Diagnosis and placental management of a viable term abdominal gestation.

The diagnosis of a term viable abdominal gestation is difficult to make. The use of modern diagnostic tools, especially ultrasound, can make this diagnosis easier. The management of the placenta following delivery at laparotomy is crucial. If there is no placental bleeding, the placenta should be left in situ to resorb postoperatively.

Colonization of teeth in humans by Streptococcus mutans as related to its concentration in saliva and host age.

The relationship of the salivary concentration of Streptococcus mutans and host age to the colonization of this organism on erupting teeth was studied in humans. Plaques were obtained from fissures and buccal surfaces of erupting permanent first and second molars of children 6 to 7 and 11 to 14 years old, respectively. In subjects of both age groups with salivary S. mutans concentrations below 5 x 10(2) colony-forming units per ml, the organism was detected on only a few of the tooth surfaces; at concentrations of 5 x 10(2) to 4.9 x 10(4) colony-forming units per ml more than half of the surfaces and at concentrations of 5 x 10(4) colony-forming units per ml or higher most of the surfaces were colonized by S. mutans. The frequency of detection and concentration of S. mutans in plaque as well as its concentration in saliva were higher in the case of the older children. However, when younger and older children with similar salivary S. mutans concentrations were compared, S. mutans was more frequently isolated from plaque from older children only in the case of children with below 5 x 10(2) colony-forming units per ml of saliva. Eleven of the 64 children studied had low or undetectable S. mutans levels in plaque and saliva. The salivary S. mutans levels of the parents of these children were lower than those of parents of a group of children with normal oral S. mutans levels.

Cardiac performance during pregnancy: serial evaluation using external systolic time intervals.

Indirect systolic time indices were used to evaluate serial changes in left ventricular function during pregnancy in 13 normal patients and five patients with compensated cardiac conditions. Changes in both groups tended to parallel each other. In both groups, serial LVET index decreased, PEP index increased, and PEP/LVET ratio increased. Q-S2 index decreased in the normal group in midpregnancy and was inconsistent in the cardiac group. In the immediate postpartum period, LVET index returned to baseline values but PEP and PEP/LVET remained increased in both groups. Results of multiple comparison tests demonstrated that the largest (or smallest) value of each systolic time interval during pregnancy was observed in either the periods of the weeks 28 to 34 or 34 to 38. No significant differences were noted in intergroup comparison between the normal and cardiac groups. These data suggest that consistent changes in cardiac function may be determined during pregnancy by the noninvasive technique of indirect systolic time interval evaluation. These changes may reflect a change in left ventricular contractility, preload or afterload. The differences may persist even in the immediate postpartum period when cardiac output and heart rate have decreased to normal, suggesting residual changes in intrinsic left ventricular function. In patients with compensated heart disease, indirect tests of left ventricular function generally reflect the changes in normal pregnant women.

Posters as an educational strategy.

Posters are visual aids that are well suited to use as independent sources of information or as support for other presentation formats. By design, the message displayed is brief, constant, and interactive with the viewer. Guidelines for developing a poster include careful delineation of content, knowledge of audience needs, and the environment or setting for the poster. The application of basic design elements, such as simplicity of composition, attractive color combinations, and title spacings, results in a presentation mode that is both attracting and lingering.
