Uterine metastases from breast cancer in a patient under tamoxifen therapy. Case report.

The authors report a case of metastases of breast cancer confined to the uterus and cervical subserous leiomyoma in a postmenopausal woman under tamoxifen therapy for two years. After abnormal uterine bleeding, pathologic examination on biopsy of a cervical polyp revealed cervical involvement secondary to lobular breast cancer. The patient underwent total abdominal hysterectomy with bilateral salpingo-oophorectomy. Pathologic examination of the surgical specimen revealed both invasion of breast origin and of the cervix until the isthmus, endometrium and cervical subserous leiomyoma. The adnexa uteri were not affected. The possibility of uterine metastases in patients suffering from breast cancer, either undergoing tamoxifen therapy or not, always has to be considered.

Effectiveness of "core biopsy" by the mammotome device for diagnosis of inflammatory carcinoma.

OBJECTIVE: To assess the usefulness of "Mammotome" device for the diagnosis of the inflammatory breast carcinoma. MATERIAL AND METHODS: We studied 6 patients, aged 43-79 years, with clinical evidence of inflammatory breast carcinoma. We compared two sampling techniques, a cytologic one by Fine Needle Aspiration (FNA) and a microhistologic one by "Mammotome". RESULTS: Cytologic sampling by FNA permitted certain diagnosis of malignant lesions in 2 out of 6 cases, while the "Mammotome" device confirmed the correct diagnosis in all 6 considered cases. CONCLUSIONS: The "Mammotome" device proved more useful in the diagnosis of inflammatory breast carcinoma than FNA and it can be a valide alternative to surgical biopsy.

Oligomenorrhea treatment by purified FSH using a fixed protocol.

This study is designed to evaluate the effectiveness, safety and tolerability of a low dose of purified FSH administration using a fixed protocol in oligomenorrheic patients not desiring pregnancy. We used a low dose (75 IU/day for 5 days) of purified FSH (Metrodin) in 10 oligomenorrheic patients (aged 18-30) for 25 cycles. The onset of menstruation occurred in 7 patients (70%) and in 19 treatment cycles (76%), the ovulation was verified in 5 of these patients (50%) for 13 cycles (52%). One patient had spotting after the treatment, 2 patients did not have any response. The treatment did not cause side effects and it showed a good tolerability.

Cyclical variation of plasma lipids, apolipoproteins, and lipoprotein(a) during menstrual cycle of normal women.

We report herein the effects of cyclical variations of endogenous sex steroids during the menstrual cycle on plasma lipids and apolipoproteins (apo) in normal women. We examined 16 normal women (age range 25-36 yr) with normal menstrual cycles of 28-31 days. The study covered the period from the 1st day of a menstrual phase (basal) until the 1st day of the following menstrual phase. During the study all women maintained a normolipidic diet (30% fat). Plasma total cholesterol and low-density-lipoprotein cholesterol were significantly higher than basal in the preovulatory phase until progesterone started to increase in the postovulatory phase [day +8 from luteinizing hormone (LH) surge]. High-density-lipoprotein cholesterol was significantly higher than basal from day -1 to the day after LH surge, whereas plasma apoAI levels were significantly higher from day -8 to day +8 (from LH surge). Plasma apo(a) increased significantly during the luteal phase in four women characterized by a single S4 band and lower basal plasma levels of apo(a). Our results indicate that endogenous female sex steroids have significant effects on the circulating levels of plasma lipids and apolipoproteins, including apo(a). More work needs to be done to elucidate the significance of the observed apo(a) changes, and the different phases of the menstrual cycle must be taken into account when evaluating the lipidic risk profile in premenopausal women.

[Echographic monitoring of follicular growth].

The sonographic monitoring of the ovarian follicular growth has been widely adopted, being a non invasive method, easily repeatable, well tolerated by the patient and low cost. A further impulse has been obtained by the endovaginal sonography. During a natural menstrual cycle the sonography can highlight the alterations that occur to the follicle and to the endometrium, supplying useful information on ovulatory timing. In pharmacologically induced menstrual cycles, the sonography enables us to evaluate the ovarian response to the therapy in tennis of number, size, site, and the type of growth of the recruited follicles, enabling us therefore to modulate the stimulation according to the individual response. Fluximetric studies are among the new applications the most promising, since they allow us not only to identify quickly the vascular structures, but also to supply a predictive value for the embryo implantation and the pregnancy evolution. The sonography at present proves to be irreplaceable in assisted reproduction programmes.

[Ovulation induction by pulsatile intravenous administration of Gn-RH to patients with hypothalamic amenorrhea].

Intravenous pump administration (Zyklomat, Ferring) of GnRH (Lutrelef, Ferring) has been used to induce ovulation in 16 patients suffering from hypothalamic amenorrhea between January 1989 and December 1991. One to 6 cycles were performed for a total of 47 cycles. GnRH pulses were set at 20 micrograms/pulse every 90 min up during the follicular phase and decreased to 1 pulse/120 min during the 3-5 days following ovulation. Thereafter, the luteal phase was supported by 1000 IU HCG/48 h up to the end of the 5th week. Response to the treatment was monitored with seriC E2, P and LH and vaginal echography. Cycles (N = 3) were cancelled when E2 did not reach at least 0.25 nmol/l after 30 days. An E2 level > 0.8 nmol/l, associated with a mature follicle (> 20 mm), and the presence of a LH peak or an increased progesterone level were used to determine the day of ovulation. In case of normozoospermy (N = 9), sexual intercourse was recommended to the patients (27 cycles), whereas an intrauterine insemination was performed in case of a male factor (N = 7, 20 cycles). Results can be summarized as follows: 39 (83%) cycles were ovulatory and led to 9 (19%) clinical and 6 (13%) biochemical pregnancies. Five (8.5%) pregnancies aborted and 5 (11%) came to full term. After exclusion of the male factors, the pregnancy rate reaches 26%/initiated cycle, 28%/ovulatory cycle and 56%/patient. No patient developed a hyperstimulation syndrome or a multiple pregnancy. Three cycles (6.3%) were interrupted due to 2 superficial phlebitis and 1 pump failure.(ABSTRACT TRUNCATED AT 250 WORDS)

The treatment of cervical carcinoma in the Sardinian population.

The epidemiological and clinical characteristics of cervical carcinoma in Sardinia were examined, with particular regard to the low incidence (6.47/100,000 women) of these neoplasias. Postsurgical adjuvant Radiotherapy in the high-risk cases has allowed a constant improvement in therapeutic results, with a low relapse-incidence and a high percentage of survival, especially in first and second stage treated cases.

Postcoital test after vaginal washing with NaHCO3.

The Authors treated with vaginal washing of NaHCO3 (Sodium Bicarbonate) 55 patients whose postcoital tests were persistently poor or negative. The cervical pH increase made the test positive in about 30% of cases and three pregnancies occurred. This treatment with NaHCO3 proved to be useful, handy and welcome to patients.

Our experience regarding precautional chemotherapy in breast cancer.

14 patient with breast cancer of loco regional diffusion (M0), 4 of whom had negative axillary lymph nodes, were administered precautional chemotherapy with CMF, on the basis of anatomo-clinical prognostic elements (size of the neoplasia, grading (tumoral limits, neoplastic embolisation etc.). Such elements with data on hormonal receptors allowed us to select the patients with high relapse risk, to undergo medical treatment.

Evaluation of urinary estriol by latex agglutination inhibition reaction.

The urinary estriol values of a morning sample determined by a new semiquantitative method was correlated with the total 24 hour estriol excretion determined by colorimetric method on patients with normal and pathologic pregnancy. The correlation coefficient, calculated on 240 assays operated on urine samples obtained from 70 normal pregnant women, is highly significant statistically (P less than 0.001). The profiles of the serial assays, obtained by these two procedures on patients with pathologic pregnancy, show a very strong analogy. The Authors believe that such a method, just for its simple and rapid realization, may be applied alternatively to RIA assay or colorimetric method for the cases which are believed to be in need of rapid evaluation of estriol and/or when another method is unfeasible.

Alpha-fetoprotein assay in patients treated with low-dose oral contraceptives.

AFP has proven to be a useful marker for liver neoplasms. The authors assayed AFP plasma levels in patients taking low-dose estro- progestinics . Patients were divided into three groups on the basis of their living habits. No difference was found in any group between AFP levels before and after treatment. However, in the group of smokers using oral contraceptives AFP levels were significantly higher.

Prolactin plasma levels and oral contraceptives at low dosage.

In the present study the Authors consider whether estroprogestins at low dosage may influence the basal levels of plasma Prolactin in a group of patients subdivided on the basis of their different ways of life. The results show that oral contraceptives containing 30 microgrammes of EE do not induce modifications on PRL levels; however these are significantly (p less than or equal to 0.01) lower in the smoker group. It has therefore been hypothesised that smoke may interfere with the regulatory mechanisms of PRL, even if the practical effects of such action in the ambit of the physiopathology of reproduction is not yet clear.
