[Creatine kinase in patients with diabetes mellitus].

In 72 diabetic patients the serum levels of creatine kinase (CK) and its isoenzyme MB were examined together with the levels of other enzymes most frequently examined in practice. A transient elevation of the CK level, mainly of the muscular isoenzyme, was found in 12.5% of the patients examined. In none of these patients lesions of the myocardium, thyroid gland or the striated muscles, which could explain the rise of CK, were found. There was a moderate correlation between the degree of glycemia and the CK elevation but there was no such correlation with other enzymes. With improvement of the carbohydrate metabolism the CK level became normal. The possible pathogenic mechanisms for the rise of the CK level in diabetes mellitus are discussed. The limited number of the patients examined does not allow final conclusions about these mechanisms.

[The limits of the reference range of serum enzymes].

The reference ranges of some of the most frequently used in the clinical laboratory practice serum enzymes and isoenzymes are determined: aspartate aminotransferase (ASAT), alanine aminotransferase (ALAT), creatine kinase (CK) and its isoenzyme (CK-MB), lactate dehydrogenase (LDH) and its isoenzyme--HBDH, and alkaline phosphatase (APh). The reference ranges of the enzymes and isoenzymes studied are calculated from the results obtained in 182 clinically healthy persons, 20-50 years of age, by the "single test" method. All analyses are made with the flexible discrete analyzer "Technicon PA 1000". The influence of several factors is studied. The age of the patients does not exert any influence on the enzymes and isoenzymes studied. Statistically significant sex dependent differences are established for CK, ASAT, ALAT and APh. The statistical processing of the results for the determination of the reference ranges of the enzymes and isoenzymes studied is accomplished with a computer "Hewlett Packard 85".

[Determination of angiotensin-converting enzyme activity in internal diseases].

A convenient and reliable colorimetric method for determination of serum angiotensin-converting enzyme is worked out by modifying the method of Niels and al. Reference serum ranges of 15.45-191.2 ME/L are established in a control group of 46 healthy persons. In pulmonary sarcoidosis statistically significantly higher angiotensin-converting enzyme activity than in the healthy controls is found while in pulmonary cancer and other pulmonary diseases the angiotensin-converting enzyme activity is in the reference ranges. The importance of angiotensin-converting enzyme as a laboratory index for the early diagnosis of pulmonary sarcoidosis is discussed.

[Intralaboratory quality control over random and systematic errors using the combined Shewart-cusum check card].

The method of the within-the-laboratory quality assurance check of the casual and systematic mistakes in the analytical process is presented. The results of the control serums are evaluated by the combined Shewart-cusum check card which is suitable for both casual and systematic mistakes while the classical check card controls the casual mistakes only. The method was used for evaluation of the analytical quality of the results determined by the automatic analyzer "RA 1000 Technicon". The general assessment of the combined method is that the systematic mistakes found are very often of no medical importance but in any case they are an "useful" information for the laboratory specialists in their efforts to provide a reliable laboratory information.
