[Secondary aorto-enteric fistula].

On the basis of five case histories, the causes, symptoms, investigations, diagnosis and treatment of secondary aorto-enteric fistulae are reviewed. In a patient in whom reconstruction in the aortoiliac segment has been undertaken, recurrent haematemesis and melaena, pyrexia and septicaemia and abdominal pain should lead to investigation for aorto-enteric fistula by means of oesophagogastro-duodenoscopy and radiological investigations if the condition permits this. Otherwise, emergency laporotomy with thorough examination of the intestine should be undertaken. The condition is associated with a considerable mortality but the prognosis is improved by rapid intervention. In cases where aorto-enteric fistula is suspected, the patient should be referred to a department for vascular surgery.

Major vascular injury during gynecologic laparoscopy. Report of a case and review of published cases.

Serious complications following gynecologic laparoscopy are rare. We report a case of a 24-year-old woman who had a common iliac artery and vein injured by the trocar, resulting in deep shock. The arterial lesion was repaired with an allograft and the vein sutured. The patient recovered with light sequelae. The literature was reviewed and we found 15 cases described in varying detail. In most cases the injury was caused by the pneumoperitoneum-needle and in only 2 by the trocar. Characteristically the terminal aorta and its major branches were injured. All the patients had a large retroperitoneal hematoma and only minimal free blood in the peritoneum. Signs of bleeding were nearly always acute. Most lesions could be treated by simple suture. Recovery is the rule, but 2 of the 15 died. Examiner's experience and correct technique are important to prevent this potentially lethal complication which is also probably underreported.

Prediction of post-concussional sequelae by reaction time test.

The aim of this study was to examine whether it was possible by a reaction time (RT) test to predict if a patient was to complain of sequelae after a cerebral concussion. 55 patients had an RT test performed 1-5 days after concussion. The test showed a significant (P less than 0.02) prolongated RT in patients having postconcussional sequelae (median 186 ms) after 1 month (27 patients, 49%) compared to the patients without sequelae (median 168 ms). Patients with sequelae after 3 months (12 patients, 22%) also had a prolongation of RT (median 183 ms) although non-significant. These results supports a structural organic etiology for postconcussional sequelae.

Repetitive bleeding from a pheochromocytoma presenting as an abdominal emergency. Case report.

Recurrent abdominal pain was the only subjective manifestation in a case of pheochromocytoma with retroperitoneal bleeding. At emergency laparotomy the tumor, showing signs of fresh and earlier bleeding, was extirpated. Sinus-type tachycardia was treated with beta-blockade peroperatively, while the diagnosis was still obscure, but hypertension did not follow. Meta-oxedrine and dopamine infusion was continued for 48 hours to sustain the blood pressure. Recovery was uneventful.

Arterial injuries. Review of 12 years experience with 32 patients.

Thirty-two patients with 41 injured arteries were operated upon during a period of 12 years. Three quarters were men, and the median age was 33 years. Accompanying lesions were seen in 87.5% of the patients. Nearly half the lesions were related to blunt trauma. Occupational accidents constituted the largest group. The diagnosis was made on clinical grounds in most cases. The median time lapse from trauma to operation was 5 hours. The principles of treatment were those that are generally accepted. No patients died, the amputation rate was 12.5%. Median follow-up was 8 months. A successful arterial reconstruction contributed to a good functional result. Postreconstruction thrombosis of arteries were found to occur within the first hours or days after reconstruction. Late thrombosis was rare and the overall long-time patency was 61.4%. Patency of repair of upper limb small vessels was 54.5%.
