The types and timing of the degenerative changes seen in the cysts during and after benzimidazole treatment of cystic echinococcosis.

In a large-scale Bulgarian study, 122 patients with abdominal and/or lung echinococcosis were randomly selected for treatment with albendazole or mebendazole. The main aims were to evaluate the effect of each drug on the hydatid cysts, to follow the changes in cyst morphology during and after treatment, and to determine how quickly each drug produced the first, detectable, degenerative changes in the cysts. Follow-up was based on periodic ultrasonography, chest radiography and computed tomography. The abdominal cysts were categorised as small (<5 cm in diameter) or large. As albendazole treatment had effects that were almost identical to those of mebendazole treatment, the results for the two drugs were combined. In the lungs and, particularly, in the abdomen, the size of the cysts being treated influenced the character and timing of the degenerative changes seen in them. The initial change seen in each abdominal cyst was detachment of the endocyst, which occurred 1-3 months (small cysts) or 2-5 months (large cysts) after the initiation of treatment (P<0.05). The abdominal cysts then developed a hyper-echoic/hyper-dense appearance, became smaller, and finally disappeared 3.3-9.3 months (small cysts) or 5.6-13.9 months (large cysts) after treatment began (P<0.05). The first degenerative change noted in the lung cysts was cyst rupture, which occurred as early as day 10 of therapy but was generally observed 1 or 2 months after treatment began. After their complete evacuation, the ruptured lung cysts shrank and became deformed, some disappearing within 5-9 months of the initiation of treatment. The degenerative changes recorded, which began significantly earlier in the lung cysts than in abdominal cysts, indicate serious damage to the cysts and the parasiticidal, curative effect of each of the two benzimidazoles employed.

Islet amyloid polypeptide in Psammomys obesus: lack of correlation between insulin resistance and plasma IAPP levels.

Sand rats (Psammomys obesus) develop insulin resistance and diabetes when given a standard laboratory diet. We have examined the presence of islet amyloid polypeptide (IAPP), or amylin, in these animals. IAPP is present in the beta-cells of pancreatic islets, its localization and intensity of immuno-reactivity are, thus, similar to that of insulin. In insulin resistant Psammomys obesus, insulin and IAPP immuno-reactivity in pancreas are markedly decreased, plasma insulin levels are 20-fold higher than in controls but IAPP levels are not different from those of normal animals. Therefore, insulin resistance in Psammomys obesus does not appear to result from an elevated plasma IAPP level nor from development of amyloid deposits.

[Morphological changes in the pancreas after ligation of the excretory duct].

The rabbit pancreatic structure at histological and ultrastructural level, in different intervals after ligature of pancreatic duct, was studied. Changes in the exo- and endocrine parenchyma were observed. Canalicular structures and conjunctive tissue replace the destroyed acinar cells. In some of the islets the B cells are degranulated or necrotic. Some islets contain infiltrations of lymphocytes, macrophages and granulocytes. Cavities formed by necrotic endocrine cells could appear. The A and B cells proliferation is observed in the new-formed clusters of endocrine cells and ductless. The mechanism of this process is similar to that of the islet formation during the embryonic development or in nesidioblastosis, but they are never transformed in the definitive islet of Langerhans.

Replication of the pseudorabies virus in cultured pancreatic islet cells: further evidence of their paraneuronal nature.

Pancreatic B cells are known to be damaged by a wide range of viruses, causing diabetes. Though these viruses belong to different taxonomic groups, their single shared characteristic is neurotropism. In the present study, pseudorabies viral infection was modelled on fetal porcine islets cultivated in vitro. It was demonstrated that the endocrine cells of the pancreas, especially B cells, were infected in vitro and so served as a medium for the replication of the virus. All stages of the morphogenesis of the virus were observed ultrastructurally within the cells. The exocrine cells located close to the endocrine ones were free from attachment and invasion of the virus. The potential of the pseudorabies virus to develop within pancreatic endocrine cells is regarded as evidence of the paraneuronal nature of these cells.

[Combined ultrastructural and immunocytochemical studies of pancreatic endocrine tumors].

9 surgically removed endocrine tumours of pancreas of female patients aged from 19 to 71 years were studied. The material was stained with hematoxylin and eosin, by Grimelius and Fontana--Masson; PAP method with polyclonal antibodies to insulin, glucagon and somatostatin was also used. Immunoreactivity was weaker in tumours with clinically pronounced hormonal activity as compared to the normal endocrine cells in the Langerhans islets. This is confirmed ultrastructurally: tumour cells contain comparatively low number of neuroendocrine granules. Functionally inactive tumours show the opposite immunohistochemical results. The multipotential properties of cells forming pancreatic endocrine tumours are discussed.

[Multiple endocrine adenomatosis manifested chiefly by hyperparathyroidism].

A case with multiple endocrine neoplasia was reported, including parathyroid adenoma, of the main cells of the gland, multiple small adenomas in the tail of pancreas, (cytologically and electron-microscopically determined as A cellular) and light-cellular adenoma of adrenal. The hyperfunction of parathyroid adenoma was manifested with hypercalcemia (3.75 mmol/l), and morphologically--with the multiple calcium metastases in lungs, kidneys and heart, established at necropsy. The cause for the death was the acutely advanced ischemic disease of myocardium, on the background of chronic pyelonephritis and renal insufficiency. The timely diagnosis of such morbid states is concluded to be important for the clinical practice and could lead to the saving of the patients by operative removal of the tumour.

Localization of the pyroantimonate-bound cations in the pancreatic B-cells organelles and their variations.

The heterogeneity of the localization of the pyroantimonate-bound cations in different B cell's organelles of normoglycemic and glucose-treated white rats was investigated. Its different variations are systematically described. It is pointed out that an active or nonactive type of localization may be presumed only after a multifactor statistical analysis. It is supposed the localization's heterogeneity of the reactive product to reflect the functional heterogeneity of the cell organelles during the secretory cycle. The pyroantimonate method is assumed to show not so much the distribution of the respective cations as the dynamic changes of pH taking place in the different compartments of the B cell.
