the topical therapy of inhalation injury through fiberoptic bronchoscopy. METHODS: In one hundred and forty four patients with inhalation injury that were admitted in 10 years were examined with bronchoscopy, and the results were analysed. RESULTS: Diagnostic criteria and classification were obtained with the aid of the bronchoscopy. It was found feasible to make early diagnosis within 8 h and also to exercise treatments in the form of suction, irrigation, etc. CONCLUSION: Bronchoscopy plays an important role in the early diagnosis and treatment of inhalation injury, and it should be used more extensively in patients suspected to have inhalation injury.

[The percentage of free prostate specific antigen used in detecting prostate cancer].

OBJECTIVE: To evaluate the value of the percentage of free prostate specific antigen in detecting prostate cancer. METHODS: Serum free prostate specific antigen (F-PSA) and total PSA were measured with immunoradiometric assay, and the free/total PSA ratio was calculated. In 117 patients, 31 had prostate carcinoma and 86 BPH. RESULTS: The free/total PSA ratio was lower in patients with prostate cancer than in those with BPH. If the total serum PSA level was limited with in 4 - 10 ng/ml, the free/total PSA ratio can be used in the differential diagnosis of prostate cancer and BPH except total PSA. CONCLUSIONS: The free/total PSA ratio can be used in the differential diagnosis of prostate cancer and BPH, especially when serum PSA level is limited with in 4 - 10 ng/ml.

[Indirect determination of berberine hydrochloride by atomic absorption spectrometry].

A method for the indirect determination of berberine in tabellae berberine hydrochloride by ion pair extraction and atomic absorption spectrometry was described. Berberine reacted with inorganic complex tetrathiocyanate cobalt(II) to form a stable ion pair associated complex at certain pH value. The ion pair associated complex was extracted with organic solvent and then transferred into aqueous solution. The content of cobalt in the solution was measured by flame atomic absorption spectrometry and the amount of berberine hydrochloride was calculated according to the content of cobalt. The result of the present method agreed with pharmacopeia method. This method is sensitive, dependable, precise, and applicable.

[Effect of nitroquine on the membrane phospholipid of intraerythrocytic Plasmodium yoelii in vitro].

AIM: To study the mechanism of antimalarial action of nitroquine. METHODS: Intraerythrocytic P. yoelii was cultured by the method of Trager and Jensen. The amount of [3H]-ethanolamine incorporation was measured as an index of the phospholipid synthesis. DPH was used as a probe to measure the plasmodial fluorescent polarization. RESULTS: The incorporation of [3H]-ethanolamine into the P. yoelii infected erthrocytes was markedly inhibited by nitroquine. The plasmodial membrane polarization and viscosity were significantly increased by nitroquine. CONCLUSION: Nitroquine could inhibit the phospholipid synthesis and decrease the membrane fluidity of P. yoelii.

[Studies on allelic zymogram of periodic Brugia malayi from Zhejiang Province].

AIM: To study the allelic zymogram of periodic B. malayi from Zhejiang Province. METHODS: 180 adult worms(male 60, female 120), 32,000 microfilaria and 1,500 infective larvae of periodic B. malayi were examined for the isoenzymes of 14 enzymes by horizontal starch gel electrophoresis with 14 different isomyme. RESULTS: Twenty-seven allelic loci were found in three developing stages of B. malayi, most of them were homozygotes, however, two of them(MPI,MDH) were heterozygotes(7.4%). 17, 19 and 9 loci were presented in adult worms, microfilariae and infective larvae, respectively. At the same time, the allelic zymogram of B. malayi from Guizhou Province was also examined. CONCLUSION: The allelic zymogram of periodic B. malayi from Zhejiang Province was similar to that of periodic B. malayi from Guizhou Province but passaged in the laboratory.

[Study on the determination of aluminum in soil samples by air-acetylene flame atomic absorption spectrometry].

This paper reported a method for the direct determination of aluminum in soil samples by air-acetylene flame atomic absorption spectrometry. The enhancement effect on aluminum of adding tetramethylammonium chloride was studied. 43 microg/mL/1% of characteristic concentration was reached. This method is simple, rapid and useful, and has been applied to the determination of aluminum in standard soil samples, and a good agreement was achieved.

Inhibition of leukocyte adherence and transendothelial migration in cultured human liver vascular endothelial cells by prostaglandin E1.

Primary graft dysfunction is a major complication of orthotopic liver transplantation, and hepatic ischemic reperfusion injury is considered to be its major determinant cause. Although oxygen free radicals play an important role, leukocytes, cytokines, and adhesion molecules also contribute to hepatic ischemic reperfusion injury. Prostaglandin E1 (PGE1) has been shown to protect against impairment and dysfunction of transplanted livers in various experimental models as well as in clinical liver transplantation. In this study, the role of PGE1 on leukocyte adherence and transendothelial migration was investigated in cultured human liver vascular endothelial cells (HLVEC). Our results indicated that stimulated, but not resting, leukocytes exhibited high adhesion and transmigration capacity. HLVEC incubated with tumor necrosis factor (TNF) promoted leukocyte adherence and transendothelial migration. PGE1 inhibited leukocyte adherence to HLVEC when it was preincubated with either HLVEC or leukocytes. Moreover, PGE1 also suppressed stimulated leukocyte transendothelial migration in a dose-dependent manner. The inhibitory activity of PGE1 was further investigated on both HLVEC and leukocytes with attention to adhesion molecules. On HLVEC, PGE1 down-regulated TNF-induced expression of endothelial cell leukocyte adhesion molecule 1 and vascular adhesion molecule 1, but not intercellular adhesion molecule 1. On leukocytes, PGE1 inhibited expression of CD11a/CD18 and membrane-bound TNF on PHA-stimulated leukocytes. PGE1 also suppressed TNF release from the stimulated leukocytes. These results indicated that inhibition of leukocyte adherence and transendothelial migration is one of the mechanisms by which PGE1 protects liver grafts.

Cloning and characterization of a Rhizobium meliloti nonspecific acid phosphatase.

Nodulated legumes require high levels of phosphorus for optimal symbiotic performance. However, the basis for this elevated phosphorus requirement is poorly understood, and very little information regarding bacteroid phosphorus metabolism is available. To develop an understanding of the relative importance of organic and inorganic phosphorus sources for bacteroids, we investigated phosphatase activity in Rhizobium meliloti. An R. meliloti plasmid library clone that complemented an Escherichia coli phosphatase mutant was isolated, and the clone was sequenced. The complementing fragment contained a 337-amino-acid open reading frame that has a potential leader sequence and processing sites characteristic of periplasmic proteins. The phosphatase activity was located in the periplasm of R. meliloti and of E. coli containing the cloned gene. The subunit molecular mass of the cloned phosphatase was 33 kDa, and gel filtration indicated the active enzyme was a 66-kDa homodimer. Lack of substrate specificity suggests the cloned gene, napD, encodes a nonspecific acid phosphatase with a pH optimum of approximately 6.5. An R. meliloti napD transposon-insertion mutant was constructed, and its symbiotic phenotype was determined to be Fix+ regardless of the level of phosphorus provided to the host plant.

Organ transplantation in the twenty-first century.

Major advances in the understanding of the immunologic process responsible for organ or cellular transplant rejection, a dramatic improvement in available immunosuppressive drugs, development of more sophisticated surgical techniques, and important progress in posttransplant intensive care over the last 30 years have led to a remarkable improvement in success following organ transplantation. Whereas excellent short-term survival of most transplanted organs is readily achieved, graft loss because of chronic rejection and the worsening problem of organ donor shortage remain major concerns in the field of transplantation. Recent advances in immunosuppressive drugs, induction of immunologic tolerance, and gene therapy strategies may help to prolong organ allograft survival in the future. Revised criteria for organ donation and xenotransplantation may one day solve the problem of organ supply. Today, as we approach the next millennium, we are optimistic that the elusive goal of immunologic tolerance will be achieved and perhaps applied to animal tissue. Such will certainly be the challenge for the next century.

Antigen mimicry by an anti-idiotypic antibody single chain variable fragment.

For the therapy of cancer patients whose disease is positive for Carcinoembryonic Antigen (CEA), we developed an active specific immunotherapy based on the idiotypic network. The anti-idiotype monoclonal antibody (mAb), 3H1 was generated by immunization of mice with the anti-CEA mAb, 8019. 3H1 mimics CEA both functionally and structurally and acts as a surrogate for CEA. To define the minimum structural requirements for antigen mimicry by 3H1, we constructed plasmid vectors for expression of single chain Fv (scFv) variants of 3H1 in Escherichia coli. Variable heavy (VH) and variable light (VL) chain domains of 3H1 were linked by a 15 amino acid linker (Ln), (Gly4Ser)3 in two constructs, VH-Ln-VL and VL-LnVH. Ln was omitted in two constructs, VH-VL and VL-VH. Each of the scFv constructs has a tag of six His [(His)6 tag] for purification by metal chelate affinity chromatography and detection by enzyme-linked immunoabsorbent assay (ELISA). Comparisons of the binding of 8019 to purified scFv proteins by ELISA and immunoblot experiments showed that only VH-Ln-VL had significant activity. VH-Ln-VL also showed maximum inhibition of binding of 8019 to CEA. Immunization of mice with naked VH-Ln-VL and VH-Ln-VL conjugated to keyhole limpet hemocyanin induced anti-CEA antibodies in mouse sera. Sera from immunized mice inhibited the binding of 8019 to 3H1 as well as CEA. Induction of anti-CEA antibodies in the immunized mice was confirmed by flow cytometric analysis using CEA positive MC-38cea cells. These results demonstrate that for antigen mimicry of 3H1 scFv, the presence of Ln is necessary and the domain order should be VH followed by VL.

Smoking initiation in youth: the roles of gender, race, socioeconomics, and developmental status.

PURPOSE: To describe smoking initiation, and to investigate factors that predict the early initiation of smoking in schoolchildren using a longitudinal approach. METHODS: A prospective study of smoking habits of children from the third and fourth grades through the eighth and ninth grades. The initial study population was 1970; 79.8% were white and 20.2% were African-American. Children were classified as "nonsmokers," "experimental smokers," or "current smokers" at five time points over 6 years. Multivariate regression models examined relationships of demographic and developmental factors with smoking initiation. RESULTS: Experimental smoking increased from 4% at Grades 3-4 to 42% at Grades 8-9, and current smoking prevalence rose from 0.4% to 9% over the same period. The mean age of initiation of smoking was 12.3 years. Smoking initiation (experimental smoking) was significantly different by racial group, socioeconomic status (SES), and pubertal development. White children and those of low SES were more likely to be experimental smokers, and also started earlier than African-American children and children of high SES. Once they started, white children advanced more rapidly to become current smokers. Boys had a higher prevalence of experimental smoking than girls at all time points. Children in rural areas were more likely than urban children to start smoking after age 12 years. Children who were at a higher pubertal stage than their peers were also more likely to experiment with smoking. CONCLUSIONS: Race, SES, and pubertal stage are important predictors of initiation of smoking in schoolchildren. This study indicates a need for smoking prevention classes in elementary and middle school, especially in areas with large numbers of white and low-SES youth. Also, smoking cessation programs, as well as smoking prevention classes, would be useful for middle school and high school students.

Eating habits, activity, lipids and body mass index in Japanese children: the Shiratori Children Study.

OBJECTIVE: To describe the eating habits and physical activity of Japanese children aged 10 y, and to investigate their relationship to lipids and body mass index (BMI); also to study the relationship between BMI and lipids in these children. DESIGN: This was a cross-sectional study conducted at six schools in Shiratori, Japan during 1992-1994. Physical examinations and questionnaires were used to collect data from 457 fifth grade children, aged 10 y. RESULTS: The diet of these children was mixed, with Japanese and Western foods almost equally reported. Boys were more active than girls. More active children had higher high density lipoprotein cholesterol (HDL-C), and children who ate a more Japanese diet had slightly lower HDL-C. BMI was positively related to cholesterol, low density lipoprotein cholesterol (LDL-C), triglycerides and inversely related to HDL-C. The amount of rice intake was positively associated with the BMI of these children. CONCLUSION: In our study of Japanese children aged 10 y, their diets were fairly Westernized, and most children were fairly inactive based on our activity score. Although obesity is low in these young Japanese subjects (14.1%), children who were obese (BMI>20) had worse lipid profiles than those who were not obese. An exceptionally high rice intake was associated with obesity, and thus may be an early warning sign for future obesity.

[Study of macroencapsulated islet xenografts for treatment of diabetes in mice].

The aim of this study was to prevent rejection after islet xenotransplantation. Islet obtained from Wistar Furth rats were macroencapsulated with agarose and collagen before transplanted into diabetic C57BL/6, B6AF1, and BALB/c mice. The results showed that the diabetic condition was reversed in 92.3% of recipients transplanted with macroencapsulated islets. The normoglycemic state of recipients was maintained 125.8 +/- 57.9 days without immunosuppression. The glucose tolerance curves in these mice were similar to those of normal mice. The macroencapsulated islets retrieved 103 days after transplantation showed no evidence of tissue reaction or fibrosis. These indicated that the macroencapsulated islets serve both to achieve and maintain normoglycemia of recipients, as well as to protect islet xenografts from rejection.

Adenoviral transfection of isolated pancreatic islets: a study of programmed cell death (apoptosis) and islet function.

Gene therapy provides a potential technique to modify immunity in vitro and therefore may prolong graft survival in vivo. However, viral infection and gene transfer may damage target cells and interfere with biologic function. Viruses, including adenovirus, are known to be capable of modulating apoptosis and initiating cell death by either inducing or suppressing specific processes, depending on the virus and cell system studied. The effect of adenovirus on islet cell viability and function has not been examined in detail. In this study, the dose-dependent effect of an adenoviral vector on islet cell death and glucose-stimulated insulin secretion (GSIS) was investigated to establish a therapeutic window for the dose of viral vector administered. Isolated pancreatic rat islets were incubated with an adenovirus expressing a beta-galactosidase gene (AdHCMVsp1LacZ) at different viral concentrations [multiplicity of infection (MOI) 1:10, 1:100, and 1:1000]. Transfection rate, in vitro and in vivo islet viability, and occurrence of programmed cell death were determined 1, 3, and 7 days after transfection. Islets, transfected at MOI 1:10 and 1:100, demonstrated apoptosis not significantly different from nontransfected controls. Islets, transfected at MOI 1:1000, demonstrated a significant increase in apoptosis at 24 hr, which decreased over 7 days of culture. The increase in apoptosis was not reflected by a significant decrease in in vitro GSIS of surviving islet cells, as assessed by stimulation index following in vitro perifusion. SCID or nude mice transplanted with AdlacZ-transfected islets (MOI 1:100 and 1:1000) remained normoglycemic for > or = 30 days. These results demonstrate that transfection of islets using adenoviral vectors can be manipulated such that efficient expression of the gene product encoded by the transfected gene (beta-galactosidase) can be achieved at lower transfecting concentrations of the adenoviral vector (MOI 1:10, 20.2%; MOI 1:100, 30.7%) while preserving islet function. This efficiency of transfection may allow pretransplant manipulation of isolated islet cells without vector-specific alteration of islet function. In cases where high virus concentrations are required for efficient gene transfer (adequate expression of the transgene product), a deleterious effect of the vector on islet cell function, with increased cell loss due to increased apoptotic events, is predicted. Using the AdlacZ vector, cell loss by apoptotic mechanisms appears limited to the first days following coculture with high viral concentrations, and does not appear to influence in vitro or in vivo cell function of the surviving islet cells.
