Molecular characterization of the t(14;19)(q32;q13) translocation in chronic lymphocytic leukemia.

The t(14;19)(q32;q13) is a recurring translocation found in leukemic cells of some patients with chronic lymphocytic leukemia (CLL). The BCL3 gene was identified on chromosome 19 adjacent to the breakpoint of the translocation, and has been proposed to be a candidate proto-oncogene which may play a role in leukemogenesis. The current study of a Japanese patient with CLL revealed that the (14;19) is reciprocal at the molecular level; the BCL3 gene was juxtaposed to the 5' side of the S alpha 1 switch region of the immunoglobulin heavy chain gene (IGH) on the der(14) chromosome, and the IGH gene 5' to the S alpha 1 region was joined to chromosome 19 sequences on the der(19) reciprocal partner chromosome. The breakpoint on chromosome 19 was 16 kb upstream of the first exon of the BCL3 gene and 7 bp of chromosome 19 sequences were deleted at the point of the junction. The t(14;19) translocations so far molecularly analyzed consistently occurred within one of the two S alpha switch regions; however, sequence analysis of the chromosome 19 regions involved in the translocation failed to demonstrate an obvious sequence similarity with the switch region. The chromosomal breaks on chromosome 19 from two CLL patients having the t(14;19) were within Alu repeated sequences.

A Bonhoeffer-van der Pol oscillator model of locked and non-locked behaviors of living pacemaker neurons.

A recent investigation of the influence of periodic inhibitory trains on a crayfish pacemaker neuron showed not only well-known locked periodic responses but also intermittent, messy, and hopping responses. This communication studies the responses of the Bonhoeffer-van der Pol (BVP) model with self-sustained oscillation when exposed to periodic pulse trains inputs. The analysis is similar to that used in crayfish and reveals interesting features, both comparable and complementary to those seen in the living preparation.

[Lymphocyte subpopulation in asthmatic children with chronic bronchial obstruction].

Lymphocyte subpopulations in peripheral blood were examined in asthmatic children, using fluorescent monoclonal antibodies. The patients were divided into three groups: Group I, no attack (FEV1.0% > or = 80% group), Group II, no attack (FEV1.0% < 80% group) and Group III, asthmatic attack group. The percentage of CD3HLADR/CD3 in the Group II and Group III were 5.58 +/- 3.59 and 5.44 +/- 4.34, respectively, which were significantly higher than that in the Group I subjects (2.70 +/- 2.17). This finding suggests that the patients with long lasting bronchial obstruction may have chronic inflammation in the airway even in the remission stage of asthmatic attacks.

[Efficacy of long-term beclomethasone dipropionate inhalation therapy in pediatric patients with asthma, and its effect on height growth and adrenocortical functions].

Seventy-eight pediatric patients with moderate to severe chronic asthma, aged 6 to 16 years, who failed to respond well to inhaled DSCG and theophylline RTC, were treated with beclomethasone dipropionate inhaler (BDI) for 0.5 to 10 years (mean: 4.2 +/- 2.4 years). The efficacy rate gradually increased with time: 61.1% at one year after the start of the treatment and 89.5% at three years. Long-term BDI therapy over five years did not cause suppression of height growth or adrenocortical functions (early morning cortisol level and rapid ACTH test). However, the safety of long-term BDI therapy in children still needs to be studied more thoroughly and established. Further investigation will be necessary since some cases did not respond to BDI therapy while others died of exacerbated asthmatic symptoms soon after the start of BDI therapy.

Inhibition of chemical carcinogenesis in vivo by azatyrosine.

A single painting of 7,12-dimethylbenz[a]anthracene on the skin of transgenic mice harboring the human protooncogene c-Ha-ras induced papillomas at 100% incidence after 20 weeks (M. Izawa et al., unpublished data). Application of L-beta-(5-hydroxy-2-pyridyl)alanine (azatyrosine) to the skin at a dose of 2 mg/mouse once every 3 days after initiation with 7,12-dimethylbenz[a]anthracene greatly reduced the percentage incidence, number per mouse, and size of papillomas. Injection of methylnitrosourea i.p. into transgenic mice induced papillomas in the forestomach after 12 weeks (2 to 12 papillomas/mouse) at 100% incidence (K. Ando et al., Cancer Res., 52: 978-982, 1992). Administration of azatyrosine i.p. at a dose of 2 mg/mouse once every 2 days for 12 weeks after initiation with methylnitrosourea completely prevented the formation of forestomach papillomas. These results clearly indicated that azatyrosine inhibits chemical carcinogenesis in vivo.

Response characteristics of the BVP neuron model to periodic pulse inputs.

The characteristics of the BVP neuron model response to periodic pulse stimuli are investigated. Temporal patterns of the output of the model are analyzed as a function of the stimulus intensity and period. The BVP model exhibits the same chaotic behavior, and a Cantor function-like graph of the response frequency (mean firing rate) as in electrophysiological experiments. This shows that the BVP model describes the complicated response characteristics of the neuron at least qualitatively.

Efficacy of a new quinolone, levofloxacin in patients with surgical infections.

The pharmacokinetics and clinical efficacy of levofloxacin (LVFX, DR-3355), an optically pure S(-)-enantiomer of ofloxacin, were studied in patients after surgery. In the pharmacokinetic study, 4 patients undergoing bile drainage were given 2 100-mg tablets by mouth. Peak levels of LVFX were from 2.22 to 4.02 micrograms/ml of plasma at 2-4 hours after the oral administration, and from 7.5 to 11.3 micrograms/ml of bile at 2-6 hours. Forty-three patients with surgical infections, including 16 skin and soft-tissue infections and 12 wound infections, were treated with LVFX. Twenty-eight (70%) of the 40 patients whose results could be evaluated had excellent or good results; 42 (93%) of the 45 causative organisms identified were eradicated. An episode of diarrhea with chills and fever occurred in a 38-year-old man. The results suggested that LVFX has satisfactory antimicrobial effects in surgical infections.

[Extrapyramidal manifestations in hereditary olivopontocerebellar atrophy--clinical study of 10 cases in three affected pedigrees].

We studied 3 pedigrees affected with autosomal dominant olivopontocerebellar atrophy (OPCA), and clinical features of 10 patients were reported. Clinically, 6 cases were dominated with cerebellar ataxia, one with Parkinsonism, and three with choreiform movements. Furthermore, they were characterized by slow eye movement and progressive loss of tendon reflexes. Generally, extrapyramidal manifestations, such as Parkinsonism or choreiform movements, are occasionally observed in dominant OPCA. However, the pathogeneses are different from each other. The choreiform movements are unique in dominant OPCA, and generally not observed in sporadic OPCA. In the distribution of degenerated foci, dominant OPCA is often associated with degeneration of not only substantia nigra, but also with other structures such as dentate nucleus, red nucleus, external segment of globus pallidus (GPe), and subthalamic nucleus. This dentatorubral and pallido-Luysian system degeneration are parts of neuropathological findings in hereditary DRPLA (Naito-Oyanagi form). In both disorders, choreiform movements is observed. Current theory indicates that hyperactivity of neostriatal dopamine neurons and degeneration of GPe play roles in the pathogenesis of chorea. Clinically, choreiform movements are common in hereditary DRPLA, but not in dominant OPCA. This difference could be explained by the nigral degeneration in dominant OPCA, which suppress the generation of choreiform movements.

Expression of alpha-amylase isozymes in human thyroid tissues.

Expression of alpha-amylase genes in thyroid tissues was studied by assaying the total amylase activity as well as by using immunohistochemical and Northern blot analysis. The amylase genes expressed were determined by a combination of the polymerase chain reaction (PCR) and blot analysis using synthetic probes specific for the three known amylase isozyme complementary DNAs. The samples consisted of tissues from 18 human thyroid carcinomas (11 well-differentiated carcinomas, 2 poorly differentiated carcinomas, 1 anaplastic carcinoma, and 4 medullary carcinomas) and 9 specimens of nonmalignant thyroid tissue (2 were from nontumorous regions of resected glands and 7 were thyroid tissue from a patient with Graves' disease). Salivary-type amylase was expressed at a relatively high level in nonmalignant thyroid tissue and well-differentiated carcinoma and could be detected by Northern blot analysis. In poorly differentiated carcinoma, it was detected only by the PCR, while in anaplastic or medullary carcinoma, it was not detected even by the PCR. Thus, the expression of salivary-type amylase was characteristic of well-differentiated follicular cells. These observations suggest that salivary-type amylase expression may be a marker for identifying the histogenesis and stage of differentiation of thyroid cancer. In addition, the AMY2B gene product was detected in all different types of cells examined, although its expression was only detectable by the PCR. Pancreatic type amylase was not detected in any of the samples.

Salivary fibronectin in man: an immunoblotting, radioimmunoassay and immunohistochemical study.

An antiserum and monoclonal antibody against plasma fibronectin recognized a 230-kDa, intact fibronectin molecule when 1000-fold diluted human plasma was subjected to immunoblot analysis. Immunoblot analysis of the parotid saliva demonstrated that the rabbit antiserum to human plasma fibronectin bound to five molecules (200, 110, 85, 75 and 65 kDa), other than the 230-kDa, intact fibronectin molecule, while the mouse monoclonal antibody recognized only the 230-kDa molecule. The 230-kDa type was not found in whole saliva with either the antiserum or monoclonal antibody. The antiserum reacted with 85-, 75- and 33-kDa molecules, and the monoclonal antibody recognized 75-, 33- and 20-kDa molecules in the whole saliva. Radioimmunoassay revealed that the mean +/- SD of fibronectin concentration in parotid saliva was 2.5 +/- 1.4 ng/ml (n = 20) and 149.8 +/- 46.2 ng/ml (n = 30) for whole saliva. Immunoperoxidase staining with rabbit antiserum and the mouse monoclonal antibody showed positive cytoplasmic staining of cells in the intralobular and interlobular ducts in parotid, submandibular, and sublingual glands. No acinar cells were stained.

Immunohistochemical analysis of amylase isoenzymes in thyroid cancer.

The expression of amylase in various histological types of thyroid cancer was studied by an immunohistochemical technique, using a polyclonal antiamylase antiserum and two monoclonal antibodies specific for salivary and pancreatic-type amylases, respectively. Amylase was expressed in 21 of 24 (88%) thyroid cancers by polyclonal antiserum analysis. Analysis by monoclonal antibodies, however, showed that only 13 (54%) cases and three (13%) cases contained salivary-type and pancreatic-type amylases, respectively. Moreover, immunoreactivity for pancreatic-type amylase was detected only in medullary carcinoma; other histological types were positive for salivary-type amylase. These results show that thyroid cancer frequently expresses amylase, and suggest that the differences between amylase isoenzymes in thyroid cancer may correlate with those found between cellular origin of tumour.

Psychiatric disorders in children and adolescents: the contribution of the child's temperament and the parents' psychopathology and attitudes.

Correlates of child psychopathology were studied in a community sample of 210 children and adolescents (with equal numbers of boys and girls). Children suffering from a psychiatric disorder had more temperamental difficulties and their parents showed a higher level of psychopathology than those without a disorder. Furthermore, a regression analysis revealed that difficult temperamental traits in the child and the parents' attitudes toward the child were primary predictors of psychiatric disorders in the child. The relationship between the parent and child as well as the environment-temperament impact on child psychopathology are discussed.

Further study on the 1,4-alpha-transglucosylation of rubusoside, a sweet steviol-bisglucoside from Rubus suavissimus.

Rubusoside (the beta-D-glucosyl ester of 13-O-beta-D-glucosyl-steviol), which is the major sweet principle of leaves of Rubus suavissimus S. Lee, was subjected to 1,4-alpha-transglucosylation by the cyclodextringlucanotransferase-starch system (the CGTase system). The tri- and tetra-glucosylated products were isolated together with the mono- and di-glucosylated products, which had already been isolated. A prominent increase in intensity of the sweetness was observed for the compounds which were di- and tri-glucosylated at the 13-O-glucosyl moiety. This result further substantiated the structure-sweetness relationship for 1,4-alpha-glucosylated compounds of steviol-glycosides reported previously. For protection of the 19-COO-glucosyl moiety against glucosylation by the CGTase system, the 4-hydroxyl group of the 19-COO-glucosyl moiety was beta-galactosylated by the beta-galactosidase-lactose system. This galactosylated compound was subjected to a regio-selective glucosylation of the 13-O-glucosyl moiety by the CGTase system, which was followed by enzymic elimination of the galactosyl group to furnish an exclusive preparation of the improved sweeteners just mentioned.

[Chronic inflammatory demyelinating polyradiculoneuropathy associated with multifocal nerve hypertrophy--report of a case with MRI study].

We reported a 29-year-old woman who had chronic inflammatory demyelinating polyradiculoneuropathy (CIDP) associated with multifocal nerve hypertrophy. She developed chronic progressive muscular weakness of distal part of her extremities and sensory ataxia of limbs and trunk for five years. Steroid therapy improved most of her signs and symptoms. No recurrence has occurred for the subsequent four years until present time. Magnetic resonance imaging (MRI) revealed multifocal and nodular hypertrophy of her peripheral nerves in the extremities. MRI also detected the hypertrophy of posterior nerve ganglia and extradural nerve roots. Hypertrophic nerve trunk comprised of multifocal and nodular hypertrophy of the nerve fiber fascicles which were detected as abnormal high signal intensity area in MR T2-weighted images. Her sural nerve contained no onion-bulb formations and presented normal low signal in MR T2-weighted images. We concluded that the hypertrophic changes occurred multifocally in and among nerve fascicles predominantly at the proximal part of the nerves of the extremities. MRI is useful method to detect the hypertrophic change of peripheral nerves deep in the trunk and the extremities in CIDP.

Chemically induced tumors in transgenic mice carrying prototype human c-Ha-ras genes.

Three independent transgenic mouse lines carrying human prototype c-Ha-ras genes were established. Approximately 50% of the transgenic offspring of these lines developed spontaneous tumors within 18 months. Types of tumors were restricted to angiosarcomas, skin papillomas and lung or Harderian adenocarcinomas. Interestingly, all angiosarcomas (16/16) had point mutations at the 61st codon of the transgenes. Furthermore, they were also very susceptible to the chemical carcinogens, N-methyl-N'-nitrosourea (MNU) and dimethyl-benzanthracene (DMBA). Within 12 weeks after administration of MNU, the transgenic mice developed forestomach papillomas and then carcinomas very frequently, at the rate of almost 100% and almost all of the tumors had point mutations in their transgenes at the 12th codon from GGC (Gly) to GAC (Asp). All the carcinomas of the forestomach, lung and spleen induced by DMBA had point mutations at the 61st codon from CAG (Gln) to CTG (Leu). Because no somatic point mutations in the transgenes have ever been detected in normal tissues of the affected mice, these mutations seemingly activated the human prototype c-Ha-ras transgene. From these results, it is suggested that the somatic mutation of the human c-Ha-ras transgene plays a causative role in the occurrence of natural tumors and those induced by MNU or DMBA administration in transgenic mice. This transgenic mouse provides a unique screening system for chemicals that induce or suppress the tumorigenesis.

[Studies of free theophylline in asthmatic patients].

Total and free theophylline concentrations were examined by fluorescent immunoassay using serum from 48 asthmatic patients treated with sustained-release theophylline formulation. Total and free theophylline concentrations showed a good correlation. The mean % free theophylline (free theophylline concentration X 100/total theophylline concentration) was 44.7% in children and 43.6% in adults. The % free theophylline had no relation to age of the patients or total theophylline concentration. However, the % free theophylline increased correlatively when serum pH or albumin was decreased. As changes in free theophylline concentration influence therapeutic responses and side effects, an assessment of serum pH and albumin in patients treated with theophylline is necessary.

Expression of pancreatic secretory trypsin inhibitor gene in human colorectal tumor.

Expression of pancreatic secretory trypsin inhibitor (PSTI) gene was examined by Northern blotting analyses in 31 human colorectal tumors that included two benign adenomas and 26 adenocarcinomas. Among the total of 28 cases which proved to be adequate for mRNA analyses, all but one showed the expression of PSTI at various levels. In contrast, PSTI expression was not detected in two malignant lymphomas of the rectum. The level of PSTI expression was not correlated with the patient's age, sex, tumor location or size, stage of differentiation, lymph node metastasis, or progression stage. Some colorectal adenocarcinomas were also shown to express genes that can hybridize with human trypsinogen cDNA probe. It looks as though in these tumors, a protease(s) and its inhibitor are produced simultaneously as part of a cellular self-defense mechanism.
