[Value of biological tests in the early diagnosis of acute steatosis in pregnancy].

Four cases of acute fatty liver of pregnancy occurring over a ten year period are reported. All four patients had a caesarean section and one died postoperatively. The seven children (three twin pregnancies) were all alive. The early clinical signs were unspecific. Jaundice was the only one occurring in all patients. Routine biological tests before the jaundice develops may be of help for a diagnosis early enough to start the treatment in patients with unspecific gastrointestinal or hepatic manifestations, especially the liver function and blood coagulation tests. These latter allow to discord the diagnosis of viral hepatitis and the "Haemolysis-Elevated Liver enzyme concentrations-Low Platelets" (HELLP syndrome) respectively. Indeed, only a liver biopsy can ascertain the diagnosis.

[Luteinized cystic hyperplasia of the ovaries during pregnancy. A case report].

Multiple lutein cyst (MLC) and luteoma are two aspects of ovarian hyperplasia induced by high levels of human chorionic gonadotrophin (HCG). Using a case report of MLC, the authors compare it with luteoma (the anatomoclinical and pathogenic data). Luteoma are more frequent with multipara, whereas MLC are more often seen with paucipara. Maternal virilization can occur in both cases but MLC does not seem to be responsible for fetal virilization. The fetus could be protected by the placenta, thanks to increased aromatase activity and sex binding globulin production. MLC and luteoma are usually found by chance. The pathology must be known, because of their spontaneous regression after delivery. So, the attitude will be conservative under the control of biopsies.

New autosomal recessive chondrodysplasia--pseudohermaphrodism syndrome.

Two siblings with a previously undescribed syndrome are presented. They both have severe dwarfism, antenatal in origin, with generalized chondrodysplasia, severe microcephaly with cerebellar vermis hypoplasia, a hypoplastic iris and a papillous coloboma (Coloboma of the optic disc). The first sibling has a 46,XY karyotype despite normal female internal and external genitalia. She has moderate mental retardation. Gestation of the second sibling was interrupted after antenatal diagnosis. The fetus was 46,XX and very similar to the first case.

[Respiratory distress syndrome in the adult and pregnancy. Hemodynamic consequences].

RDS in adults is an unusual complication of pregnancy. We analysed a case of adult respiratory distress syndrome during pregnancy and assessed the haemodynamic data under controlled ventilation whilst a spontaneous delivery was carried out. The rapid drop in pulmonary artery pressure and in the QS-QT complex after the delivery suggests that there may be a placental shunt which gives a too high reading for exaggeration of the left to right intrapulmonary shunt figures obtained.

[Retrocervical cystic hygroma: diagnosis, prognosis and management. A series of 13 cases].

Fetal cystic hygroma (CH) are congenital malformation of the lymphatic system which are seen in antenatally ultrasonography from the end of the first trimester of pregnancy. From a 13 CH retrospective study, the authors discuss the ultrasound diagnosis, the prognosis and the management of CH. In our study, CH is associated with lymphedema (69%), hydrops (46%), diminution of amniotic fluid (69%) and other abnormality (31%). In all cases a diminution or an absence of fetal movement are found. In seven cases a cytogenetic analysis are performed on amniocentesis or fetal tissues: there are five 45XO, one 47XX + 13 and one 46XX. In six cases the chromosome culture failed: two male phenotypes and two histologic appearances of chromosomic anomaly. The prognosis depends on chromosome analysis, other ultrasound abnormality and the size of CH. It is usually fatal in utero. If chromosome anomaly are found, a genetic counsel is proposed.

[Stage 1 and 2 cancers of the endometrium. Value of the combination of pelvic radiotherapy and curietherapy followed by a total hysterectomy without lymph node removal].

In a retrospective study of 108 patients with stage I-II EC, treated between 1976 and 1983 at CGFL (Dijon), we performed an analysis of the efficacy and tolerance of a treatment approach consisting of external and intracavitary pelvic irradiation followed, 6 weeks later, by hysterectomy without lymph node dissection. Forty-three patients underwent this treatment sequence (G1) and 65 patients (G2) were treated by other modalities (33 intracavitary irradiation and hysterectomy, 27 hysterectomy alone or followed by pelvic irradiation, 5 irradiation alone). The mean follow-up was seven years (5-12). The five year total actuarial survival rate was 82% for G1 and 74% for G2 (p = NS). The five-year disease-free survival probability was 80% for G1 and 67% for G2 (p less than 0.05). The post-therapy complications were 2% for G1 (one late post-irradiation complication) and 14% for G2 (one late post-irradiation complication and 6 post-surgical complications). Although G1 group has more pejorative clinical and histopathological features (72% T1b or T2 for G1 vs 42% for G2 (p less than 0.005) and 77% grade 2 or 3 for G1 vs 61% for G2 (p = 0.05], the efficacy and tolerance of EIH treatment sequence appear at least comparable to those of G2. These results redefine certain therapeutic aspects of stage I-II endometrial carcinoma which should be considered in future prospective approaches.

[The association of uterine lipoma and cancer of the endometrium. Diagnostic and etiopathogenic approach].

Uterine lipoma (UL) is a rare tumor frequently presenting as leiomyolipoma. Even more uncommon is the association of UL and endometrial carcinoma (EC) for which only two cases have been reported. We present the case of a 73 years old female of French origin complaining of vaginal bleeding. Initial examination under general anesthesia found a 13 cm length uterine cavity with a large tumor located in the anterior wall. After curettage, the histopathologic analysis diagnosed a moderately differentiated adenocarcinoma of the endometrium. Patient work-up showed no evidence of extension outside the uterus (stage Ib). Treatment consisted of pelvic and iliac lymph node external irradiation with subsequent vaginal intracavitary irradiation followed, 6 weeks later, by total extra-fascial hysterectomy without lymph node dissection. Pathologic examination found a small EC with limited infiltration of the myometrium (stage Ia) associated with a 10 x 9 x 7 cm yellowish fibrolipoma type tumor without smooth muscle pattern. First described by Lobstein in 1816, UL are uncommon and casually diagnosed. The association with EC is rare and without evident relationship. Ultrasound (US) and computed tomographic (CT) appearances allow a diagnostic approach. US show a highly echogenic central tumor with a thin moderately echoic rim. CT appearance of UL is a central, well delimited fat-density tumor. The etiopathogenesis of UL is unclear, attributed to a possible leiomyoma evolution for Willen or a myometrial cell metaplasia for Brandfass.

Non-Hodgkin's lymphoma of the urethra: a rare extranodal entity.

We report a rare case of non-Hodgkin's lymphoma of the urethra in a female patient. A review of the literature showed that all such reported cases were described in females, were of varying histologic grade, and presented as a polyp or caruncle. Whether this entity is often localized and has the excellent prognosis of early carcinoma of the urethra will have to be determined by study of more cases.

Infectious pneumoperitoneum as an uncommon presentation of endometrial carcinoma: report of two cases.

Two patients with pneumoperitoneum are reported; in both cases, the cause was severe infection of the upper genital tract. Investigation led to the finding of an underlying endometrial carcinoma. Literature review of the etiology of pneumoperitoneum, nature of the usual infecting organisms, and therapeutic principles are presented. Endometrial carcinoma should be considered in the differential diagnosis of infectious pneumoperitoneum, especially where patient risk factors are present.

[Morphological study of the muscle of Treitz (m. suspensorius duodeni)].

The differences of opinion concerning the morphology of the human suspensory muscle of the duodenum conducted the authors to perform a study upon 31 cadavers including 21 adults and 10 foetuses. In most cases they describe a single muscle arising from the duodeno-jejunal flexure and reaching the right side of the diaphragm oesophageal opening. Microscopic examination confirms that it consists of unstriped muscle fibers except cranially near from his diaphragmatic insertion where striped muscle fibers can be observed.
